QUESTION & ANSWERS
Question 1. Is this a new requirement or existing requirement? Incumbent Contractor's name, period of performance, contract value. Incumbent employee length of services on this contract (if any).
Response: Behavioral Health Services is a follow-on requirement. The incumbent contractor’s name and employee information will be provided to the awardee.
Question 2. During the Phase-In period is contractor required to staff and perform on this contract at no cost to the government?
Response: Reference revised PWS paragraph 1.10.19 “Phase In/Phase Out Period“.
Question 3. When is government intending to make the award? There might not be enough time for Phase-In Period/Transition for anticipated start date of September 1, 2015.
Response: There are incumbent contractor Health Care Providers (HCP) presently working at Ft Belvoir Community Hospital. The total number of HCP on site will be provided to the awardee.
Question 4. Please clarify for "The technical information shall include the company’s name, address, individual names, CAGE Code, DUNS number on the first page." --- Should this be in header/footer or on the Cover Page of Volume I?
Response: The location of the company’s name, address, individual names, CAGE Code, DUNS number shall be placed anywhere on the first page of Volume I.
Question 5. Compensation Matrix --- Please inform if we are required to provide hourly rate for all elements of the Compensation Matrix (Base Salary, FICA, FUTA, SUTA, etc.), or is this to be Annual Amounts for each Period?
Response: Reference compensation matrix posted on www.fbo.gov. The compensation matrix will be reposted to include an attachment number (Attachment 9).
Question 6. Is Contractor Manpower Reporting to be a stated on the Compensation Matrix? If yes, should it in hours or a flat amount and on which column should it be ported in the Compensation Matrix Worksheet?
Response: Reference Compensation Matrix, Contractor Manpower Reporting (CMR) is listed under column labeled “Labor Category”. The CMR unit of issue is the same as stated on CLINs 0012, 1012, 2012, 3012 and 4012.
Question 7. Is Compensation Worksheet also to be incorporated in Volume I sub factor1? Or to be provided as separate attachment?

Question 8. Please confirm that the Excel version of Compensation Matrix is not counted towards 10 page limitation for Volume I (Sub factors 1 and 2).
Response: The Compensation Matrix counts toward the 10 page limitation for Volume I.
Question 9. Is Volume I to be submitted in 2 parts, that is, separate volume for each Sub factor1 and Sub factor 2?
Response: Reference ADDDENDUM TO 52.212-1, PART IV: RFP Documents (e), Volume I must be submitted separately by Subfactor.
Question 10. Past Performances --- Are all the 3 Past Performance to be provided by Prime or can it be a combination between Prime and Major Subcontractor?
Response: Reference ADDENDUM TO 52.212-1, Part II: Past Performance (a) and (b)(1) excerpt:
(a) “The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor.
(b)(1)“Offerors shall submit a list of three (3) or more contracts or subcontracts completed in the last three years including contracts and subcontracts currently in process, which are similar in nature to the requirement”.
Question 11. Out of 3 Past Performance requirement, can we submit 2 Past Performance for a Major Subcontractor on our team?
Response: Reference response to question 10, offeror shall submit a list of “3 or more” contracts or subcontracts.
Question 12. How many Past Performances must a Prime provide when using a subcontractor and team members.
Response: Reference ADDENDUM TO 52.212-1, Part II: Past Performance (b)(1), see question 10.
Question 13. On Call Coverage --- Are all the positions required to be on-call or only select positions? Only select positions identified by unscheduled hours on the clin i.e., Psychologist unscheduled, NP unscheduled etc.
Response: Reference PWS paragraph 1.10.1.12 “On Call Coverage”. All HCP who are scheduled to be on call (issued a pager) are required to be on call.
Question 14. Section 1.15 -- Mobilization and Other Contingency Planning --- This is contradictory to Section
1.10.1.15 - Overtime is not authorized. Please clarify as Overtime pay for WD personnel has an impact on our pricing.
Response: Section 1.15 -- Mobilization and Other Contingency Planning does not require overtime. If hospital staff are deployed the contractor staff would be required to fill these positions. If additional HCP are required by the Government a modification shall be prepared to cover the additional cost.
