Statement of Work Subsistence Supplies Delivery

I. Description of Work to Be Performed: The Contractor shall deliver subsistence supplies (food items) on an as needed basis to the Navajo Area Indian Health Service, Chinle Comprehensive Healthcare Facility (CCHCF), Nutrition and Dietetics Department, Off Highway 191 & Hospital Drive, Chinle, AZ 86503.

II. Period of Performance: November 01, 2015 through September 30, 2016 with the option to extend to October 31, 2016. Obligations will be made on a monthly or quarterly basis.

III. Deliverable Schedule:

a. CCHCF will order supplies twice a week on Monday and Wednesday, and the Contractor shall deliver supplies within 24 hours or less from when order is placed.

b. Contractor shall make deliveries between the hours of 6:30am to 2:30pm, Monday through Friday.

c. Due to the lack of storage space, supplies will be ordered as needed with no minimum order amount mandated.

d. Since some subsistence items are perishable, then the contractor shall not have food in transit more than 4 hours.

IV. Special Requirements (e.g., security clearances, travel, special knowledge):

a. Contractor shall provide capability for CCHCF to place orders through an on-line website or by telephone.

b. Contractor shall provide on-line web-based reports on order history, account summary, etc.

c. Contractor shall provide customer service available through an on-line website or by telephone for inquiries concerning food allergies or ingredients.

d. Contractor shall provide an on-line web-based catalogue with a comprehensive variety of items.

e. Contractor shall have the capability to provide special order supplies if these supplies are not found in the company’s catalogue.

V. Performance Standards:

a. Contractor shall provide stellar customer service.

b. Contractor shall have experience in providing services to Federal Government or other large commercial/government entities within the last three (3) years of operation.

c. Contractor shall provide reference letters from entities with whom the contractor has worked within the last three (3) years of operation.

VI. Description of work to be performed – major duties and responsibilities:

a. Performs accurately as specified in this statement of work.

b. Ensures accuracy of orders.

c. Coordinates ease of ordering and returning process.

d. Delivers on time; delivery shall be done on the days specified in this statement of work.

e. Due to lack of storage space and the service to patients Nutrition and Dietetics Department at the Navajo Area Indian Health Service, Chinle Comprehensive Healthcare Facility will not accept late deliveries.

f. Plans timely deliveries and the pickup of items that need to be returned.

VII. Contractor furnished equipment – Contractor shall provide a large commercial grade coffee dispensing equipment as a part of this order, with timer included. Contractor shall will be responsible for providing maintenance and service to the equipment. Contractor shall provide maintenance service within 24 hours from the time the contractor was informed of the defective equipment.

VIII. Points of contact, inspection, and acceptance. Tanya Benally, Emma Davis, Glendora Davis, and/or Alina Hall, Supervisor. Depending on staff schedule inspection and acceptance individuals will be appointed at the time of delivery.
