Navajo Area Indian Health Service Pinon Health Service

Solicitation IHS1397372 MSPI Program

This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Navajo Area Indian Health Service, Chinle Comprehensive Health Care Facility (CCHCF) Health Promotion’s Methamphetamine and Suicide Prevention Initiative (MSPI) is requesting proposals from interested Offerors to provide consultant contracting services to provide education and experience with an emphasis in Methamphetamine Suicide Prevention. Review the enclosed instructions and provide this office with a proposal, utilizing the information contained herein. The Government contemplates award of a firm fixed price contract resulting from this solicitation. Services shall be performed in accordance with the attached Statement of Work (SOW). The Contractor shall provide an all-inclusive rate, and is responsible for all costs associated with providing said services, i.e. travel, per diem, lodging/housing, etc. Refer to the SOW and Pricing Schedule for more detail.

Chinle Service Unit - Indian Health Service in the Heart of the Navajo Reservation - Northern AZ. Chinle Comprehensive Health Care Facility provides care to approximately 11,000 patients - the majority of which are Navajo and many are traditional Navajo who still speak the language. A collegial group of providers offer services in primary care, women's health, counseling, dental, dietary and community nutrition, health promotions, lab, x-ray, native medicine, optometry, pharmacy, physical therapy, speech therapy, podiatry and public health nursing.

EVALUATION CRITERIA

The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the Best Value as defined by FAR 2.101. In determining Best Value, Price and other Evaluation Factors will be considered: Past Performance, Personnel, Qualifications, and Logistics. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value.

SUBMITTAL PROCESS

To be considered for award, your offer shall include the following documents:

1) Provide and complete the Pricing Schedule (attached form). The Government will consider competitive rates upon negotiation.

2) Past Performance: Provide 1-2 letters of reference. Include name, telephone number, email address of reference.

3) Experience: Provide evidence in experience in providing services of similar scope and complexity to Federal Government or other large commercial/government entities within the last three (3) years of operation. Be specific, provide project description, name of organization, contact point, telephone number, and e-mail address.

4) Personnel: Provide a list of personnel who will be involved in the project.

5) Qualifications: Provide evidence that the Contractor’s SME holds a degree, certification or has extensive knowledge or is viewed/reputable in the field that they are presenting on.

6) Logistics: Provide a preliminary schedule that shows project milestones/activity plan.

CLAUSES AND PROVISIONS

1) The Contactor shall have a background checks process for their employees who will have direct contact with children.

2) The Contractor shall indemnify the Government by waiving all liability against the Government.

Department of Health and Human Services Applicable Clauses:

352.202-1 Definitions. (JAN 2006) 352.224-70Privacy Act (DEC 2015)

352.237-70 Pro-Children Act. (Dec 2015) (a) Public Law 103-227, Title X, Part C, also known as the Pro-Children Act of 1994 (Act), 20 U.S.C. 7183, imposes restrictions on smoking in facilities where certain federally funded children's services are provided. The Act prohibits smoking within any indoor facility (or portion thereof), whether owned, leased, or contracted for, that is used for the routine or regular provision of: (i) kindergarten, elementary, or secondary education or library services or (ii) health or day care services that are provided to children under the age of 18. The statutory prohibition also applies to indoor facilities that are constructed, operated, or maintained with Federal funds.

(b) By acceptance of this contract or order, the Contractor agrees to comply with the requirements of the Act. The Act also applies to all subcontracts awarded under this contract for the specified children's services. Accordingly, the Contractor shall ensure that each of its employees, and any subcontractor staff, is made aware of, understands, and complies with the provisions of the Act. Failure to comply with the Act may result in the imposition of a civil monetary penalty in an amount not to exceed $1,000 for each violation and/or the imposition of an administrative compliance order on the responsible entity. Each day a violation continues constitutes a separate violation.

352.237-71 Crime Control Act of 1990-Reporting of Child Abuse (Dec 2015)(a) Public Law 101-647, also known as the Crime Control Act of 1990 (Act), imposes responsibilities on certain individuals who, while engaged in a professional capacity or activity, as defined in the Act, on Federal land or in a federally-operated (or contracted) facility, learn of facts that give the individual reason to suspect that a child has suffered an incident of child abuse.

(b) The Act designates covered professionals as those persons engaged in professions and activities in eight different categories including, but not limited to, teachers, social workers, physicians, dentists, medical residents or interns, hospital personnel and administrators, nurses, health care practitioners, chiropractors, osteopaths, pharmacists, optometrists, podiatrists, emergency medical technicians, ambulance drivers, alcohol or drug treatment personnel, psychologists, psychiatrists, mental health professionals, child care workers and administrators, and commercial film and photo processors. The Act defines the term child abuse as the physical or mental injury, sexual abuse or exploitation, or negligent treatment of a child.

(c) Accordingly, any person engaged in a covered profession or activity under an HHS contract or subcontract, regardless of the purpose of the contract or subcontract, shall immediately report a suspected child abuse incident in accordance with the provisions of the Act. If a child is suspected of being harmed, the appropriate State Child Abuse Hotline, local child protective services (CPS), or law enforcement agency shall be contacted. For more information about where and how to file a report, the Childhelp USA, National Child Abuse Hotline (1-800-4-A-CHILD) shall be called. Any covered professional failing to make a timely report of such incident shall be guilty of a Class B misdemeanor.

(d) By acceptance of this contract or order, the Contractor agrees to comply with the requirements of the Act. The Act also applies to all applicable subcontracts awarded under this contract. Accordingly, the Contractor shall ensure that each of its employees, and any subcontractor staff, is made aware of, understands, and complies with the provisions of the Act.