Question 15. Are 8(a) JV allowed to submit their response to this solicitation?
Response: Yes
Question 16. Are there any limitations or requirements for page margins for each volume?
Response: No
Question 17. In the Sanitized Version of our Volumes, are we required to mask our company/team name in the body of the volumes also? Or just on the cover page, footers, headers, etc.?
Response: Reference Addendum to 52.212-1, see excerpt below:
Proposal Copies: Offerors shall provide two separate electronic copies of their proposals. Copy One (1), Not- Sanitize; Copy Two (2) Sanitized. Copy two shall NOT contain the name of or reference to the Offeror, any Joint Venture Partner, or any Subsidiaries. This copy, will be used in the Evaluation of Proposals. Offerors are advised hereby that without a Sanitized copy of their Proposal their submission must be deemed as Non-Responsive, as the proposal, cannot be evaluated. FORMAT: Submit your proposal in four (4) separate volumes, Technical must be labeled by Sub Factors 1 and 2.
Question 18. Please provide the option year prices for the contract.
Response: Current contract pricing cannot be released. All offerors will submit option year pricing in accordance with the solicitation.
Question 19. I noticed that on the Solicitation HT0014-15-R-0016 SF1449 for naics code 621330 the size standard populated is 7.5k; however on the SBA naics size standard is listed as 7.5M - can you confirm if this is correct and that the bid is limited to 8(a) companies who have an annual revenue of 7.5k or less?
Response: The size standard located in block 10 of the SF 1449 is changed by amendment from: 7.5k to: 7.5M.
Question 20. Is there currently an incumbent? If so are all positions currently filled?
Response: Yes, there are incumbents. This information will be provided to the awardee
Question 21. The Compensation Matrix does not automatically calculate the pricing totals. Are we allowed to add formulas or will an updated Compensation Matrix be released?
Response: The compensation matrix will be revised to include the Attachment number and to add a column for total annual cost. The compensation matrix will not be revised to include formula’s, this is the responsibility of the offeror.
Question 22. Are Unscheduled Hours considered on-call hours or are they emergency/back up hours?
Response: Unscheduled hours are hours designated to cover if the provider who is on call has to come in to the facility.
Question 23. Is on-call just pertaining to the Child Clinical Social Worker?
Response: No, On Call covers all providers who will be scheduled to pull on call under the unscheduled clins.
Question 24. Are the Unscheduled Hours for Psychologist & Social Worker Child or Adult?
Response: The unscheduled is for both adult and child.
Question 25. Is there an incumbent contractor for these services? If so, can you please provide the company name, contract number, and contract award value for the base and all option periods?
Response: Reference response to question 1.
Question 26. Are all positions currently filled? If not, can you please provide a list of vacancies by specialty? What is the historical fill rate for each CLIN?
Response: Reference response to question 20.
Question 27. For any positions currently filled, can you please provide a list of hire dates by specialty?
Response: Information pertaining to incumbent HCP employees will be provided to the awardee.
Question 28. Referencing the incorporated Service Contract Act and Wage Determination Tables, can you please provide what CLIN(s) is applicable and what SCA code(s) the Government is conforming the respective CLIN(s) to?
Response: Psychiatric Nurse Practitioner, reference the Department of Labor SCA Directory of Occupations.
Question 29. Will the Government consider a Best Value Trade-off basis for award instead of LPTA? A Best Value Trade-off will allow the government to consider a proprietary, unique, and/or innovative approach as well as the performance record of a contactor during evaluation to ensure continuity-of-care, quality-of-service, and risk mitigation for this most important mission and the affected beneficiaries.
Response: Reference Addendum to 52.212-1 (g) Basis of award will remain LPTA.
Question 30. Are the above positions that are designated SCA covered currently filled with incumbent staff?
Response: The only position SCA covered is the Psychiatric Nurse Practitioner.
Question 31. If yes, what is the contract anniversary date/tenure of the incumbent contract staff?
Response: Reference response to Question 27.
