STATEMENT OF WORK
PROJECT NUMBER KELL 14-0001
GENERAL FACILITY RENOVATIONS A/5/8/9, ROOMS 210, BLDG 2007
JBSA LACKLAND, TX

1. GENERAL DESCRIPTION:
1.1. The contractor shall provide all labor, materials, supplies, tools, equipment, supervision , quality assurance, incidentals and all other items needed and necessary for the general facility renovations of A/5/8/9, rooms 210, 210A, 210B, 210D, 210E and 210F in the Policy, Plans & Assessment section of BLDG 2007. All work shall be accomplished in accordance with all applicable codes and standards, compliant with OSHA and UFC’s requirements, applicable standards of the National Fire Protection Association (NFPA), National Electrical Code and in accordance with JBSA Lackland general requirements. The contractor shall accomplish all tasks incidental to the general facility renovations of A/5/8/9, room 210-210F in B-2007 as necessary to provide a safe, fully functional, completely finished and operating system ready for immediate use by the Government.
1.2. Building 2007 is located on JBSA Lackland, TX (Security Hill). All work associated with the renovation shall be coordinated with the Government to mitigate any potential impacts to current Air Force missions. This project is located within a secured area and all personnel and construction work is subject to security and other restrictions. No electronic transmitting devices, phones, recorders or cameras are allowed within the facility. Escorts will be required and provided by the Government. All work shall be coordinated with the 502 CONS Contracting Officer/Administrator and the 502 CES COTR/Project Manager. All construction materials, components and debris removed under this contract shall become the property of the contractor for proper disposal, unless otherwise stated within this SOW.
2. WORK TO BE ACCOMPLISHED:
2.1. All construction work shall be in strict conformance with design drawings, specifications and shall include the furnishing of all materials, labor, plant, tools, equipment, and services necessary to completely accomplish the provisions of this SOW. The work performed by the contractor shall result in a complete and fully functioning facility and systems consisting of, but not limited to: Removing, transporting and storing all office furnishings and equipment to warehouse 2089 (a Government storage facility) prior to construction. Removing current wall paper/finishes and retexturing, finishing and paint all walls. Reconfigure lighting layout to include installation of new receptacles, communication data outlets, conduit and wires, install new lighting fixtures, switches and plates. Reconfigure HVAC system to include some ductwork, install new diffusers and grills and assure system will maintain adequate temperature.
Refinish cabinets and counters to include replacement of hardware as needed and install new pre-engineered counter tops. Refinish doors and install new ceiling tiles, flooring and paint walls to create a new finish. Returning, transporting and setting up of all previously stored office furnishings and equipment. Once project is completed all items to including furniture and
SOW General Facility Renovations A/5/8/9, Room 210, BLDG 2007 Page 2 of 22 equipment layout, electrical, HVAC and communication systems should be fully operating, functioning, current to industrial standards and ready for use immediate use by the Government.
2.2. The work shall mean to provide, install and make all connections for a completely finished, operating, functioning HVAC and electrical system; including communication telephone, computer network and NIPRNET, SIPRNET, JWICS and NSTS systems current to industrial standards and ready for use by the Government.
2.3. The contractor shall secure all office furniture and equipment and prep for moving.
Contractor shall remove all office furniture and equipment from its current location in building 2007 and transport to building 2089 and store furniture and equipment in warehouse. After project is completed, the contractor shall return all office furniture and equipment from building 2089 to building 2007. The contractor shall set up all furniture and equipment in designated rooms and assure operability. Remove all storage and package material and clean furniture as needed.
2.4. The contractor shall make way for construction and finishes and accomplish the demolition work necessary to make all structural, mechanical, architectural and electrical connections within construction area.
2.5. The contractor shall remove and demo all existing carpet, base and cove base throughout the construction area. The contractor shall provide and install all new all floor carpet, base and cove base throughout the construction area. Carpet style and color will be base standards selected by the customer.
2.6. The contractor shall demo all surface mounted conduit and wire mold, to include miscellaneous data/communication outlets, conduits and wire mold exposed on walls, floors or ceiling throughout the construction area. All miscellaneous data/communication cables shall be rerouted and flushed mounted within walls. No surface mounted electrical/communication hardware, devices, associated boxes, conduits, raceways, wire mold, cable or wiring will be left when project is completed.
2.7. The contractor shall remove and re-install existing alert notification strobes (amber in color) in construction areas. Locations of alert strobe are approximations only. The contractor shall properly locate all alert strobe so as to avoid interference with lighting fixtures, ceiling air diffuses, sprinkler heads, or other structural features. Alert strobe will be flush mounted.
2.8. The contractor shall remove and re-install existing ceiling speakers. Locations of speakers are approximations only. The contractor shall properly re-locate all speakers so as to avoid interference with lighting fixtures, ceiling air diffuses, sprinkler heads, or other structural features.
