PAST PERFORMANCE QUESTIONNAIRE

(Offeror to complete items II A. and II B. prior to issuing this questionnaire to the Evaluator.)

In order to assess potential vendors under the MCRC Web Proxy Server procurement, the Marine Corps Regional Contracting Office (RCO) is obtaining past performance information with respect to each Offeror. In addition to this questionnaire, you may receive a follow-up phone call to confirm or clarify information.

This is a critical acquisition to support the Marine Corps Recruiting Command , and your support is sincerely appreciated. We thank you in advance for your time, effort, and cooperation in responding to this questionnaire.

The completed Questionnaire shall be submitted directly via email from the customer reference to Denise M. Hill at denise.hill@usmc.mil.

By the due date established for receipt of offers. The “subject” line in the submission email shall clearly indicate: “MCRC Non-cognitive Testing RFQ Past Performance” and the message shall originate from the Evaluator’s corporate or government email system.

The Offeror's past performance could have been improved. The Offeror required major agency resources to ensure achievement of contract requirements.
Unsatisfactory
The Offeror's past performance did not meet customer expectations and/or contract requirements.
