3. Description of Supplies and Services to Include Contract Vehicle/Number Utilized:

This requirement is for DHIS Management and Consulting Services Task Order 75D30122F15055. Modification 00002 is to increase level of effort for Task 4.4.4 and move Task 4.4.5 to an optional task for base and option periods.

The estimated value of the proposed action, including all options, is $1�_____________ � The level of effort needed to meet the deliverables of Task 4.4.4 is higher than anticipated from when the Task Order was awarded. The Modification 00002 is to add additional level of effort for Task 4.4.4 for base and option periods for Public Health Support Services providing statistical models and interpretation of analysis and studies. The Modification 00002 will also make the Task 4.4.5 an optional task for base and option periods. This change in level of effort is needed as soon as possible in the base period starting in January 2023 until the base period ends on September 15, 2023. The increased scope will apply for the base and all option periods. There are three (3) option periods of twelve (12) months each.

□ ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title Ill of the Federal Property and

Administrative Services Act of 1 94 9 (41 U.S.C. 251, et seq.); Title 40 U.S.C. 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception and for each discuss how the cited authority applies in accordance with FAR 8.405-6(c)(2)(iv).