2.9. The contractor shall demo all existing 2’ X 4’fluorescent lighting fixtures, down lights and exit lights throughout construction project areas.
2.10. The contractor shall provide, install and connect new 2’X4’, recessed lay-in grid type, fluorescent lighting fixtures throughout the construction areas. Use Lithonia ES8 High- Performance T8 light fixtures.

2.11. The contractor shall provide, install and connect new emergency battery packs to fluorescent lighting fixtures. Battery packs shall be an integral part of fixture. The unit shall consist of a solid-state charger in accordance with the previous wattage requirements. The unit shall automatically supply illumination to one lamp in the event of a power failure. One emergency fixture will be in each office and two emergency fixtures will be install in the conference room and main entry office.
2.12. All recessed lighting fixtures shall be secured with safety wire at two opposing corners.
Down light shall be secured as well with safety wire.
2.13. The contractor shall provide, install and connect new L.E.D. exit light fixtures with battery back-up unit.
2.14. Ceiling speakers shall be reused and mounted in close proximity to original location.
2.15. The contractor shall clean and/or replace air diffusers and air grills in the construction area as necessary.
2.16. The contractor shall remove and demo all wall paper/wall covering throughout construction area.
2.17. The contractor shall prep all surfaces to include any fur outs. Patch and fill all voids and provide and apply new lite texture and make ready for painting. Apply one coat of primer and two coats of paint throughout the project construction area. All paint shall meet all federal specification and all paint colors shall meet Lackland AFB color schemes.
2.18. In the conference room: The contractor shall repair and/or adjust the HVAC system.
Conference room is always too cold. Contractor shall assure temperature and humidity can be controlled correctly in the conference room without negative effects on the remaining system.
The finished HVAC product expectation for the system is to provide a balanced air exchange ratio and provide the customer with a healthy and comfortable working climate throughout the project area.
2.19. In the conference room: The contractor shall remove and demo the door and frame in the back of the conference room, between room 210B and 210D. Fill opening with sound batt insulation/material. Match the existing wall thickness and finishes. The contractor shall provide and install new drywall where door was removed to create a smooth finish.
2.20. In the conference room: The contractor shall demo entire ceiling grid to include decorative boarders and HVAC supply/return grills.
2.21. In the conference room: The contractor shall provide and install new 2’ x 2’ suspended ceiling grid system, ceiling tiles and all other supporting components area as necessary. New ceiling height will be approximately nine feet (9’).
2.22. In the conference room: The contractor shall raise sprinkler heads as necessary to achieved new ceiling height and provide new HVAC supply and return grills.

2.23. In the conference room: The contractor shall refinish/stain all cabinets and counters.
Contractor shall provide and install new pre-engineered type counter tops.
2.24. The contractor shall remove, refinish/stain and reinstall existing wood crown molding, chair railing and baseboards throughout construction area. Contractor to add matching crown molding, railing and baseboard as needed to complete sections.
2.25. In the conference room: The contractor shall remove video camera and screen (camera and screen will remain government property and be given to the government).
2.26. In the conference room: The contractor shall demo all surface mounted wall lighting fixtures (sconces) to include control switches.
2.27. In the conference room: The contractor shall relocate data lines.
2.28. In the conference room: The contractor shall provide, install and connect new fluorescent dimmable down lights controlled by three-way dimmer switches. Switch locations will be at the entryway and north west control corner of the conference room. The contractor also shall provide, install and connect three way switches to control the 2’ X 4’ light fixtures. Switch locations will be entryway and north west control corner.
2.29. The contractor will assure 2’ X 4’ light fixtures are arranged, centered and installed equally spaced to provide uniformed coverage throughout the construction area.
2.30. The contractor shall reconfigure lighting and switching circuits for new lighting layout throughout the construction area and remove all unnecessary switches.
2.31. The contractor shall provide, install and connect a 120V/20A power receptacle at 66” in all office areas and conference room. Install an empty outlet box next to the receptacles. Empty outlets box to consist of 4-11/16” X 4-11/16” boxes with 2” X 4” plaster rings and 1” EMT conduit to be stubbed out 6" above the ceiling with 90-degree bends at the ends, to include pull string and blank cover.
2.32. In the Director’s office: The contractor shall demo upper receptacle, remove and reinstall white board and provide, install and connect dimmer switch to control light fixtures.
2.33. In the Deputy’s office: The contractor shall adjust sprinkler head, remove and reinstall white board (in a different location) and provide, install and connect dimmer switch to control light fixtures.
2.34. The contractor shall refinish/stain or paint all doors in the construction area.
2.35. The contractor shall field verify all existing electrical conditions and equipment, to include the communication system, prior to any construction activities. All items non-operational, in abnormal condition, or broken will be pointed out to the project manager prior to commencement of work.

