About this file
This is a Terms and Conditions document from the Alabama Department of Public Health (ADPH) for a laboratory supplier contract. ADPH seeks to establish a contract effective June 29, 2026, to provide cytology and pathology services including reading and reporting on Image-Guided Pap Smears, Aptima HPV tests, HPV primary screening, biopsy tissue samples, genotyping, and p16 biomarker analysis. Biopsy samples will be collected from multiple sources including colposcopy, endocervical curettage, endometrial biopsy, LEEP procedures, cervical polypectomy, endocervical brushings, and punch biopsy. The contract will replace an existing contract expiring June 28, 2026. Award will be made on an "all or none" basis to the lowest responsible bidder meeting all specifications. Questions must be submitted through the Alabamabuys supplier portal by Thursday, March 19, 2026. The contract term is 12 months with options to extend for four additional 12-month periods under the same pricing, terms, and conditions, with written approval required from both parties no later than 30 days prior to expiration of each period.
Bidding will occur through the Alabamabuys supplier portal, with freight charges on an f.o.b. destination basis included in bid prices. Suppliers must carry general liability insurance of $1,000,000 per occurrence in effect for the contract period, with the State of Alabama listed as certificate holder, and must provide a minimum of three references of similar service with contact information. The State does not guarantee specific specimen quantities; orders will be placed by agency as needed with complete shipping instructions. Suppliers must accept multiple forms of electronic payment at no additional cost to the State, including state credit cards, P-cards, and EFT. Price escalation is permitted only with documented evidence and written approval from both parties, with increases limited to corresponding CPI increases; any price de-escalation must be passed to the State immediately. Suppliers defaulting on contract obligations may be subject to suspension, debarment, and liability for additional procurement costs incurred by the State. Any additional information requested from suppliers must be furnished within five business days.