Question 32. In Section 52.212-1 Instructions to Offerors—Commercial Items on page 121 of 149
Q3.1: Should the volumes be sent in one email or four separate emails?
Response: Volumes can be sent in one email or 4 separate emails, but all email must identify the volume submitted. It is the offerors responsibility to request verification from Ms. Te-o-Lewis at faatalivale.m.teo-lewis.civ@mail.mil to confirm receipt.
Question 33. What labor categories are to be used for pricing “On-Call” services for CLIN 0011, 1011, 2011, 3011 and 4011, respectively?
Response: Psychiatrist, Psychologist, Nurse Practitioner, and Social Workers, Child/Adult.
Question 34. Section 1.10.1.2 Page 74 states “The POC will be responsible for ensuring replacement personnel are dispatched to the MTF’s within 2 hours of notification that a replacement is needed”. What is the intent of the requirement? The Solicitation requires staff working at the MTF to have immunizations, credentials, licenses, security clearances, and medical privileges (granted by the MTF Commander), etc. on file prior to start of work.
For example the review and approval of new employees credentials could take up to 10 days. Therefore, it may be impractical to expect the Offeror to dispatch a replacement (new) Adult Psychiatrist or any of the other highly credentialed labor category employees within 2 hours of notification.

Response: Due to the credentials and security process, PWS paragraph 1.10.1.2 will be modified to read, “The contractor will be responsible for backfill of a candidate to cover HCP who becomes absent for an extended period of time of 45 days or more”. See amendment 1.
Question 35. Are Credentials, Licenses, and Immunization Records for each incumbent employee transferable to a newly awarded contractor?
Response: The practice is that the contractor must present on their company letterhead the acceptance of all current credentials and certifications etc., and assumes ownership of said documents.
Question 36. Is there an incumbent currently performing the behavioral health services? If yes, can you name the incumbent? Can the incumbent submit a response to the Solicitation?
Response: Yes, there is an incumbent. The incumbents information will be provided to the awardee. This requirement is an 8(a) set aside and will accept all offers submitted in accordance with solicitation.
Question 37. A Department of Labor Wage Determination is provided in Attachment 8. Which positions are to be priced using the Wage Determination schedule?
Response: Reference response to question 30.
Question 38. Are there incumbent HCPs? If so, can the Government clarify the positions for which there currently are not incumbent HCPs, i.e. the current vacancies? (Background: Because of the quick turnaround time after contract award in which the Government requires resumes, it is in the Government's interest to let offerors know this data so that industry will appropriately price their offers to meet the Government's requirements, particularly given the extremely narrow seven-working-day turnaround time to present certain documents under PWS 1.14 in the instances where there are not incumbent HCPs.)
Response: Reference question 20.
Question 39. On page 122, regarding Part I: Technical Information, the RFP states "The technical information shall not exceed 10 pages", which would seem to cover both Subparts 1 and 2. On page 124, the RFP also seems to limit "both the Compensation Matrix and the Recruitment & Retention Plan" to 10 pages. Given that the Government-provided "Compensation Matrix" itself already totals more than 10 pages, could the Government please clarify whether the ten-page limit applies only to offerors' written technical submission under Subfactor 2?
Response: Yes, reference solicitation Addendum to 52.212.1 Part IV RFP Documents (g) chart, Compensation Matrix and the Recruitment and Retention Plan combined total 10 pages.
Question 40. On page 133, the RFP reads, "The Government will evaluate the Retention and Retention plan, particularly to determine the extent to which the plan and its methodology will fill required positions with quality HCPs and provide for low turnover rates." What is the Government's acceptable rate of turnover for this requirement?
Response: The HCP fill rate of 95% is not stated in the solicitation, an amendment will be prepared to incorporate.
An amendment will be processed to incorporate.
Question 41. On page 133, the RFP reads, "The plan will be evaluated as to the degree the plan presents a comprehensive and realistic approach to retain qualified HCPs and maintain the required HCP fill rate." What is the required HCP fill rate?
Response: The solicitation will be revised to include a 95% fill rate, an amendment will be forthcoming.