2.36. The contractor shall remove and replace with new all switches, receptacles, fire audio/visual devices, to include any thermostats and or sensors in the project construction area.
The routing of all circuits shall be confirmed by the contractor at project site. The contractor shall maintain circuit continuity for all circuits whose continuity was disrupted due to construction work. All devices shall be correctly located.
2.37. The contractor shall reuse all existing power/lighting circuits the maximum extent possible for new work installation and requirements; to include fire alarm, alert notification and data/communication circuits.
2.38. Additional circuits. It is the contractor’s responsible to research, verify, validate and provide any and all additional circuits as needed, and to assure potential circuits are feasibly, operably and in accordance to the latest edition of the NEC, UFC’s, NFPA’s and Communication/Data standards.
2.39. The contractor shall provide, install and connect new single pole, 3-way and dimmer light switches.
2.40. In the main office (3ea) and the deputy’s office (2 ea): The contractor shall provide, install and connect new 120V/20A convenient duplex receptacles as per UFC and current edition of the NEC. New power receptacles and circuits are to be recessed in walls, ceilings or columns.
Contractor shall label power circuits on the outside of receptacle cover. All 20-amp circuits shall not have a connected load greater than 16 amperes.
2.41. The contractor must be cleared or monitored to handle classified lines.
2.42. (CS) As needed, the contractor shall provide, install and connect new data, telephone, NIPRNET, SIPRNET, JWICS and NSTS outlets (with all communication circuitry) for system furniture as per 502CS base standards, UFC’s and current to industry standards. New outlets/circuitry are to be recessed in walls, columns and above ceilings.
2.43. (CS) As needed, the contractor shall provide, install and connect new NIPRNET, SIPRNET, JWICSNET and NSA data outlets. The contractor shall pre-wire construction area for NIPNET data network cabling in accordance with Base Communications Policy. The facility shall be supplied with NIPRNET data outlet boxes. Outlet boxes to consist of 4-11/16”X 4- 11/16” with 2” X 4” plaster rings and 1” EMT conduit to be stubbed out 4" with 90-degree bends above the ceiling.
2.44. (CS) As needed, the contractor shall provide, install and connect new data and telephone outlets. All jacks shall be identified I.A.W current base standards. All cable terminations shall be in accordance with TIA/EIA wiring standards. All copper cabling must be tested end-to-end and certified to meet TIA/EIA performance standards. All test results and certifications will be provided in a report, prior to completion of construction project, to the electrical project manager.
2.45. (CS) Telecommunications (Telephone & LAN/Data) Outlets: As needed the contractor shall provide and install NIPRNET, SIPRNET, JWICSNET and NSA communication lines/drops to each room and workstation. Quad outlets will be installed at each telephone & LAN/data
SOW General Facility Renovations A/5/8/9, Room 210, BLDG 2007 Page 6 of 22 location unless specified otherwise. Each quad shall contain 1ea telephone outlets and 1ea LAN/data outlets. Telecommunications outlets shall be Category 5e, RJ45 8-pin voice and data modular outlet assemblies housed in a quad-hole, flush wall-mount faceplate.
2.46. (CS) All terminations shall be done according to T568-B wiring scheme as required by TIA/EIA-568-B. Wall-mount telephone outlets (for wall-mounted telephone sets) shall be 8-pin modular outlet assemblies (type RJ-45 modular wall-telephone jack) mounted in a simplex housing as indicated on the drawings. Each modular outlet shall accept a 4-pair Category 5e cable (8-conductors) and each modular outlet assembly shall be numbered for easy identification and location. All outlets will be wired directly to the Common User Patch Panel located in the Telecommunications Room serving the floor that the outlets are located on.
2.47. (CS) As needed the contractor shall provide/construct: LAN drops that are capable of passing 1000 Mbs data and capable of recording the data; test results to the Base 502 CS and the 690P th P ISS. All wall jacks must be at a minimum, 3 meters (9 feet) apart along the walls in the room per AETC Guidance (First 400 feet) dated 10 Mar 06. All jack labels shall reference the distance end wall plate. All markings shall be IAW the site cable plan. Labels shall be placed under clear plastic label protectors on the wall plates.
2.48. (CS) The contractor shall Tag and Test NIPR, SIPR, JWIC and NSA cables from desktop to Communications room. (502 CS) ETL 02-12 (mandatory for all new construction, renovation, modernization or rehabilitation) states pre-wiring will be included in MILCONs and Minor Construction Projects and AFI 65-601 provides amplification to ETL 02-12.
2.49. Circuits may be combined in existing conduits for the most effective conduit installation, provided all N.E.C. requirements and EMSEC/TEMPEST standards are adhered to. All conduits shall be run concealed within walls, or above ceiling.
