Attachment 1- Scope of Work
Scope of Work
For
Physical Therapy Equipment
12/01/2015
1. Isokinetic System. The contractor shall provide a System to diagnosis patient impairments.
The system shall be a multi-mode and computerized robotic set of equipment.
1. 2. As a minimum the system shall be able to accommodate athlete and pediatric applications.
The system shall have a software interface which provides a basis for treatment and diagnostic outcomes. The system shall facilitate normative data for patients ages 5 through 83.
1.3. The system shall integrate with patient care provided and shall provide ease of operation.
As a minimum the system shall have a chair and measurement setting for use diagnosis of physical impairments.
1.4. The system shall be able to treat evidence based protocols and provide valid outcomes for Sports, industrial and pediatric medicine, Neurorehabilitation medicine. For sports medicine at a minimum the system shall provide Hamstring Injury Risk Management, Knee Osteoarthritis, Lateral ankle sprains, patellofemoral dysfunction, Anterior Cruciate Ligament and preseason screening, injuring prevention and athletic performance enhancement.
1.5. The entire system shall be a minimum of 64 square feet requirement for usage.
1.6 The System shall be 220 Voltage and a motorized seat height, front-to-back chair adjustment, fully assisted dynamometer height adjustment, and side-to-side adjustment is required.
Touch screen interface for quick, easy multi-mode operation; isokinetic, isometric, isotonic, reactive eccentric and passive.
• Concentric speed up to 500 deg/sec
• Eccentric speed up to 300 deg/sec
• Concentric torque up to 500 ft-lb (680 Nm)
• Eccentric torque up to 400 ft-lb (544 Nm)
• Passive speed as low as .25 deg/sec
- Passive torque as low as .5 ft-lb
- Isotonic torque as low as .5 ft-lb
1.7. A clinical data station operating on a windows platform shall provide patient monitoring with an LCD Flat Panel Touch Screen Color Monitor and speakers. A Color printer shall be provided with the unit.
1.8. The protocols required shall be six modes of operation to include an Isokinetic resistance model, reactive eccentric mode, passive motion mode, isometric mode, isotonic mode, and customized motor control.
1.9. The items to be used with the system shall include a closed chain attachment, ankle attachment, shoulder input tube, anti-shear (left pivot), anti-shear (right pivot), Iso Map, Hip attachment and chair wedge.
2. Trainer Treadmill. The contractor shall provide a system for training and rehabilitative treatment during physical therapy. The system shall have an instrumented deck that monitors and records step length, step speed and right to left time distribution (step symmetry).
2.1. The system shall also enable patient access for therapist manipulations and accommodate Body Weight Supported Treadmill Training (BWSTT) and compatibility with an Unweighing System.
2.2. The system shall integrate with patient care provided and shall provide ease of operation and shall provide eexercise summary and progress reports track progress and document outcomes.
The system shall track progress and outcome for specific gait parameters, including: average walking speed over time, total exercise time, total distance and steps taken, average step length:
RT vs. LT, step length variability: RT vs. LT and Time on each foot: RT vs. LT.
2.3. The system shall provide for quality data codes (G-codes) and calculations that provide a basis for evaluation for claims in accordance with the Center for Medicare & Medicaid Services to track beneficiaries. The system shall allow for effective impairment level reporting and increase efficiency and productivity in patient tracking.
2.4. The unit shall provide for age and gender-based comparisons for health population assessments. The system shall have a heart rate monitoring system to ensure level of intensity required. The unit shall have a minimum of a 12.1” color touch screen display and be able to operate windows system. The unit shall also allow connection to larger monitors with LCD projectors to enhance interaction for visually impaired patients. The system shall have a USB port to enable data transfer and software upgrades. A patient data collection software utility shall also be available.
3. Physical Examination/Therapy Tables. The contractor shall provide three (3) physical therapy tables for rehabilitative treatment during physical therapy.
3.1. The treatment tables shall provide an adjustable head and lower limb section with a fixed center section. This table adjusts to multiple positions permitting patient positioning in the most effective position. The head section drops 90 degrees and rises to 85 degrees; the lower limb section shall raise to 90 degrees.
3.2. The tables shall provide for stability and comfort. The provision for a durable and adjustable head section shall allow accurate positioning and is supplied with a key-hole cut out for comfortable positioning for neck treatments. The treatment table shall be able to lower to 20” and rises to 38”. A control shall be located on both sides of the base of the table in 6” strips, 3” designated for “up” control, and 3” for “down” control. The table shall be able to be raised and lowered from the 3” strips located on the base of the left and right legs at front of the table, with the “down” control on the left, and “up” control on the right. Dual head support.
3.3. A key-hole head section cutout with filler, Positive locking system for head section adjustments, gas spring for lower limb section adjustments. A fixed center section Antimicrobial Naugahyde Upholstery. The desired color scheme to match existing décor shall be royal blue and frame shall be white.
3.4. The tables shall be 3- section hi-low tables and 30 degrees wide treatment surface, (30” X77”) to fit in a designated section of a medical facility in Whiteriver. The table shall have a 500# lifting capacity.
4. Unweighing System. The contractor shall provide a system to assist in learning/re-learning to walk after an injury or disability. The preferred method of the rehabilitative process that shall be used is the Gait rehabilitation method. This therapy shall be used to provide physical therapy in attaining repetitive motions. The system shall be a multi-mode computerized robotic unit.
4.1. The system shall be able to shall assist and help when loss of the ability to ambulate or any other limited medical need. The unit will assist in the three main components of locomotion -posture, balance and coordination – which can be affect a variety of neurologic or musculoskeletal pathologies resulting in the disruption of an individual’s ability to walk normally. Partial Weight Bearing Gait Therapy (PWB) shall be the method used to help and provide physical therapy. The use will be the most appropriate way to assist in rehabilitative therapy and shall be prescribed for patients who are unable to support their own body.