Question 42. Could the Government clarify if it wants offerors to submit Part III on the SF 1449 continuation sheets?
Response: Yes
Question 43. In the Sanitized Technical, is replacing references to the offeror with the offeror's CAGE code acceptable
Response: No.
Question 44. Does the Government have a preferred naming convention for the file submissions?
Response: Yes, reference Addendum to 52-212-1 Part IV: RFP Documents (g):

Question 45. Can the Government indicate how many of these requested positions are currently filled by incumbent providers/staff? How many years’ experience do the incumbents have for the solicited positions?
Response: Reference question 20.
Question 46. Reference PWS Section 1.10.1.2: Section states that “[t]he POC will be responsible for ensuring replacement personnel are dispatched to the MTFs within 2 hours of notification that a replacement is needed.”
There is no other mention of backfill services in the PWS, please confirm if backfill will be required for positions.
Response: The paragraph has been modified to: POC will recruit backfill for periods of absence of 45 days or more.
Question 47. Reference PWS Section 1.13.2: Section requires that Contractors “must submit credential package to the Credentialing Coordinator and the COR for approval.” Is this a full credential package or a preliminary package for initial approval?
Response: There is only a Psych RN, an error was made and the Child Psychiatric Clinic Nurse Specialist portion of the qualifications/tasks will be omitted.
Question 48. Attachment #1 provides qualifications & responsibilities for a Child Psychiatric Clinical Nurse Specialist/Advanced Practice Nurse, however, this position is not requested to be priced in the provided CLINs.
Was it the Government’s intention to include this position?
Response: The reference to Advanced Practice Nurse, will be deleted. An amendment will be forthcoming.
Question 49. For the On-call services it is stated on pg. 76, “Compensation for ON-CALL Coverage on weekdays will be compensated at a flat-rate fee per day.” Does this mean that they will receive a certain daily rate no matter how many hours are performed?
Response: This means that the provider is paid a flat rate for carrying a pager during the on-call hours which is outlined in the PWS. If the HCP is called in to the facility then they are paid under the unscheduled hours clin.
Question 50. On-Call Billing is done under the unit “each”. Does this mean per hour or per occasion, to perhaps line up better with a daily flat rate fee that may need to be paid?
Response: On-Call is only the carrying of the pager fee for that period of being on-call.
Question 51. What positions is the On-call needed for? If multiple should there be multiple CLINS to bill against?
Response: The on-call CLIN covers the designated unscheduled CLIN and is not broken out in multiple CLINs.
Question 52. It is stated that no overtime is allowed and that call back it to be paid at the regular rate. However will overtime be allowed in the call back would bring an individual over the weekly allotment, would overtime need to be priced/added?
Response: No
Question 53. Schedule B - Pricing contains a number of CLINs for Unscheduled hours for the Direct Healthcare Providers to include the Psychiatrist, Psychologist, Nurse Practitioner and Clinical Social Worker. Para 1.10.1.12 provides a description for on call coverage. There is no description provided for Unscheduled Hours. Please provide a response as to when Unscheduled (Work) Hours may be requested by the Government.
Response: Unscheduled hours are not known because they are applied only if the contractor has to physically report to the site to work while on call.
Question 54. Does this requirement represent a new staffing effort? If not, please advise on the Incumbent and incumbent contract value.
Response: Reference response to question 20.
Question 55. Para 1.2 Scope of Work states that the Direct Healthcare Providers will perform addition and behavioral health services only. In the event that additional Behavioral Healthcare services may be provided, how will the Government procure these services?
Response: No additional services outside of the PWS will be added.
Question 56. Para 1.10.1.2 addresses notification of absences to the COR/Contracting Officer. This para further states that the contractor is responsible for providing replacement personnel to the MTF within 2 hours of notification that a replacement is needed. Does this 2-hour staff backfill apply to each Direct Healthcare Provider labor category? Please provide an example as to when a replacement "may be needed"?
Response: With the absence of any HCP on the contract for more than 45 days, the contractor will be required to backfill the position.