2.50. All equipment and metallic surfaces of the electrical and communication wiring system shall be effectively grounded by an equipment-grounding conductor.
2.51. Circuit Directory: Contractor shall identify and mark all circuits on electrical panel directory. To include changes due to electrical demolition, new and existing circuit work. Use a computer or typewriter to create directory; handwritten directories are not acceptable.
2.52. Upon completion of the construction work, the contractor shall restore and clean the project area and return to the Government ready for immediate use.
2.53. There are no hazardous materials associated with this project and a survey is unavailable.
As with all construction projects, the contractor shall immediately contact the Contracting Officer in the unlikely event something other than normal construction condition is discovered.
3. GENERAL PLANNING AND SUBMITTAL REQUIREMENTS:
3.1. The contractor shall be responsible for developing design analysis, plans, and specifications to accomplish the provisions of this SOW. The Contractor shall perform all services required to complete investigations, fabrications and installations of plans related to this procurement. To
SOW General Facility Renovations A/5/8/9, Room 210, BLDG 2007 Page 7 of 22 meet the contract requirements, the Contractor shall prepare and furnish complete technical calculations, survey results, sketches, drawings, planning analyses, fabrication schedules, submittal schedules, and as-built drawings as required for the accomplishment of the project scope in accordance with the requirements of this Statement of Work. The Contractor shall provide all drawings, complete details, material samples, product data from manufacturers/suppliers and the proposed work schedule for Air Force review and approval prior to commencement of any work. Fabrication planning drawings and specifications shall be submitted for review and approval during the Construction Design development stages.
3.2. The Contractor shall identify and comply with all applicable federal, state, local, Department of Defense (DoD), Air Force, and JBSA-Lackland statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments), and Presidential Executive Orders, in effect on the date of issuance of this contract.
3.3. Fabrication shall not begin until Contracting Officer has approved all the design documents.
3.4. Investigative Requirements: The Contractor shall perform all investigation necessary to accomplish the intent of the work contained this Statement of Work. The Contractor’s services shall include any surveying required for completion of the design. The Contractor shall also furnish all labor, tools, materials and equipment to accomplish this investigative work.
3.5. Planning Documents: The text for delivered documents shall be prepared on standard letter size white bond paper (8.5" x 11"). Preliminary and final documents shall be printed double-sided (head to head). Attachments shall be fully identified, referenced in the text and folded to conform to the size paper used in the report. All documents will be produced both electronically and hard copy and will become property of the U.S. Government upon completion of the plan.
The Contractor shall also deliver copies of any analyses, reports, etc. using Microsoft Office on compatible program media version to the Contracting Officer.
3.6. Design: The Designer of Record (DOR) shall use the Unified Facilities Guide Specifications (UFGS) and requirements contained in this SOW to fully develop the technical specifications and construction drawings. The technical criteria contained and cited in this SOW and UFGS establish minimum standards for the design and construction quality.
3.7. The contractor shall be responsible for preparing, filing, testing, and paying for any and all fees/services required to obtain all necessary permits to meet the provisions of this SOW. All testing and/or permitting fees/services shall be included as part of the base bid.
3.8. Record As-Built drawings, one-line and prints from the original and subsequent construction effort “if” available will be provide, for informational purposes only, as part of the solicitation package in the Appendix. The Contractor shall field investigate, verify and validate for accuracy all information provided by the Government and all required dimensions, measurements and information themselves to produce the design documents. The Contractor shall be responsible for physically investigating all accessible areas as required. Non-accessible areas can be opened by base personnel with prior coordination if the Contractor determines that the inspection is necessary to properly define the design. The Contractor shall provide the

Contracting Officer 14-day advance written notice prior to any scheduled inspection of concealed spaces that require Government assistance. The Contractor shall develop accurate electronic plans in AutoCAD format and pdf format. Electronic plans developed by the Contractor, not the Government furnished record drawings, shall be used as the basis for design drawings.
3.9. The contractor shall submit for government review and approval, bound paper copies and electronic copies of 65% and 95% completed design packages consisting of detailed design analyses, plans and specifications for the accomplishment of all construction work described within this SOW. The Contractor shall submit a 100% design package incorporating all correction from the 65% and 95% review. Mobilization and construction effort will only be allowed to begin upon the receipt of signed and sealed 100% design package. All electronic copies shall be provided to the government in the native file format, AutoCAD and PDF format.
3.10. RESERVED
4. SHOP DRAWINGS:
4.1. All designs and drawings will be developed and furnished by the Contractor and shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed. The drawings shall follow section 01 35 13.20 00 (See Appendix F: Section 01 35 13.20 00 Special Project Procedures -CAD). Each sheet or drawing will include a border and a graphic scale bar to indicate the appropriate scale for the work shown. Drawing dimensions shall be in English units.
