seven days a week, and blood product transfusions can be required at any time. The volume of blood products transfused at the PIHC does not justify on-site blood product services (due to the additional cost and compliance requirements incurred), but rapid access to these services is critical to ensure standards of care are met.

3. Description of the supplies or services required to meet the agency’s needs (including the estimated value).

a. Project title. STAT Reference Laboratory Testing and Blood Bank Services provided by the La Paz Regional Hospital on a sole source basis.

b. Project description. All STAT specimens collected at the PIHC to be tested by the contracted laboratory service will be delivered by PIHC courier or staff drivers to the contractor’s facility, where the blood specimens will be processed by certified and/or licensed staff to assure quality results and proper matching of blood products with intended recipients of such products. Results will be available within 2 hours of receipt of specimens or PIHC staff will be notified of delays with expected time of completion provided. Once matching of blood products is completed, PIHC will be notified so that arrangements can be made for the blood products to be returned to the PIHC by courier or staff drivers.

The blood products will be administered as indicated at the PIHC.

This laboratory must also be able to provide transfusion compatibility testing for blood and/or blood products on an as needed and if necessary emergency basis. As blood products expire within 30 minutes of leaving blood bank monitored refrigeration if transfusion has not begun, the blood and/or blood products must be acquired within a 15 to 20 minute radius of the IHS Parker Indian Health Center, Parker, Arizona (allowing time for the required pre-transfusion blood/blood product and patient preparation).

La Paz Regional Hospital is the only laboratory that can perform emergent clinical testing and blood/blood product support within a 15 to 20 minute radius of the IHS Parker Indian Health Center and/or provide at most a 1 hour turn-around-time of result reporting for emergency room patients. The next closest facility is over 1 hour away by ground transportation.

6. Description of the efforts made to ensure that offers are solicited from as many potential sources as practicable. Indicate whether a FedBizOpps notice was or will be publicized as required by FAR Subpart 5.2 and, if not, which exception under FAR 5.202 applies. This effort will be synopsized in FedBizOpps in accordance with requirements in FAR 5.2. Any responses received will be evaluated prior to award of a contract. The notice will specifically state the Government’s intent to award a sole source contract to the La Paz Regional Hospital.

7. Determination by the Contracting Officer that the anticipated cost/price to the Government will be fair and reasonable. The Contracting Officer will determine if the anticipated cost to the Government for the requirements covered by this J&A will be fair and reasonable. The fair and reasonableness of the cost/price will be based on the analysis of the vendor’s proposal, as well as previous procurement history for this requirement. This analysis will be conducted using experienced project managers, contract specialists and filed pricing support.

8. Description of the market research conducted (see FAR Part 10) and the results, or a statement of the reasons market research was not conducted.

Informal research was accomplished to determine the extent of the competition.

Internet searches were conducted but no vendor was found other than the La Paz Regional Hospital that can provide the blood products and testing in the time period required for patient care.

9. Any other facts supporting the use of other than full and open competition.

No other facts that were not previously mentioned in the above narratives.

10. Listing of sources, if any, that expressed, in writing, an interest in the acquisition. No other sources have expressed an interest, in writing, in the proposed acquisition.