Question 57. Attachment #1 Professional Qualifications /Responsibilities combines the Adult/Child Psychologist qualifications and responsibilities but list no delineation or difference between the two Direct Healthcare Provider labor categories, primarily the Child Psychologist. Further, there are 2 individual CLINS for each labor category.
Please clarify.
Response: The qualifications come through previous experience with child and adolescents work experience. The Adult/Child Psychologist remain two separate CLINs.
Question 58. Section entitled Clauses Incorporated by Reference (page 120 of 149) letter (d) that the Government will make a "Best Value" determination. Letter (f) - Basis for Award 1st sentence states Lowest Price Technically Acceptable contract award basis. Is the contract award Best Value" to the Government of Lowest Price Technically Acceptable? Please clarify.
Response: Basis of award is “Lowest Price Technically Acceptable”.
Question 59. Are the compensation part and the recruitment section to be submitted as one document, or two separate files?
Response: The compensation and recruitment and retention plan are to be submitted in two separate files.
Question 60. Is the 10 allowed technical pages including the comp matrix and recruitment section?
Response: Reference Addendum to 52.212-1 Part IV RFP Documents (g). -
Question 61. If the comp matrix and recruitment section are to be submitted in two files, will the Govt. consider accepting one combined file to make the best use of our 10 pages.
Response: The Compensation Matrix and Recruitment and Retention Plan Volume I must be submitted in 2 copies in accordance with solicitation.
Question 62. On page 122 of 149, PART II: PAST PERFORMANCE, (a), “The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor”, is it the government’s intent that each past performance reference be no more than one page for a Past Performance Volume II not to exceed 10 pages total?
Response: Reference response to question 10, offeror shall submit a list of “3 or more” contracts or subcontracts.
Question 63. Page 119 - Section L.3: We understand that we cannot use former DoD/DHA Employees to support proposal development. However, are there restrictions to using these individuals as proposed personnel?
Response: There is one restriction in using former DoD/DHA employees as proposed personnel: the company is to certify that it will create a "fire-wall" between the former DoD/DHA employee and the company in which the employee is not to discuss matters with the company which the employee was personally and substantially involved in during their DoD/DHA employment.
Question 64. Page 122 - Sub Factor 1: Does the populated compensation matrix counted in the page limitation?
Response: The Compensation Matrix counts toward the 10 page limitation for Volume I.
Question 65. Page 122 - Past Perf (a): Past Performance can be submitted earlier than the remainder of the proposal. Is this a requirement? What is the due date?
Response: The Questionnaire due on or before 10:00am EST 22 July 2015. Page 123 Solicitation.
Question 66. Page 122 - Past Perf (a): the last sentence of this paragraph doesn’t make sense ("The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor.”) Please clarify.
Response: Reference response to question 10, offeror shall submit a list of “3 or more” contracts or subcontracts.
Question 67. Compensation Matrix, Instructions. May the Offerors add other applicable Indirect Costs (such as Overhead, Material Handling, etc.) as columns in the Compensation Matrix to comply with their applicable Cost Accounting policies and procedures?
Response: No, a revised compensation matrix will be forthcoming.
Question 68. Page #76, Clause 1.10.1.12 On Call Coverage – Please clarify which behavioral health professionals are required to be involved with On-Call services.
Response: Specialties that have a CLIN that specify unscheduled hours will have On Call Services.
Question 69. Page #86, #89, and #95 states the contract service provider shall support local and remote Solider Readiness Processing (SRP) events that prepare large numbers of service members for mobilization deployment, demobilization and redeployment in clinical and non-clinical settings. Please clarify what is meant by local and remote events.
Response: The solicitation will be revised to remove “remote”. Local refers to local events such as on post or
WRNMMC.
Question 70. Page #91 & #92 references a NLN Psychiatric Medication Test for Adult and Child Psychiatric Clinical Nurse Specialists or Advanced Practice Nurses. Are the Psychiatrists, Psychologists, and Social Workers required to take any tests? If so, please provide specific details.
Response : No, just the Psychiatric Nurse Practitioner because they dispense medication.