4.2. The characteristics are: Dynamic suspension - allows for natural vertical displacement of center of gravity. The unit is a Patented off-loading mechanism - maintains constant force;
simply dial in amount to off load; Single-point suspension - permits functional pelvic rotation;
Unobstructed open access - Offset design provides full access for easy patient interaction;
Attachment rings - on support vest allow single or two-point pelvic stabilization when desired;
Low-effort manual operation - no power cord, no weights, no air compressor; Adjustable and removable patient handrails - accommodates diverse patient populations and allows for easy treadmill access; Unweighing Harness- fits a variety of patients; Adjustable height - can be assembled at reduced height for use in rooms with 8-foot ceilings; Adjustable suspension -accommodates children and adults; and Therapist seats - provide a place to sit during assistive exercise and provides a safer environment for therapist interaction
4.3. The minimum dimensions shall be dimensions are: 41.5" w x 50" depth x 106" h (105 x 127 x 270 cm) accommodates patients from pediatric up to 6' 11" (210 cm) on a standard treadmill;
alternate height setting for facilities with low ceilings: 93.5" h (241 cm) accommodates patients from pediatric up to 6' 2" (188 cm) on a standard treadmill. The system shall also accommodate a treadmill up to 35” wide; vertical adjustment of 60”, unloading weight capacity of 180 lbs.;
user capacity of 500 lbs.; a weight of 160 lbs.; and a minimum warrant of two years parts and one year labor.
5. Water rowing System. The contractor shall provide one (1) water rowing machine and monitor. The Unit shall have a free-flowing water flywheel to replicate actual rowing. The frame shall be an aluminum and powder coated steel frame and shall be of commercial medical grade.
5.1. The rowing machine shall replicates actual rowing feel flywheel that sits in enclosed water tank to provide smooth, quiet, self-regulated resistance. The unit shall include a performance monitor to track workout intensity, stroke rate, and heart rate. A coated tubular steel construction complements other commercial equipment.
5.2. The extra-tall seat shall be able to store upright. The unit shall measure 84 x 30 x 22 inches (W x H x D) to fit in a designated area in a medical facility in Whiteriver, Arizona. The unit shall weigh 200 pounds.
6. Physical Examination Tables (Neurological bariatric). The contractor shall provide two (2) neurological bariatric treatment table for rehabilitative treatment during physical therapy.
6.1.. The treatment tables shall have two sections, hand and foot pedal controls a 600# lifting capacity, dual lever retractable 4” caster wheels. The tables shall provide for stability and comfort.
6.2. A key-hole head section cutout with filler, Positive locking system for head section adjustments, gas spring for lower limb section adjustments. A fixed center section Antimicrobial Naugahyde Upholstery. The desired color scheme to match existing décor shall be royal blue and frame shall be white.
6.3. The tables shall be 2-section tables and 48” X77.5” treatment size to fit in a designated section of a medical facility in Whiteriver. The table shall have a 600# lifting capacity.
6.4. The Hi-Low/Bariatric table shall provide for postural exercises, rolling, turning and balance training. The table shall be a two section tables able to provide effective usage for patients who have a loss of sensation or poor balance. The table shall have the capability to treat neurological conditions; stroke, head injury and orthopedic treatments. The table shall provide for the ability to obtain stability and resistance to movement. The table shall utilize lifting columns to elevate and lower the table.
7. 7. Balance System (SD). The contractor shall provide a system to assist in learning/re-learning for balance and fall prevention. The preferred method of the rehabilitative balance assessments and objectively measuring the risk for falls and injury prevention is the Balance System method. This therapy shall be used to provide physical therapy in attaining repetitive motions static positioning. The system shall be a multi-mode computerized robotic unit.
7.1. The balance system will provide for twelve levels of platform control as well as static force setting – allows testing, training and rehabilitation programs for diverse populations.
7.2. Balance Training programming and protocols should include proprioception and stabilization exercise, range of motion and weight shift exercises.
7.3. Balance system will provide objective documentation through a color printer interface.
Printed color reports track progress and document outcomes – ideal for patient feedback, documentation and billing.
7.4. Balance system should have automated G-code Calculations and Impairment Level Reports Purpose for having the automated G-code software capabilities is CMS mandate for objectifying a patient’s impairments and functional limitations – important for Medicare reimbursement.
Having this capability also increases efficiency and productivity, improves documentation of rehab effectiveness, helps with audits and reduces claim denials. It should also allow for custom reporting and to allow for entry of unique comments to test results and assignment of CPT codes.
7.5. Balance system will provide an enhanced sensory integration balance testing capability. This provides for the ability to perform a modified version of a Balance Error Scoring System (BESS) test of postural stability, popular for concussion management. Modify existing or create custom protocols for both the CTSIB and BESS tests.
7.6. Balance system should provide at a minimum six training modes and five testing protocols.
7.7. Balance system will include a standardized fall screening and athlete knee injury screening tests capabilities. The test results are compared to age-based normative data to help identify fall candidates and athletes predisposed to knee injury.
7.8. Balance system software should include normative data to compare healthy populations stored for test comparison of older adults for fall screening and student athletes for concussion management. Balance system should also have audio and visual biofeedback features.
7.9. The balance system dimensions should not exceed: 30" wide x 44" depth x 74" h (76 x 112 x 188 cm). It should include a color touch-screen display with a diagonal viewing area of approximately 12” and have an adjustable screen to approximately 45 degrees.
8. The contractor shall deliver, install and assemble in the Whiteriver Service Unit in Whiteriver, Arizona.
9. The contractor shall coordinate with the on-site person David Farley (david.farley@ihs.gov);
