SOW Design and Content
1.0 Introduction
The VA Maine HCS requires eleven (11) Midmark Chairs for the Orthopedic, Wound, and Dermatology departments in the Augusta, Maine location.
2.0 Background
Our goal in the VA is to provide veterans the world-class benefits and services they have earned and to do so by adhering to the highest standards of compassion, commitment, excellence, professionalism, integrity, accountability, and stewardship. In on order to accomplish this and provide exceptional care, Surgical Service has many departments that are in need of updating to proper exam tables and chairs that are specialty specific. This allows for the comfort of our aging population while undergoing various forms of evaluations and procedures.
3.0 Scope (Provide an overview of the PWS emphasizing the most important aspects of your requirement and defining the boundaries of the contractor’s performance responsibilities).
Safety is by far the biggest concern when dealing with operating room/clinic equipment and Surgical Services always looks to make sure we are providing the latest and safest equipment to provide the standard of care that is owed to our veterans. The chairs that we are requesting provide many benefits such as lower seat heights, motion profiling, as well as features and accessories that promote proper patient positioning that work together to help improve the patient and provider experience. I do have to mention that the proper positioning of patients also protects from staff injuries on trying to accommodate to deliver care. The Midmark line ensures that both veterans and staff are positioned properly so that outstanding care can be provided and increasing the likeliness of the veteran’s confidence in the VA system.
4.0 Applicable Directives (List all documents referenced in the next section 5.0, Performance Requirements, with which the contractor will have to comply in performance of the contract effort. If only portions of a given document apply, specifically identify the applicable portions).
The system must meet the manufacturer’s specifications and be designed and implemented within the vendors 510K system design as an FDA regulated medical device/system.
5.0 Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks).
No additional requirements for these units. There is no installation.
6.0 Deliverables (List all data required to manage or monitor contractor performance).
This project will include all and any peripheral items to ensure a fully functional system. (misc. hardware or req. hardware upgrades, network switches, video cards, KVM switches, cabling, mouse, keyboards etc.) to ensure the functionality of the final product.
Midmark will provide one year of part and labor for the purchased chairs. No training is necessary for this purchase.