Question 71. Please confirm that a DEA is required for the Psychiatrist and Adult/ Child Psychiatric Clinical Nurse Specialists or Advanced Practice Nurses.
Response: Yes, providers must have an active DEA license to prescribe medication.
Question 72. What is the breakdown between inpatient and outpatient services for CLIN 0001, 0003, 0004, 0006, 0008, and 0009?
Response: Inpatient consists of providing patient care on lock wards Adult Inpatient Psychiatry Service (4-North) Addictions Residential Treatment Center (4-South)

Question 74. On page 124 of the solicitation, the summary of electronic proposal documents to be submitted states: “The technical information shall not exceed 10 pages (combined total for both the compensation matrix and the recruitment & retention plan”
Response: The Compensation Matrix does not count toward the 10 page limitation for Volume I.
Question 75. Can you please explain how sub factor 1 – Compensation Matrix will count against the page limit since it was attached as an excel file? Ex: 1 tab = 1 page?

Question 76. Instructions for the Past Performance states: “Offerors shall submit a list of three (3) or more contracts and subcontracts completed in the last three years” Is there a maximum number of references that can be submitted?
Response: Reference response to question 10, offeror shall submit a list of “3 or more” contracts or subcontracts.
Question 79. Ref. PWS (Atch 1) Specific Duties/Tasks. Psychiatrists, Psychologists, and Adult LCSW. "The contract service provider shall support local and remote Soldier Readiness Processing (SRP) events...” Does "remote" SRP event mean outside the National Capital Region, e.g. other than Ft Belvoir or WRNMMC?
Response: No, SRP will not be outside of FBCH or WRNMMC

PWS (Atch 1) Specific Duties/Tasks, Professional Qualifications / Responsibilities Adult Psychiatric Clinical Nurse Specialist or Advanced Practice Nurse (para. c., pg 91) and Child (pg 92). Question: Under "Certification", why does the following sentence begin, "If certification is required. . ."? It appears in both cases that the PWS requires certification. Is this correct?
Response: Yes certification is required for both.
Question 81. Ref. PWS (Atch 1) Specific Duties/Tasks. Professional Qualifications / Responsibilities, Adult Clinical Social Worker (pg 93) and Child (pg 96). What are the differences in qualifications between Adult LCSW and Child LCSW, as none are listed in the PWS? Is any specific education, training, certification, or experience specific to treating Child patients required?
Response: Child LCSW must have 2 years or more of child work experience. There is no specific education, training, or certification.
Question 82. Ref. SF 1449 Continuation Sheet CLIN descriptions. The CLINs state Behavioral Health Services at Fort Belvoir Community Hospital/DeWitt Healthcare Network. Is there any requirement for staff to be assigned to, or travel to, the Family Health Centers in Fairfax, VA and Dumfries, VA, which are part of the DeWitt Healthcare Network?
Response: Yes, some of the requirements duty station will be Fairfax and Dumfries, VA Behavioral Health Services at those locations.
Question 83. The RFP states "Offerors shall provide two separate electronic copies of their proposals. Copy One (1), Not-Sanitize; Copy Two (2) Sanitized." Are we to sanitize all 4 proposal parts, or only the technical? Also, are we to include the Cage code or any other distinguishing item on the sanitized versions?

Question 84. Can you offer more detail about Clin #11 (pg. 10 of 51)? Is this an on-call psychiatrist or psychologist?
Is it on call hours for different position in the solicitation?
Response: Reference revision CLIN 0011 description, this is on-call for all of the unscheduled hour CLINS.
Question 85. On page 122 of the solicitation and under the heading Past Performance, section (a) states that: “… The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor”.
However, page 124 of the solicitation and under the heading “A summary of electronic proposal documents to be submitted”, it states that the Past Performance must be 10 pages.

(c) This acquisition is being conducted utilizing the policies and procedures of FAR Parts12 and 19 in conjunction with appropriate parts of FAR 15 for the acquisition of commercial services. The Government intends to award a firm fixed-price personal services contract under the authority of Section 1091 and 1089 of Title 10, U.S.C.
