ATTACHMENT 1
AGGREGATE BASE COURSE SUPPLY
MILL CANYON ROAD
Statement of Work and Technical Specifications
1. GENERAL CONDITIONS
1.1. Summary of Work – The work covered by this contract consists of the furnishing 2,460 tons of aggregate base course material delivered to Mill Canyon Road located in Grand County, Utah.
1.2. Project Location – The Mill Canyon Road is located off the west side of Hwy. 191 approximately 14 miles north of the town of Moab, Utah. The stretch of road where the material is to be placed begins approximately 0.26 miles from the highway turnoff and extends for approximately 0.70 miles in a westerly direction. (See attached location map)
1.3. Aggregate Source Requirements – The aggregate materials furnished under this contract shall be obtained from a commercial sand and gravel operation holding a current operating permit/license issued by the state in which the pit is located.
1.4. Submittals – Upon award, Contract shall submit a copy of a laboratory analysis report demonstrating that the material source complies with the specific material requirements found in Section 2 below. The laboratory report shall be prepared by a third party testing agency qualified to perform such work.
1.5. Measurement – Material shall be measured in tons. A printed weight ticket showing the date, type of material, and net weight shall be provided for every load. Contractor shall provide one copy of each weight ticket at the time of delivery and attach one copy of each ticket to invoice submitted for payment.
1.6. Payment – This is a lump sum contract. Payment shall be made for the contract quantity of material delivered to the project site in conformance with this specification.
Contract price shall include all costs associated furnishing the material, including but not limited to labor, equipment, permits, profit and overhead.
1.7. Quantities – Contractor shall be responsible for tracking the quantity of material delivered to the site over the course of the project. No payment will be made for material that is in excess of the contract quantity unless such extra material is delivered at the request of the Government and a modification to the contract is approved in advance by the Contracting Officer.
2. MATERIAL REQUIREMENTS
2.1. Aggregate Base Course: Clean, hard fragments or particles of crushed stone, or crushed or natural gravel, and natural or crushed sand. Materials that break up due to freeze-thaw or wet-dry cycling shall not be used.

2.1.1. Fraction passing No. 40 sieve shall have a liquid limit not greater than 25, and a plasticity index not greater than 6, as determined by AASHTO T89 and T90, respectively.
2.1.2. Material shall meet the following sieve analysis by weight:

3. DELIVERY REQUIREMENTS
3.1. Equipment: Material may be delivered by belly-dump or end-dump type of truck at the discretion of the Contractor. Tandems or trains are permitted.
3.2. Placement: Material is to be deposited at the locations specified by government representative. Material will be spread and compacted by others.
3.3. Rate of Delivery: Material shall be delivered at a steady rate between 350 tons/day and 700 tons/day unless otherwise agreed to by both parties.
3.4. Schedule: Delivery shall occur between the dates of April 13, 2015 and April 30, 2015.
Once delivery commences, the work shall be continued at a steady rate until completed.
Delivery days shall be limited to Monday – Thursday, 7:00 am to 5:30 pm unless otherwise arranged in advance with the Contracting Officer’s Representative.
