Justification for Single Source Awards IAW FAR 13.106-1

For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID:

1. Contracting Activity: Department of Veterans Affairs, VISN 8, Network Contracting Office (NCO) 08, located in Orlando, Florida in support of station 675 , Daytona MultiSpecialty Outpatient Clinic located at 1776 N. Williamson Blvd, Daytona Beach, Florida 32114.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The

Daytona Multi-Specialty Outpatient Clinic, Opthalmology Department requires to purchase a brand name Haag-Streit BQ 900 Unit Slit Lamp one each (1), to include software, Beam Splitter and Observer Tube with Eyepiece. Estimated total is .

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Opthalmology Department in Daytona requires to purchase an additional Brand Name Haag-Streit BQ 900 Unit Slit Lamp to increase capacity in the facility. There are currently 26 Haag-Streit Slit Lamps between Orlando and Daytona Opthalmology facilities. The Haag-Streit BQ 900 Slit Lamps has the best quality optics for the providers to diagnose and treat occular diseases of veteran patients. In addition, purchasing another brand would require the facility to purchase additional new accessories that was not budgeted for original request, as well as spend more time away from patient care while training staff on new machines. Accordingly, the Veteran’s Health Administration would experience substanstial duplication of cost that could not be expected to be recovered through competition and unaccpeatble delays in fulfilling it’s requirement

4. Description of market research conducted and results or statement why it was not conducted:

Market research confirmed there are SDVOSBs, and Small Businesses that can distribute the product, however North American Industry Classification System (NAICS) code waiver is not available for NAICS code 339112. SDVOSBs must comply with Limitation on Subcontracting 852.219- 76(a)(2)(iii), “The offeror certifies that it will supply the end item of a small business manufacturer, processor, or producer made in the United States, unless a waiver as provided in 13CFR 121.406(b)(5) has been issued by SBA or If this is a multiple item acquisition, the offeror certifies that at least 50% of the estimated contract vaue is composed of items that are manufactured by small business concerns. The items required are manufactured by a large business Haag-Streit, Inc. It is in the best interest of the Government to award procurement directly to the OEM.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