Standard fonts, material indications and symbols for mechanical and electrical equipment shall be used. Project title sheets shall have an index that lists the drawing number, drawing name, sheet number of total sheet number, i.e. Sheet X of Y, and CAD file name.
4.2. Drawings shall be prepared on 24" X 36" sheets unless specifically waived by the Contracting Officer on a case-by-case basis. The drawings shall be legible when reduced to one-half the original sizes. The contractor shall separate Architectural, Structural, Civil, Mechanical, Electrical, Fire Alarm/Fire Protection sections into their own individual sections within the drawing.
4.3. Construction Design Submittal (100 Percent Design). In the 100% construction design submittal, the Contractor shall provide those services necessary to prepare drawings and other documents setting forth in detail the requirements for fabrication of the project for approval by the Contracting Officer. This work shall include, but not be limited to, the following:
4.4. CONTRACT DRAWINGS: Drawings shall be prepared on AutoCAD release 2012 or compatible format and pdf.files. All drawings shall utilize A/E/C CAD Standards, Rel 4.0 or latest release and SDSFIE 2.61 for compliance with the installation standards. All contract drawings shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed.
The drawings shall be drawn to appropriate scales and dimensioned completely and accurately.

Extensive explanatory notes on the drawings should be held to a minimum or placed in the specifications. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible. Drawings shall be delivered in two (2) hard copies, standard black line quality paper, and two electronic copies (CADD and pdf.). Both hard copies and electronic copies of the drawings shall be provided to the Contracting Officer for review at the 65% design phase and the 95% design phase. Upon the acceptance of the 95% design drawings, the Contractor shall provide to the Contracting Officer signed and sealed final design submittal. All review drawings shall be 24” X 36” in size.
4.5. SHOP DRAWINGS: If submitted as part of the Repair Plan, shall comply with contract drawings requirements.
4.6. DESIGN SERVICES: Site Design Data: The contractor shall perform field reconnaissance, surveys, and site investigations required to obtain engineering information and design data necessary to comply with the provisions of the contract documents in accordance with the requirements of this SOW.
4.7. Government Review: The Contracting Officer and/or their authorized representative may furnish the contractor review comments on the data submitted. The government's review is intended to be limited to functional aspects, with limited technical review of a general cursory nature only. Any review by the government on technical items shall not be construed to relieve the contractor of responsibility for technically correct and complete documents in compliance with applicable codes, industry standards, and the intent of this SOW. Review comments for compliance with Federal Acquisition Regulations (FAR) will also be provided by the Contracting Officer. The Contractor shall comply with the review comments in the development of data for subsequent submittals. If any review comment requires clarification and/or amplification to assure compliance, the contractor shall notify the Contracting Officer or his/her authorized representative in writing. After each review, the contractor will be furnished one set of comments to be annotated and returned to the government. Comments annotated by the contractor with a "D" - do not concur, "E" - exception, or "X" - delete, shall have an explanatory note added to justify the noncompliance with the comment. The Contractor shall furnish these annotated comments to the Government no later than 3 calendar days after receiving the comments.
5. AS-BUILT DRAWINGS: The contractor shall provide complete detailed as-built drawings representing the entire project as constructed under the contract. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Drawings shall be delivered to the government on 14” x 20” hard copy, standard black line quality paper and electronically on AutoCAD release 2012 or compatible format and as PDF on CD. Extensive explanatory notes on the drawings should be held to a minimum. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible.
5.1. Preliminary as-built drawings shall be submitted with an AF Form 3000 for government review/approval. The preliminary As-builts shall be submitted concurrently with the request for

Upon government approval of the preliminary as-builts, the contractor shall submit the entire final set of hard copy and electronic as-builts drawings for final approval and acceptance. As-builts shall be included as a separate item in the progress schedule AF Form 3064.
5.2. Two (2) hard copies of the drawings shall be submitted to the Government upon completion and shall be reviewed for accuracy. Upon approval, the contractor shall provide two (2) hard copies and two (2) electronic CADD and pdf. Files on CD or DVD following section 01 35
13.20 00 (See Appendix F: Section 01 35 13.20 00 Special Project Procedures -CAD).
5.2.1. Design Analysis:
5.2.2. Drawings: Drawings shall be final, complete and include, but not be limited to, the sections, profiles, details and elevations.
5.2.3. Fabrication Schedule: The contractor shall provide a detailed fabrication schedule. The schedule shall indicate all work tasks, including but not limited to, material and equipment delivery periods, and indicate the anticipated critical path.
5.2.4. Catalog Cuts: Complete package of catalog cuts and manufacturer’s data used in the design, including, but not limited to, mechanical, electrical, plumbing, fire surpression, fire protection…
6. SUBMITTALS. During the design phase the DOR shall update the provided Schdeule of Material Submittal with additional items for government approval to comply with the intent of this Statement of Work (SOW), these items shall include all newly installed products and/or reference material that are to remain government property at the conclusion of this project.
