1. How many employees will be covered by the EAP program?

Answer: There are approximately 750 employees.

2. What was the historical cost of the EAP program in 2015?

Answer: $7,650.00.

3. How many EAP counseling sessions were provided in 2015?

Answer: There were approximately 50 counseling sessions;

however, that is subject to change based on the need for employees to use EAP.

4. Is the rate stated $39,525.00 for the full 60 months of service?

Answer: The rate stated covered the full term listed in the solicitation.

5. What was the fee structure (i.e. capitated, fee for service, line item, etc.)?

Answer: The fee structure was a monthly line item fee structure.

6. What is the fee structure?

Answer: The fee structure is a monthly line item fee structure. A chart illustrating the line item structure is included in the solicitation.

7. Is the service currently provided face to face or is it telephonic service?

Answer: The EAP sessions are face to face.

8. Is the client satisfied with the service they are receiving from the current vendor?

Answer: “Yes.”

9. What is the historical Utilization rate?

Answer: Based on current rate is about 15% of our employees have used EAP services.

10. What method is being used to calculate the Utilization?

Answer: Used the number of employees that have used EAP divided by the number of employees eligible to use EAP.

11. What is the number of sessions covered under the EAP?

Answer: Three sessions are covered under the EAP.

12.What has been the rate of utilization of EAP services in the past three years?

Answer: There were approximately 50 counseling sessions. However, that is subject to change based on the need for employees to use EAP.

13. RFQ mentions on-site EAP services, how many hours per week are you expecting an on-site counselor?

Answer: We would like an on-site counselor for new employee orientation, which is every other Monday for two hours.

14. Would you like the EAP to include legal/financial consultation, and work/life web based services?

Answer: The customer did nor provided an answer. Please provide a quote for the services listed in the solicitation.

15. By “local “ telephone access, do you mean Maryland based, or does an 800 number that meets the stated staffing requirements satisfy this requirement?

Answer: An 800 number is fine, it does not have to be a local number.
