JUSTIFICATION FOR OTHER THAN

FULL AND OPEN COMPETITION

I. CONTRACTING ACTIVITY

The Department of Air Force, 1st Special Operations Contracting Squadron, Hurlburt Field, FL.

This document serves as a “Justification for Other Than Full and Open Competition”. The purchase request number is F2FF116215AW01.

II. NATURE AND/OR DESCRIPTION OF THE ACTION BEING PROCESSED

This action is a new sole source contract for the three (3) GAUMARD Trauma HAL

Mannequins and accessories. This action will be competed and awarded as a firm fixed price contract.

III. DESCRIPTION OF THE SUPPLIES/SERVICES REQUIRED TO MEET THE

AGENCY’S NEEDS

GAUMARD Trauma HAL Mannequins will provide training that will exponentially increase the quality of the units Tactical Combat Critical Care (TCCC) program as well as the AFSOC

SG/SGAM internal doctor and medical technician training program. These mannequins will aid in the well-being of injured personnel and prevent loss of life. Required features include:

a. Trauma HAL medium skin tone adult simulator. Rugged and resilient.

b. Suitable for being utilized in outdoor/inclement weather conditions without reduced performance including temperatures between:

(1) Operating temperature: 50-95 degrees F (10-35 degree C)

(2) Humidity: 5%-95% (non-condensing)

c. Rugged construction for combat, disaster, trauma care, and Chemical, Biological, Radiological, Nuclear Enhanced (CBRNe) conventional weapons simulation.

d. Completely wireless and tether less. All operating system components are housed inside the simulator.

e. Rugged shoulder and hip joints for field and evacuation exercises.

f. Splash proof for CBRNe water spray decontamination.

g. Can be a full body mannequin similar in size to a medium build male or female and can fit in a NATO litter; cover with blanket without overheating.

h. Control HAL using rugged wireless tablet PC from up to 900ft away HAL is fully functional on battery power for point-of-injury care and care in motion.

i. Bleeding trauma limbs and wound sites for training emergency bleeding control techniques 1.5 liter “On-board” simulated blood supply; blood loss and blood reservoir level shown on tablet PC.

j. Fully operational on battery power or AC adapter. Internal rechargeable battery provides 10 hrs. Of continuous use.

k. Design for fast scenario resets with easy to use fluid refill ports and interchangeable limbs.

l. Real time CPR quality and performance feedback.

m. Built in library of programmed scenario included: modify them or create your own.

n. Blinking eyes with light reactive pupils.

o. Software controlled fluid secretion at ears, eyes or mouth.

p. Mild and sever convulsions.

q. Intubate ET tube, LMA, or King LT. Programmable difficult airway with tongue edema and pharyngeal swelling.

r. Laryngospasm with tight seal preventing intubation.

s. Surgical airway for cricothyrotomy, tracheostomy, and tracheal hook exercises.

t. Permits liquids in the airways for suctioning.

u. Streaming audio; hear responses at distances up to 150ft. Pre-record and program verbal responses in any language.

v. Palpable ribs and realistic chest cavity.

w. Shallow and deep needle decompression with audible hiss and needle placement detection. Bilateral chest drain sites.

x. Unilateral chest rise with right main stem intubation.

y. Automatic chest rise and fall with selectable respiratory patterns.

z. Selectable anterior lung sounds.

aa. Programmable heart sounds, monitor ECG using standard medical equipment with no special adapters required.

bb. Built in library of cardiac rhythms, pacing, cardioversion, and defibrillation.

cc. Measure blood pressure using a real blood pressure sphygmomanometer.

dd. Detectable oxygen saturation using standard OSAT monitors.

ee. Brachial pulse at bend in the arm antecubital vein for IV training.

ff. Software controlled visible cyanosis.

gg. Radial, brachial, carotid, femoral, popliteal and pedal pulses and blood pressure dependent.

hh. Supports sternal intraosseous infusion I/O (F.A.S.T.1 and more).

ii. Intraosseous tibia I/O access with replaceable tibia bones.

jj. Lower arms and legs are easily replaceable with trauma stumps that bleed in sync with heart rate and blood pressure.

kk. Bleeding wounds at axilla and groin can be packed using combat gauze and respond to applied pressure.

ll. Sensors in the arm and legs respond to tourniquet application and applied pressure.

mm. Bleeding from two arteries in the forearm and three arteries in the legs respond to tourniquet application.

nn. Pressure on femoral artery reduces or stops distal bleeding in leg.

oo. Visible gastric distention with esophageal intubation.

pp. Internal fluid bladder with male genitalia for catheterization.

qq. Bowel sounds in four quadrants.

rr. Instructor table with control software interface to include state, palettes, scenario, action logs, monitors, speech and medication.

IV. STATUTORY AUTHORITY PERMITTING OTHER THAN FULL AND OPEN

COMPETITION.

In FAR 13.501, it is stated that the “Sole source (including brand name) acquisitions conducted under simplified acquisition procedures”. This particular action is a brand name only acquisition conducted under simplified acquisition procedures.

