SCOPE OF WORK

CHINLE QUARTERS PLAYGROUND EQUIPMENT DEMOLITION AND REPLACEMENT

CHINLE HOSPITAL –FY 2019

I. Description of the work to be performed: Chinle Comprehensive Healthcare Facility requires demolition and removal of it’s aging playground structure in the housing area followed by resurfacing and installation of a new playground structure with shading.

II. Background: The Chinle Comprehensive Health Facility operates a 2500 SQ FT playgoround within it’s government‐owned quarters for the use and well‐being of children of government employees. The existing equipment is aged, deteriorating and unsafe for continued play, creating a liability for the facility should children sustain injuries on the current equipment. The existing rubber tile surfacing is peeling up from the ground and is inadequate to protect children from injury should they fall from the equipment. Maintaining a safe and comfortable playground is important to the Chinle Comprehensive Healthcare Facility mission of providing highest quality public health services. Chinle Comprehensive Healthcare Facility healthcare providers live in government owned and operated quarters. Providng recreational opportunities for these employees and their dependents is essential to recruiting and retaining the highest quality healthcare professionals to such a remote location. Such employees do not have access to housing or recreational facilities in the ruaral community of Chinle.

III. Location of Work: The Chinle Comprehensive Health Facility, Housing Compount, Off Highway

191 & Hospital Drive, Chinle Arizona, 86503

IV. Period of Performance: One time demolition, resurfacing and installation

V. Deliverable/Schedule: ARO 60 days

VI. Performance Standards/Requirements:

a. Contractor shall demolish and remove the existing playground surfacing including existing asphalt and rubber tile surface and dispose of materials and debris in an EPA approved location off‐site.

b. Contractor shall demolish and remove the existing playground structure and dispose of the structure at an approved location off‐site.

c. The contractor shall install a new playground structure to fit within the existing space.

The structure must meet or exceed Counsumer Safety Protection Commission safety standards for playground equipment and include elements suitable for play by children ages 2‐12 years.

d. Contractor shall install a shade structure to provide shading to the entire new playground structure.

e. Contractor shall install poured‐in‐place rubber surfacing to cover the entire playground area

(including the main structure and supplemental equipment) of sufficient thickness to protect against injuries from a fall of playground height.

f. Contractor shall provide bench seating for parents/caregivers of children using the equipment.

g. Contractor shall provide playground equipment and draininage layout options.

h. Contractor shall provide site restoration –minor landscaping, seeding, repairs, etc.