Contractor shall utilize the updated Schedule of Material SuÄãittalë to provide submittals on AF IMT 3000 Material Approval Submittal Form for all listed items, prior to work or installation of materials. Do not purchase or conduct work until the approved submittal form has been returned from 502 CONS. At a minimum, the Contractor shall allow 14 calendar days for Government’s review of the submittal for approval or disapproval.
6.1. Meeting Minutes: The Contractor shall submit meeting minutes of all design review meetings and meetings held with the Government personnel prior to this submittal.
6.2. Project Disk: DVD or CD containing the following:
A. All updated/as-built 100% drawing files in AutoCAD and .pdf. All external reference files, or X-Refs, shall be fully resolved or bound such that each drawing is a standalone file.
B. The entire 100% Construction Design Analysis package for use on Microsoft Word or compatible software.

6.3. QUALITY ASSURANCE. The Contractor shall provide a Quality Control (QC) person at the work site at all times for continuous surveillance of project activities to ensure workmanship, methods, materials, and equipment conform to contract requirements. QC shall have the “stop work” authority to prevent or stop performance inconsistent with the contract requirements.
6.4. The Contractor shall submit a Quality Control Plan (QCP) for Government approval to the Contracting Officer at least 14 calendar days prior to commencing work. The work cannot proceed without an approved QCP. At minimum, the QCP shall discuss the following topics:
- Introduction: task order description, and quality commitment statement
- References
- Project Organization: key project personnel and responsibilities
- Quality Control Plan Elements
- Submittals
- Quality Management Procedures
- Testing / Inspection / Monitoring
- Closeout Inspection
- Documentation and Control
- Forms and Records
- Nonconformance and Corrective Action
- Conferences and Meetings
6.5. In addition to normal review meetings, a formal interchange meeting may be held if deemed necessary by the Government. Otherwise, the Contractor may informally call or visit the Contracting Officer at any time to clarify the work or expedite the preparation of the plan. The Contractor shall be responsible for making memoranda for the record of any conversations and minutes of any meetings with Government personnel concerning this project.
7. CODES AND REGULATIONS:
7.1. All work shall be accomplished in accordance with the latest editions of OSHA requirements, applicable safety standards of the National Fire Protection Association (NFPA), UFC’s, in accordance with JBSA Lackland codes, standards, and/or regulations. In the case where two or more standards are applicable, the contractor shall ensure compliance with the more stringent of the standards. The Contractor shall also comply with the most recent edition of following specifications:
7.2. United States Guide Specifications to include those specific to JBSA Lackland, TX.
National Fire Protection Association (NFPA 101) - Life Safety Code.
7.3. National Fire Protection Agency (NFPA 780) - Standard for the Installation of Lightning Protection Systems.
7.4. National Electrical Code (NEC).

7.6. Unified Facilities Criteria (UFC-3-520-01) Interior and Exterior Lighting and Controls.

8. PROTECTION OF GOVERNMENT PROPERTY: Contractor shall protect all Government and personal property in the affected area from damage. Any damage caused by the contractor shall be replaced or repaired to Government satisfaction prior to final payment.
9. GENERAL SAFETY REQUIREMENTS:
9.1. The Contractor shall provide a Site Safety and Health Officer (SSHO) at the work site at all times to perform safety and occupational health management, surveillance, inspections, and safety enforcement for the Contractor. The Contractor QC can be SSHO on this project.
9.2. The Contractor shall submit a site specific Accident Prevention Plan (APP) for government approval to the Contracting Officer at least 14 calendar days prior to commencing work. Work cannot proceed without an approved APP. All changes to the approved APP shall be made with the knowledge and concurrence of the Contracting Officer and 502 CES PM. At minimum, the APP shall discuss the following topics:
• Site Background
• Project Organization: key project personnel and responsibilities
• Site Hazards: chemical, physical, biological
• Exposure Monitoring
• Site Control
• Personal Protective Equipment
• Decontamination
• Safety Policy and Procedures
• Emergency Response Plan
• Training
• Medical Surveillance
• Recordkeeping
9.3. The Contractor shall obey the posted base speed limits and wear seat belts on base IAW AFI 91-207. Report contractor accidents involving injury to personnel or damage to government property to the Contracting Officer and the 502 ABW OL-A/SEG, Ground Safety Officer immediately.
9.4. The Contractor shall familiarize and comply with the general safety requirements IAW DOL regulations, OSHA 29 CFR Part 1926, as well as applicable AFOSH standards and AFRs. These standards and AFRs are available in the 502 ABW OL-A/SEG, Ground Safety Office, building 2484.