Offerors are instructed to ensure that their offered rates. DO NOT include costs associated with medical malpractice liability insurance. Offerors must quote a firm-fixed price for each Contract Line Item.
(d) The Government intends to award a contract without further communicating with contractors. Consequently, contractors are highly encouraged to quote their best technical and pricing quotes in their initial submissions.
However, the Government reserves the right to communicate with any or all contractors submitting a technical and price quote, if it is determined advantageous to the Government to do so. This statement is not to be construed to mean that the Government is obligated to communicate with every offeror. A contractor may be eliminated from consideration without further communication if its technical and/or pricing proposal are not among those
Contractors considered most advantageous to the Government based on a best value determination.
(e) Any questions with respect to the solicitation shall be submitted not later than 08 July 2015 @ 10:00 a.m. EDT to Mrs. Fa’atalivale (Vale) Te’o-Lewis at faatalivale.m.teo-lewis.civ@mail.mil. Responses to questions will be posted on www.fbo.gov via RFP amendment.
(f) BASIS FOR AWARD.
Award will be made on a Lowest Price Technically Acceptable (LPTA) basis. Award will be made to the Offeror provided its technical proposal conforms to the requirements of this solicitation; it has an acceptable record of past performance; its price is determined to fair and resoable by the contracting officer; and the contracting officer determines the offeror is responsible. The Contracting Officer intends to award without discussion, but reserves the right to do so in his or her sole discretion.
(g) Data Universal Numbering System (DUNS) / CAGE Code. The offeror shall obtain these said Codes and submit them to Contracting Center. Registration in the System for Award Management (SAM) https://www.sam.gov/portal/public/SAM/ and the Online Representations and Certifications, if you are a U.S.
business entity. Award cannot and shall not be made to an offeror without these codes.
(h) Proposal Preparation Costs. The Government will not pay any costs incurred in the submission of an offer, in making any necessary studies for the preparation thereof, or for any visit(s) the Contracting Officer may request for the purpose of clarification of the offer.
(i) The offeror is responsible for ensuring that its proposal is complete. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of any stated claims. The proposal shall not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to perform the requirement. Offerors shall assume that the government has no prior knowledge of their experience and will base its evaluation solely on the information presented in the offeror's proposal.

(j) Protests received at a level higher than the contracting officer will be referred to the contracting officer. Protestor may request a review by the contracting officer or an independent review at a level higher than the contracting officer. Requests for appeals or independent review will be addressed to the contracting officer for referral to the review authority. Questions on this process will be referred to the cognizant contracting officer identified in the solicitation.
(k) The non-FAR Part 12 discretionary FAR, DFARS, AFARS, and LOCAL provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision

PART I: TECHNICAL INFORMATION: The information provided by the offeror is a significant factor in the
Government’s determination of technical acceptability as it pertains to the offeror’s ability to hire qualified personnel as well as to provide continuous service based on a sufficient incentive to retain those qualified personnel.
Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked. Within the technical portion of the offeror’s proposal, the offeror shall provide:
Sub factor 1: Compensation. See attachment entitled “Compensation Matrix”. Offerors are directed to fill-in / populate the “Compensation Matrix” that includes employee base salary amount, employee fringe benefits, as well as company profit, overhead, and G&A for the geographic area of service (Fort Belvoir, Va)). If the salary is less than the amount paid in the local area, the offeror must demonstrate that its “Compensation Matrix” is adequate to recruit and retain quality health care providers and ensure uninterrupted performance of services throughout the life of the contract. The escalation factor or other means for price changes in the Option periods must be addressed within the narrative of the “Compensation Matrix”.
Sub factor 2: Recruitment and Retention Plan. The Recruitment and Retention Plan shall describe the offeror’s policies, strategies and procedures to recruit those health care providers with the qualifications as stated in the PWS.
Offerors shall describe the methods used and describe the proposed employee benefits provided to retain quality health care providers. Offerors shall describe how they will ensure proper certification, including training and educational plans offered. Offerors shall describe their plans to fill positions that become vacant during performance for the geographic area of service (Fort Belvoir, Va)
The technical information shall not exceed 10 pages and must be submitted in Microsoft Word compatible format. The technical information shall include the company’s name, address, individual names, CAGE Code, DUNS number on the first page.
