1.0 GENERAL INFORMATION

1.1 This is a non-personnel services contract to provide service maintenance for the two GeneXpert analyzers.

1.1.1 Description of services/introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personnel services necessary to perform service maintenance on the GeneXpert analyzers, as defined in this Performance Work Statement (PWS).

1.1.2 Background: The manufacturer provides maintenance services for both analyzers. Manufacturer sends a service technician for annual Preventive Maintenance (PM)s and calibration. Additionally, a service tech is available if any equipment malfunction occurs.

1.1.3 Scope:

The contractor shall accomplish services for annual PM and all other service types. Services include furnishing all labor, materials, transportation, and replacement parts to maintain two (2) GeneXpert analyzers.

1.1.4 Period of Performance (PoP): The PoP shall start 1 October 2023, with one 12 month base period and four 12 month option periods.

1.2 Administrative specifications

1.2.1 Place of performance: The work shall be performed at in the 45th Medical Group facility, building 1380, room 1151, Patrick Space Force Base (SFB) FL.

1.2.2 Recognized Federal holidays: Contractor is not required to perform services during the holidays but must be available to provide emergency services for the next duty day.

1.2.3 Hours of operation: The contractor is responsible for conducting business Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The contractor must always maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.

1.2.4 Emergency Services: On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours. The Contractor shall respond on-site to emergency calls within Twenty-Four (24) clock hours of notification by the Government.

1.3 Contractor Identification

1.3.1 Contractor personnel performing services in a contractor capacity in a Government facility are required to possess and wear an identification badge that displays their name and the name of their company. Ensure all those with whom the person interacts in any face-to-face dealings while supporting the Government understands that the person is contractor support personnel.

1.4 Claims of Government Misuse or Abuse:

1.4.1. If the Contractor claims any service calls are the result of Government abuse or misuse, the damage must be shown immediately to the Medical Equipment Repair (MER) section for verification before the repair is made.

Disagreements as to responsibility will be resolved by the Contracting Officer.

1.4.2 Additional charges for corrective work must be submitted by the Contractor in the form of a firm quote and approved by the Contracting Officer or his/her representative before work is begun.

1.5 Telephone Assistance:

1.5.1. The Contractor shall provide telephone assistance 24 hours a day, 7 days a week, excluding Government holidays for equipment covered under this contract. Telephone assistance shall be toll free to the Government and shall be manned by a qualified repair technician. The Contractor shall only accept calls from MER or laboratory personnel. A list of personnel authorized to place calls is attached.

1.6 Equipment Updates:

1.6.1. The Contractor shall furnish and install all software and equipment modifications normally provided by the manufacturer. Modifications shall be furnished at no additional cost under the contract.

These modifications can be provided during the next preventive maintenance inspections (PMI) or as a service call during the term of the contract.

1.7 Alternate Services: The Contractor shall not use subcontractors in the performance of the contract.

1.8 Liability: The Contractor will be held liable for errors and/or omissions during the performance of this contract.

1.9 Safety: The Contractor shall comply with Air Force safety standards at all times while on Government property.

IAW Air Force Occupational Safety and Health (AFOSH) Standards, safe-working practices shall be followed.

These include, but are not limited to:

a. Wearing of seat belts/shoulder harnesses at all times while driving on Patrick Space Force Base;

b. Observe maximum speed limits as posted on Patrick SFB;

c. Contractor personnel shall remove all jewelry prior to servicing mechanical and electrical circuits;

d. Any system left in a non-operational state shall be tagged “DO NOT OPERATE” and reported to MER for possible lock-out/tag-out procedures;

e. Comply with the no smoking policy within the 45 MDG;

f. Report any problems with building utilities to MER

g. Comply with all 45 MDG fire regulations.

h. Only hands-free cell phone use is allowed on Patrick SFB.

2.0 SPECIFIC TASKS

2.1 Work consists of furnishing all labor, materials, transportation, and replacement parts to maintain two (2) GeneXpert Cepheid computer, analyzer, serial numbers 110004413 and 839198, Equipment Control Number (ECN) 008265 and 009149, located in the 45th Medical Group facility, building 1380, room 1151, Patrick AFB FL.

2.2 Training/Experience Requirements: All Contractor personnel performing services under this contract, except for the original equipment manufacturer, Cepheid, shall show proof of factory training on this or like systems.

If factory training occurred more than three years ago, proof must be presented that the individual has been actively engaged in repairing the listed equipment for 12 of the last 18 months. Personnel must show credentials to the MER section prior to beginning work on the equipment listed in the contract. If, at any time, the Contractor is required to work on the listed equipment, and a credentialed technician is not available, the Government will contract with another vendor to provide that service, at the prime Contractor’s expense.