10. UTILITY OUTAGES:
10.1. Utility outages shall be defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.
10.2. All utility outages shall be coordinated with the Using Agency through the 502 CES PM.
Notice of the necessity for an outage shall be provided by the Contractor to the Contracting Officer, in writing, not less than “15 working days” prior to the time of the required outage; and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration. AF Mission requirements may dictate that utility outage be performed on weekends, after normal working hours or on holidays.
10.3. Once a utility outage is arranged and work has begun, work shall continue until all utilities are restored to the affected facility or facilities.
10.4. In the event a scheduled utility outage is cancelled by the Government, notification will be given to the Contractor as soon as the Government is aware of the pending cancelation (normally at least 24 hours in advance of the time for the outage to start). Rescheduling of a cancelled outage must be coordinated with written notification as described above.
11. CONTRACTOR'S USE OF ELECTROMAGNETIC EMISSION DEVICES ON
BASE:
11.1. The Contractor shall comply with the AFMTC Regulation 700-14, Jul 89. The regulation is available for inspection at the Communications Squadron, Building 1050, JBSA Lackland.
Electromagnetic emission devices include: Radio/Radar Transmitters; Navigational Aids/Instrumentations; Signaling, Intrusion Detecting, and Identification Devices; Mobile and

Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur Radio Stations. None are permitted within facility 2000-2007.
11.2. RADIOACTIVE EQUIPMENT: Use of radioisotopes or radiation producing equipment (density gauging, NDE weld testing, etc.) requires a Nuclear Regulatory Commission (NRC) Radioactive Material License, with all documentation submitted and approved prior to bringing the material on base. To obtain authorization, contact the Base Radiation Protection Officer (RPO), at 559 AMDS/SGPB, Bioenvironmental Engineering Office, Building 7065, JBSA Lackland; phone: (210) 671-7235. The Contractor shall also comply with all requirements of
AFI 40-201.
11.3. Contractor shall forward application to the Contracting Officer 30 calendar days prior to commencing operations using radioactive materials.
11.4. Contractors possessing Agreement State Licenses must also submit an NRC Form 241 to NRC in compliance with 10 CFR 150.20. Request shall include the following:
• Description of proposed activities on NRC Form 241.
• Procedures established to ensure radiological health and safety of all personnel.
• Name of responsible contractor representative.
• Current copy of application for NRC or Agreement State License. The license must specifically state the installation by name or approval for temporary job sites anywhere in the United States where the NRC or Agreement State maintains jurisdiction.
• The part of the Air Force contract describing work to be done at the base and the inclusive dates of such work.
• An acknowledgement that the base RPO can make periodic checks to ensure the contractor is following applicable radiological health and safety practices.
• Contractors requiring more than 180 calendar days per calendar year of operation must possess an NRC license.
12. FIRE REGULATIONS AND PREVENTION PRACTICES:
12.1. The contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association, other recognized fire prevention agencies, and base regulations.
Contractor shall comply with local, state, federal, and base safety requirements. Contractor shall obtain a FIRE PERMIT from the Fire Dept Ph: (210) 671-2921 before any burn work and/or welding is performed.
12.2. EXPLOSIVE OPERATED HANDTOOLS: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.
13. GENERATED REFUSE: The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system. All contractor generated refuse and waste shall
SOW General Facility Renovations A/5/8/9, Room 210, BLDG 2007 Page 15 of 22 be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in trucks with tight-fitting beds and covers to prevent spillage on roadways. The construction site shall be kept neat, orderly, and safe for workmen at all times. All required manifests shall be obtained prior to the transportation of refuse off-site.
14. ENERGY REQUIRMENTS: Design of new construction and renovations shall be in full compliance with ANSI/IESNA/ ASHRA Standards with specific attention to ASHRA Standard 90.1-2004 or latest edition. In addition, new construction and renovations for existing facilities must comply with Energy Policy Acct of 2005 (EPACT 05), Executive Order 13423 and energy conservation standards under Unified Facilities Criteria, UFC 3-400-01 05 July 2002, UFC 3- 410-01FA 15 May 03, UFC 3-410-02A 15 May 03, UFC 3-440-01 14 Jun 02 or latest edition for all. Air Force Design Sustainability shall be applied in the development concepts of planning, design, construction, environmental management, operation, maintenance and disposal of facilities and infrastructure project, consistent with budget and mission requirements. Design Energy Compliance Analysis (ECA) must be provided with the design package.
15. INVENTORY OF INSTALLED PROPERTY: The Contractor shall prepare and submit a list of new equipment or units of equipment that require gas, water, or electrical utilities connections per the attached JBSA Lackland Environmental Protection specification, see Appendix C. (Examples: AHUs, fans, air conditioners, compressors, condensers, chillers, boilers, furnaces, heat exchangers, pumps, cooling towers, tanks, fire hydrants, sinks, water closets, urinals, lavatories, shower stalls, and other large plumbing fixtures, lighting fixtures, electric heating equipment, motors, and other electrical equipment). The list shall be reviewed periodically by the government to verify completeness and accuracy. Completed list shall be turned over to the Contracting Officer's Technical Representative (COTR) at least 14 calendar weeks prior to the pre-final inspection.