V. DEMONSTRATION THAT THE CONTRACTOR’S UNIQUE

QUALIFICATIONS OR NATURE OF THE ACQUISITION REQUIRES THE USE OF

THE AUTHORITY CITED ABOVE (APPLICABILITY OF AUTHORITY)

The GAUMARD Simulators Trauma HAL Mannequins are the only mannequins on the market that meet the requirements addressed in paragraph III. Specifically, there is no other mannequin that provides the rugged tether less features suitable for outdoor/inclement weather conditions, has sensors that provide feedback on proper tourniquet application for extremity hemorrhage, and has both axillary and groin wounds to teach junctional hemorrhage control techniques. This is the only mannequin found that will allow for the assessment/control of non-compressive hemorrhage (coagulant gauze).

Finally, it is the only TCCC type model with 12-lead EKG and quantitative capnography (CO2) measuring capability. This allows for higher level medical training for medics and doctors.

Additional special features not available with other models include:

a. Cardiovascular/Circulatory system that:

1. Assesses all pulses to include carotid, radial, dorsalis Pedi, femoral, popliteal and brachial.

2. Performs blood pressure measurements, pulse oximeter monitoring, cardiac auscultation (heart sounds) and the mannequin can be placed on a cardiac monitor.

3. Mannequin allows for external chest compressions, automatic external defibrillation (AED) and defibrillation using live defibrillators and cardiac pacing.

4. Allows insertion of peripheral intravenous lines and obtains intraosseous access to tibial tuberosity, manubrium, or sternum.

5. Can administer IV fluid therapy and simulated intravenous medications.

b. Respiratory system that:

1. Allows for training to manage difficult airway: tongue swelling, pharyngeal obstruction as well as perform suctioning of airway (Nasopharyngeal, Oropharyngeal) and insertion.

2. Can perform Laryngeal Mask Airway insertions as well as esophageal tracheal airway (Combitube), endotracheal intubation (oral and nasal).

3. Can perform a Surgical Cricothyroidotomy, tracheostomy, assess respiratory rate, rhythm, depth and can assess bilateral chest rise/fall.

4. Can assist breathing with barrier device or face mask and can perform bag-valve-mask ventilation with chest rise and fall.

5. Can auscultate/assess lung sounds and can perform needle decompression midclavicular as well as insertion of chest tube and maintain closed drainage system.

6. Can assess secretions from nose to mouth.

c. Muscular system with ability to control bleeding (direct pressure, elevation, pressure points) by applying tourniquet and can assess/treat amputated limbs, administer intramuscular injections in thigh and deltoid.

d. Skeletal system with ability to apply cervical collar.

e. Urinary system that can perform male urinary catheterization (straight, Foley & indwelling) and can assess urine output from catheterization.

f. Nervous system with ability to recognize/treat seizure activity.

g. Integumentary system that can recognize/treat cyanosis.

h. Digestive system that can auscultate bowel sounds.

i. Eye features that can assess pupillary reaction/variable pupil sizes and can access secretions from eyes.

j. Ear features that can assess secretions from ears.

k. Ability to perform basic head to toe patient assessment, perform decontamination.

l. Mannequin speaks/responds to questions/makes sounds (pre-programmed responses, as well as live person can speak through the speaker).

m. Ability to be used for CRBNE training (it is spray-proof and can be run through a decon line).

n. Tetherless system relying on radio frequency with a 900ft range.

This comprehensive capability facilities’ full spectrum medical training: POI care, tactical field care, casualty evacuation, and trans load.

VI. DESCRIPTION OF EFFORTS MADE TO ENSURE THAT OFFERS ARE

SOLICITED FROM AS MANY POTENTIAL SOURCES AS DEEMED PRACTICABLE

A sources sought notice was posted on FBO on 19 Aug 2016 and due on 25 Aug 2016; however, only 1 response was received from Gaumard Scientific Company, Inc.

VII. DETERMINATION BY THE CONTRACTING OFFICER THAT THE

ANTICIPATED COST TO THE GOVERNMENT WILL BE FAIR AND REASONABLE

It is anticipated that the price will be considered fair and reasonable based on prices received from competitive quotes.

VIII. DESCRIPTION OF THE MARKET RESEARCH CONDUCTED AND THE

RESULTS, OR A STATEMENT OF THE REASONS MARKET RESEARCH WAS NOT

CONDUCTED

Twenty-seven different mannequin models were reviewed to include GAUMARD, CAE, Laerdal, and KGS MATT products. There are no other products on the market that will meet the fifty-seven required training objectives. There are a number of mannequins that provide most of the requirements and features discussed in this justification; however, they do not provide all of the requirements compiled into one mannequin like the GAUMARD Trauma HAL.

Through multiple means, AFSOC SG/SGAM will continue to monitor market information/sources and technical publications to determine whether other products are available to meet AFSOC’s critical requirements. In addition, 1SOCONS will continue to issue sources sought synopses or publicize the solicitation in an effort to identify potential vendors, interested sources, or new products in the industry.

XII. CONTRACTING OFFICER’S CERTIFICATION

The contracting officer’s signature on the Coordination and Approval Document evidences that they have determined this document to be both accurate and complete to the best of their knowledge and belief.

XIII. TECHNICAL/REQUIREMENTS PERSONNEL’S CERTIFICATION

As evidenced by their signatures on the Coordination and Approval Document, the technical and/or requirements personnel have certified that any supporting data contained herein, which is their responsibility, is both accurate and complete.