PART II: PAST PERFORMANCE
(a) Offerors shall submit the information requested below as part of their proposal for both the offeror and any proposed subcontractors for subcontracts expected to exceed 25% of the requirement. The information may be submitted prior to other parts of the proposal in order to assist the Government in reducing the evaluation time. The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor.
(b) (1) Offerors shall submit a list of three (3) or more contracts and subcontracts completed in the last three years, including contracts and subcontracts currently in process, which are similar in nature to the requirement.
(2) The contracts and subcontracts listed may include those entered into with Federal, State, local governments and commercial businesses, which are similar in scope, magnitude, relevance and complexity to the requirement described in the RFP. Include the following information for each contract and subcontract listed:
(a) Name of contracting activity.
(b) Contract number.
(c) Contract title.
(d) Contract type.
(e) Brief description of contract or subcontract and relevance to this requirement.
(f) Total contract value.
(g) Period of performance.
(h) Contracting Officer, telephone number and e-mail address, if available.
(i) Program manager/project officer, telephone number and e-mail address, if available.
(j) Administrative Contracting Officer, if different from (h) above, telephone number and e-mail address (if available).
(k) List of subcontracts (if applicable).
(c) Offerors should not provide general information on their performance on the identified contracts and subcontracts. General performance information will be obtained from the references.
(1) Offerors may provide information on problems encountered and corrective actions taken on the identified contracts and subcontracts.
(2) References that may be contacted by the Government include the Contracting Officer, program manager/project officer, or the Administrative Contracting Officer.
(3) If no response is received from a reference, the Government will make an attempt to: contact another reference identified by the offeror, contact a reference not identified by the offeror, or complete the evaluation with those references who responded. The Government shall consider the information obtained from other sources when evaluating an offeror’s past performance (such as PPIRS, FAPIIS, etc.).
(4) Attempts to obtain responses from references will generally not go beyond two telephone messages and/or written requests from the Government, unless otherwise stated in the solicitation. The Government is not obligated to contact any of the references identified by the offeror. It is the offeror’s responsibility to ensure that their references submit the
Questionnaire to the Government. The Government is not responsible for the failure of any source to respond to a request for a past performance assessment on a timely basis.
(c) Offerors must send a Past Performance Questionnaire (see Attachments to the solicitation) to each reference listed in their proposal to assist in the timely processing of the past performance evaluation.
Offerors are encouraged to consolidate requests whenever possible (i.e., if the same reference has several contracts, send that reference a single notice citing all applicable contracts). Offerors may send Past
Performance Questionnaire to references with copies forwarded to the Contracting Officer.
(1) If an offeror has no relevant past performance history, an offeror must affirmatively state that it possesses no relevant past performance history.
(2) Past Performance Questionnaires should be sent to the individual references on or around the tenth working day after the proposal release date, but no later than 5 working days prior to the proposal closing date. The offeror may forward a copy of the Past Performance Questionnaire to the Contracting Officer simultaneously with mailing to references.
(e) The questionnaire should be emailed directly to faatalivale.m.teo-lewis.civ@mail.mil ON OR
BEFORE 10:00AM EST 22 July 2015. The offeror should instruct POCs not to send a copy back to the offeror, but to faatalivale.m.teo-lewis.civ@mail.mil as listed on the questionnaire. Please request that any questionnaire submitted includes the following information in the subject line so the email is not misconstrued as SPAM or otherwise deleted: Past Performance Questionnaire HT0014-15-R-0016.
(g) Past performance information may be used for both responsibility determinations and as an evaluation factor for award. The Past Performance Questionnaire included with the Attachments will be used to collect information on an offeror’s performance under existing and prior contracts/subcontracts for services similar in scope, magnitude, relevance and complexity to this requirement in order to evaluate offerors’ consistently in accordance with the past performance evaluation factor set forth in the Evaluation…