352.237-72 Crime Control Act of 1990-Requirement for Background Checks (Dec 2015)(a) Public Law 101-647, also known as the Crime Control Act of 1990 (Act), requires that all individuals involved with the provision of child care services to children under the age of 18 undergo a criminal background check. Child care services include, but are not limited to, social services, health and mental health care, child (day) care, education (whether or not directly involved in teaching), and rehabilitative programs. Any conviction for a sex crime, an offense involving a child victim, or a drug felony, may be grounds for denying employment or for dismissal of an employee providing any of the services listed above.

(b) The Contracting Officer will provide the necessary information to the Contractor regarding the process for obtaining the background check. The Contractor may hire a staff person provisionally prior to the completion of a background check, if at all times prior to the receipt of the background check during which children are in the care of the newly-hired person, the person is within the sight and under the supervision of a previously investigated staff person.

(c) By acceptance of this contract or order, the Contractor agrees to comply with the requirements of the Act. The Act also applies to all applicable subcontracts awarded under this contract. Accordingly, the Contractor shall ensure that each of its employees, and any subcontractor staff, is made aware of, understands, and complies with the provisions of the Act.

Health Insurance Portability and Accountability Act (HIPAA) The Indian Health Service (IHS) is required to comply with HIPAA in the provision of health care to IHS patients.

HIPAA was implemented by the U.S. Department of Health and Human Services, Office of Civil Rights, under the Code of Federal Regulations, Part 160 and 164. Accordingly, all health care providers, including contracted health care providers are required to comply with HIPAA requirements (Full text will be furnished upon request).

Youth Conference

Project Background:

The Department of Health and Human Services, Indian Health Services (IHS) has the responsibility for providing U.S. Federal Health services to American Indians and Alaska Natives (Al/AN).

American Indian and Alaska Natives and U.S Federal Government developed a special government-to-government relationship, which was established in 1787 based on Article I, Section 8 of the Constitution and has been form and substance by numerous s treaties, laws, Supreme Court decisions, and Executive Orders. The IHS provides a comprehensive health service delivery system for approximately 2.2 million American Indian and Alaska Natives who belong to 567 federally recognized tribes.

The Navajo Area Indian Health Services (NAIHS) administers health centers and hospitals providing health care to approximately 244,000 members of the Navajo Nation. The Navajo Nation is the largest land-based tribe in the United States with 25,000 square miles of land in northern Arizona, western New Mexico and southern Utah. The NAIHS provides Inpatient, Emergency, Outpatient, Public Health, preventive health services for members of the Navajo Nation and Southern Paiute Tribe. The Chinle Service Unit of NAIHS serves 16 communities of central Navajo entailing approximately 37,000 user population.

Location :

The Contractor shall provide services to the population with the Chinle Service Unit area.

Description Statement:

The mission of the Methamphetamine Suicide Prevention Initiative is to increase awareness, education and response of Substance Abuse (including methamphetamine, alcohol, marijuana, other substances), injuries that result from impairment, and suicide among Native American Youth . The HP program has created the Nahat'áh Bee'anooséél: Growing Leaders project to address staggering rates of suicide and suicidality by engaging youth in activities and events that promote resilience, positive self-image and promote cultural engagement thereby reducing risk behaviors associated with substance abuse and suicide. The Methamphetamine Suicide Prevention Initiative is managed by the Chinle Health Promotion Department, who is requesting contract services to provide education to meeting the following objectives:

a. To provide education and experiences that help students frame and develop their leadership philosophy.

b. To acquaint students with issues, needs, resources and other factors that influence our community's future.

c. To provide insights into the workings of local business, education, government, non- profit and other entities.

d. To inspire students to become ambassadors of community leadership among their peers.

e. To create a strong and diverse base of future leaders for the community.

f. To provide sustainable cultural and language specific interventions as protective factors.

Qualification Requirements:

a. Knowledge of coordinating youth conference.

b. Be able to maintain full attendance and participation during the entirety of conference.

c. Be inclusive and knowledgeable about cultural sensitivity and awareness.

d. Experience in working with site program in developing and coordinating multiple youth conference.

e. Must comply with HIPAA and Privacy Act.

f. Education: Bachelor of Science in education and/or 3 year experience in working with youth.

Government’s Quality Assurance:

The Chinle Comprehensive Health Care Facility Health Promotion’s MSPI Project Director, Annette Gonnie; will work closely with all the Contractor. Actively involves in the design of the programs so it benefits their clients. The Health Promotion Health Promotion secretary will also ensure the programs accountability by working closely with all the Contractor. The Government requires an evaluation on the activities.

Supervision

Contractor will function under general supervision of MSPI project director. Elements of Contractor evaluation will be based on:

1. Level of communication via written, email, phone, and response to request for assistance with evaluation

2. Invoices that are timely and accurately reflect services provided

Contractor Deliverables:

a. Contractor will provide a one-day youth cultural camps for 6 hours with four concurrent sessions in the morning and afternoon for 15-20 youth per session. Cultural camp will include adolescence right-of-passage family education. Provide rug weaving education for four months. Contractor will provide presenters that fulfill project objectives Cultural camp will be held during the months of March to September 2020 at three locations (Chinle, Pinon and Tsaile or adjacent location to these three sites). Contractor will provide materials, agenda and items needed for a successful youth conference.

b. SERVICE REPORT SUBMISSION: Upon completion of work and prior to leaving, the Contractor shall submit a Service Report to the Supervisory Point of Contact, and obtain the SPOC’s signature on the Service Report. This signature constitutes "Acceptance" by the Government, and is required for payment. The Service Report shall include the Contractor's name as shown on the purchase order, the purchase order number, a brief description of services performed, and the dates services were performed. This Service Report shall become a permanent part of Government file(s).

b. Housing and access to lHS equipment (i.e. computer, copier, fax, phone) will not be needed. Contractor will access to space to conduct meetings specific to this contract and activities.
