BRAND NAME JUSTIFICATION

1. Contracting Activity: Department of Veterans Affairs (VA)

2. Description of Action: Brand Name Only. NCO 8 intends to procure the Zeiss Humphrey Field Analyzer 3 (HFA3) Model 840 as a brand name procurement action using a firm fixed price purchase order for delivery and installation. This equipment will enable The Department of Veterans Affairs (VA), VISN 08, Bruce W. Carter VA Medical Center to provide comprehensive eye examinations to render diagnosis and treatment.

3. Description of Supplies or Services: Zeiss HFA 3 Model 840 Visual Field Analyzer and Software. This is specialized eye examination equipment used for Visual field testing, also called perimetry, to check if the field of vision has been affected by glaucoma. This test measures side (peripheral) vision and central vision to evaluate the severity and to monitor progression of the disease. Salient characteristics meeting the Government’s minimal needs are: Software: Operating System: Windows 7-64 bit, Glaucoma Hemifield Test (GHT); STATPA, Kinetic Testing, SITA-SWAP/ Blue-Yellow Perimetry, RelEye; Digital Imaging and Communications in Medicine (DICOM) connectivity conformant with FORUM (minimum version 3.2) with FORUM Glaucoma Workplace (minimum version 2.0.3), Visual Field Index. Hardware: electrical,100-120v- 50/60 Hz, 4.0A, 500 Gb. Internal Storage, Dimensions - 18.11 L x 20.47 W x 22.83 H (inch). Fixation Controls: Video eye monitor, Heijl-Krakau blind spot monitor, Gaze Tracking, Head Tracking, Vertex Monitoring, Liquid Trial Lens, SmartTouch interface.

4. Statutory Authority: The statutory authority permitting this action is FAR 13.501(a)(ii):

Sole source (including brand name) acquisitions.

5. Rationale Supporting Use of Authority Cited Above: This new equipment replaces an existing model that has reached life expectancy and now follows a newer HFA 3 Model 840 to support operational needs. The equipment connects to the VA Vista Imaging System for backup and restore purposes; test results must be entered into the patient medical record as it is necessary to refer to the test results repetitively for continued patient treatment. Since there is no cure for glaucoma; patients with glaucoma need to continue treatment for the rest of their live; the disease can progress or change without warning. The equipment includes Digital Imaging and Communications in Medicine (DICOM) capability to transmit, store, retrieve, print, process, and display medical imaging information compatible to transfer data to the VA Vista Imaging System.

6. Efforts to Obtain Competition and Market Research: Market research was conducted to assess market brands capabilities to fulfill the requirement. There have been several brands identified that offer visual field analyzer units which do not provide operational attributes current used for historical merging of existing patient records at the Bruce W. Carter VA Medical Center. The Government has identified two predominant brands on the market: CARL ZEISS and HAAG STREIT, as well as several other brands. A brand name or equal solicitation will offer the opportunity for other brands to be reviewed for future consideration for the same or similar equipment to be purchased in the future.

7. Other Facts: Glaucoma progression and severity analysis is critical to the prevention of partial or complete vision loss. The Humphrey Visual Field Analyzer monitors all progression stages across the entire spectrum of glaucoma severity with prosperity software Swedish Interactive Thresholding Algorithm (SITA) providing an answer to the crucial question whether progression is happening at a rate that suggests elevated likelihood of visual disability during the patient’s expected lifetime. Clinical use indicates results are the fastest and most accurate visual field assessment to date using this software. The advantage of this testing algorithm also enables a direct historical comparison to patient's data to age-matched populations.

8. Sources: One source has expressed, in writing, an interest in the acquisition.

9. Actions to Increase Competition: A brand name or equal solicitation will offer the opportunity for other brands to be reviewed for future consideration for the same or similar equipment to be purchased in the future.

10. Technical and Requirements Certification: I certify that the supporting data under my cognizance, which are included in this justification, are accurate and complete to the best of my knowledge and belief.

11. Fair and Reasonable Cost Determination: I hereby determine that the anticipated price to the Government for this contract action will be fair and reasonable based on a comparison to historical VHA purchase found during market research.

12. Requirements Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