16. INSPECTION AND ACCEPTANCE: Inspections and final acceptance will be made as completed by the contractor with the following additional provisions:
16.1. The Contractor shall give the Government, in writing, a minimum of three (3) working day’s notice for all requested inspections. Inspections shall be staggered such that two inspections will not occur at the same time.
16.2. The Contractor shall make sure that the work is ready for all inspections. The presence of an unreasonable number of deficiencies, as determined by the Contracting Officer's Technical Representative (COTR), may cause the inspection to be rescheduled.
17. DETERIORATED CONSTRUCTION AND FINISH: The 502 CES PM will be the governing authority in determining if existing construction and finishes are deteriorated and require repair and/or replacement.
18. CONTRACTOR’S WARRANTY: The Contractor shall guarantee all materials and workmanship for a period of one year from the date of project completion, Beneficial

Occupancy, and shall within that period correct any and all defects in materials/workmanship.
Correction of all defects under this warranty shall be at the expense of the contractor.
19. DD FORM 1354: The Contractor shall prepare and submit for approval a DD Form 1354, Transfer and Acceptance of Military Real Property prior to acceptance of the project by the government. The Contractor will use the checklist provided in Appendix E, and complete all applicable information to develop the DD Form 1354. A preliminary copy of the form along with the checklist shall be submitted for review by the Government to verify completeness and accuracy at least 30 days prior to the pre-final inspection. The completed DD Form 1354 along with the checklist shall be turned over to the Contracting Officer's Technical Representative (COTR) at least 14 calendar days prior to pre-final inspection.
20. CONTRACTOR REQUIREMENTS: Records: The contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publication, IAW with the records management procedures in Air Force Instruction (AFI) 33-322, Records Management Program, Air Force Manual (AFMAN) 33-363, Management of Records, AFI 33-364, Records Disposition-Procedures and Responsibilities, and the Air Force Records Disposition Schedule (AF RDS) located at 30TUhttps://www.my.af.mil/gcss-af61a/afrims/afrims/rims.cfmU30T.
20.1. Site Conditions: The contractor shall prepare a site plan indicating the proposed location and dimensions of any area to be isolated/fenced for use by the Contractor, the number of trailers to be used, avenues of ingress/egress to the project area, and laydown/storage area. Any areas, which may have to be graveled to prevent the tracking of mud, shall also be identified. The contractor shall also indicate if the use of a supplemental or other staging area is desired. Please see Appendix A for general location of the site. Contractor site plan shall be submitted for approval by the government 14 working days in advance prior to mobilization for construction.
The contractor is solely responsible for securing the staging area.
20.2. O&M Manuals: As applicable, the contractor shall provide O&M Manuals to the Government for all installed equipment. In addition to the manuals, the contractor shall provide a brief operational training on each piece of equipment in the system to the Area Maintenance Unit for future maintenance of the equipment.
21. CONTRACTOR WORK IN A SECURED AREA REQUIREING ESCORTS:
21.1. PROJECT CONSTRUCTION SECURITY REQUIREMENTS: All persons requiring access within the boundaries of the project and restricted controlled area shall comply with all security requirements. All contractor and sub-contractors accessing the restricted secured area shall undergo a personnel security process. One primary requirement is that all persons shall provide a biographical data containing full name, current address, Social Security Number, Date of Birth, Place of Birth, Driver’s License & State and proof of citizenship (all personnel performing on this contract on-site shall be U.S. Citizens). Local and national criminal and subversive file checks are conducted by Security Services or other designated security and law enforcement authorities at no cost to the contractor. All vehicles requiring access into the restricted area shall have to be processed for authorization to enter the compound. 21.2. Identification of Employees. The contractor shall be responsible for furnishing to each employee, and for requiring each employee engaged on the work to display, identification as approved and directed by the Contracting Officer. Prescribed identification shall immediately be delivered to the Contracting Officer for cancellation upon release of any employee. When required, the contractor shall obtain and provide fingerprints of persons employed on the project.
All contractor and sub-contractor personnel shall wear identifying markings on hard hats and safety vests clearly identifying the company for whom the employee works.
21.3. Employee Parking. Contractor employees shall park privately owned vehicles in an area designated by the Contracting Officer. This area will be within reasonable walking distance of the construction site. Contractor employee parking shall not interfere with existing and established parking requirements of the military installation. Please see the Appendix A for general location.
21.4. Job Site Tools and Equipment. All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations. All tools and equipment shall be at hand prior to the contractor commencing work.
21.5. Project Signage.