2.3 Spare Parts/Test Equipment Requirements: Prior to performing any maintenance on the systems, the Contractor shall provide proof that he/she has access to the original manufacturer’s spare parts and the recommended test equipment required for (PMI’s), to include calibration and corrective maintenance.

2.4 Contractor Reporting: During duty hours, 0730 to 1630 hours, prior to and after accomplishing any work specified in the contract, the Contractor shall report to the laboratory front desk to be escorted to the site of the equipment. After maintenance completion the Contractor shall send the service report to MER. The contractor shall notify MER during regular duty hours to schedule any work to be performed during non-duty hours.

2.5 Required Services

2.5.1. Scheduled Maintenance: The Contractor shall provide one (1) PM per year, as part of the annual calibration verification for both analyzers. The first PMI shall be performed during the month of July. The Contractor shall perform all tests, adjustments, lubrication, etc., in accordance with the manufacturer’s maintenance instructions and schedule. The Contractor’s preventive maintenance plan should, at a minimum, be equal to or greater than the manufacturer’s PMI.

2.5.1.1. The scheduling of preventive maintenance services shall be coordinated with and approved by the Noncommissioned Officer in Charge (NCOIC), MER, or Technical Supervisor (TSV). The PM shall include, but not be limited to, the following:

1. Perform operational checks of all systems and correct any deficiencies noted

2. Verify that the internal and external calibrations are within manufacturer’s specifications

3. Replace worn parts as required

4. Clean & lubricate mechanical parts in accordance with applicable manufacturer’s service manual

5. Perform an electrical check on instrument and all (as required) accessories included in the contract to ensure compliance with manufacturer’s specifications

6. Replace excessively worn or defective parts

7. Report user maintenance not being done to the MER section

8. Consult with the equipment operator regarding equipment performance

9. Use Air Force approved and supplied forms for documentation of calibration

2.5.2. Scheduling of Preventative Maintenance: All PM visits shall be coordinated with the TSV or NCOIC at least five days in advance.

2.5.3. Corrective Maintenance: The Contractor shall provide on-site repair support to correct all malfunctions which may occur. All tools, test equipment, parts, and supplies necessary to return the system to original operating specifications shall be the responsibility of the Contractor.

2.5.4. Response Time: Emergency services, as deemed by the MER section, shall be available Weekdays 0730 hours to 1630 hours excluding Government holidays. The Contractor shall respond on-site to emergency calls within Twenty-Four (24) clock hours of notification by the Government.

2.5.5. Point of Contact for Services: The Contractor shall provide a 24-hour point of contact: name, title, address, and toll-free number for maintenance support.

2.7 Documentation:

2.7.1 The MER section will report and monitor system performance, noting dates and times of notification of maintenance requests, type of request (emergency or routine PMI), response times and completion of work. The log will provide the Government data to determine if requirements of the contract are being met and will be available for review by the Contractor.

2.7.2 At the completion of each PMI or emergency service call, the Contractor shall provide the MER section with a legible service report the within one (1) business day. Failure to provide a service report will result in a delay in payment. Service reports shall include, but not be limited to:

1. Type of service; PMI, emergency service call;

2. Description of all services rendered during the visit;

3. Listing of any discrepancies that may still exist with any item on the contract;

4. Complete listing of replacement parts used by part number;

5. Completed service report shall be turned in to the MER section prior to the Contractor representative departing the 45 MDG;

6. Furnish documentation that shows calibration results.

2.8 Replacement Parts:

2.8.1 The Contractor shall provide and use, at no additional cost to the Government, only manufacturer’s new replacement parts in the performance of this contract. If required replacement parts are not immediately available to the Contractor during the PMI or service calls, the Contractor shall notify the MER section of the deficiency.

2.8.2 The Contractor shall keep or dispose of any removed parts the contractor’s representative replaces on the covered equipment item.

2.8.3. The Contractor shall not remove any United States Air Force owned equipment from the medical center for service at the Contractor’s establishment without the written consent of the NCOIC, MER and Commander, Medical Logistics Flight.

3.0 Qualifications/Considerations

3.1 Special Qualifications/Considerations: The contractor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the equipment/system shall be in accordance with the OEM terms and conditions.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 7 days of contract expiration.

52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 7 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (JAN 2023)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

“Payment request” and “receiving report” are defined in the clause at 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(b) Electronic invoicing. The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as authorized by Defense Federal Acquisition Regulation Supplement (DFARS) 252.232- 7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.sam.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order:

(1) Document type. The Contractor shall submit payment requests using the following document type(s):

(i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher.

(ii) For fixed price line items—

(A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting Officer.

(B) For services that do not require shipment of a deliverable, submit either the Invoice 2in1, which meets the requirements for the invoice and receiving report, or the applicable invoice and receiving report, as specified by the Contracting Officer.
