Statement of Work

Preventative Maintenance of Cardiovascular and Weight Equipment

At

USSS Fitness Facilities

1. Description of Work:

Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to inspect and repair cardiovascular and resistance exercise weight equipment at six (6) United States Secret Service (USSS) Facilities located in the greater Baltimore-Washington Area.

The USSS Fitness Center Director or designee for each respective facility will approve all repairs with the contractor in person or by phone prior to beginning any/all work. All replacement parts required for maintaining equipment will be ordered within forty eight (48) hours after the service call.

4. SERVICES TO BE PROVIDED BY THE CONTRACTOR:

Preventative Maintenance The contractor shall provide scheduled preventative maintenance in accordance with the equipment manufacturer specifications for equipment indentified herein.

Preventative maintenance shall be accomplished within the prescribed timeframe as recommended by the equipment manufacturer. This consists of the care and servicing of the equipment for the purpose of retaining the equipment in a serviceable condition in accordance with (IAW) the manufacturer’s maintenance specifications. The specifications are listed in the manufacturer’s technical literature.

This inspection includes, but may not be limited to, checking for proper operation, detection, and correction of incipient failures, either before they occur or before they develop into major problems. This includes adjustments, calibration, and interior cleaning of all equipment, and replacement of all damaged or deteriorated parts.

Unscheduled Service Calls

The contractor shall respond to unscheduled service calls within forty-eight (48) hours of notification by the USSS Fitness.

Written Record of Service The contractor’s service technician will prepare a service report for each service call (schedule or unscheduled) to include work accomplished, work pending, and any other recommendations in regards to the equipment. A signature from the USSS Fitness Center Director or designee must then be obtained to certify maintenance and/or repairs are completed and accepted.

The USSS Fitness Center Director or designee is required to sign all service reports, verifying the satisfactory completion of the service(s) performed.

Types of Equipment & locations The contractor shall maintain, service, and repair the following equipment. Additional equipment may be added to each facility. Equipment may be similar to the brand listed but may not be the exact model.

5. USSS Points of Contact:

Each facility’s USSS Fitness Center Director with their respective contact data will be provided upon contract award.

6. Contractor Personnel:

The contractor shall designate a specific individual to be responsible for the completion of the repair work at each facility. If this individual is not available, an alternate, shall be designated to become the person responsible. The name(s) of the individual(s) shall be provided to the USSS Fitness Center Director or designee for each respective facility, at the time of contract award by the Contracting Officer.

Prior to arrival at the respective gym facility, contractor personnel will contact the appropriate Fitness Center Director and/or designee to them at the appropriate entry/access point and be issued a Temporary Contractor Badge. Contractor personnel will be issued and required to display either a non-escort or escort required badge during their physical stay within facility and surrender this badge upon work completion and subsequent exiting the facility.

Contractor personnel must undergo a Secret Service Contractor Access Investigation coordinated by the Security Clearance Division (SCD) within the US Secret Service. All contractor personnel are required to be United States Citizens.

The USSS reserves the right to restrict the access under this contract of any contractor personnel, or representative contractor who is identified as a potential threat to the health, safety, security, general well being, or operational mission of these facilities or its patrons.

The contractor will be notified when contractor personnel are restricted access by the Contracting Officer (CO), and shall provide an alternate who can meet the contract requirements.

7. Hours of Operation:

The contractor shall schedule with the respective facility USSS Fitness Center Director or designee all work to be done on equipment between the hours of 8:30 am and 5:00 pm EST, Monday-Friday, excluding Federal holidays, which are as follows: New Year’s Day, Martin Luther King Day, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving and Christmas.

The contractor will coordinate with the USSS Fitness Center Director or designee for each respective facility, prior to arrival, all monthly preventive maintenance inspections and/or services, which shall be performed at the beginning of each month and completed no later than (NLT) the 7th day of each month.

8. Contractor Furnished Parts and Materials:

Replacement Parts: The USSS Fitness Center Director or designee for each respective facility will approve the contractor supplied part(s). All replacement parts will be new factory warranty parts.

Before replacing parts on any piece of equipment, the contractor will provide, to the USSS Fitness Center Director or the designee, a list of parts required, as well as the costs of parts. No part will be replaced without approval. The contractor will obtain estimates for parts from the supplier. The contractor will then forward the information to the USSS Fitness Center Director or designee who will gain approval for purchase. Upon approval, notification will be made to the contractor to proceed with the part order.

10. Security Clearances

A security background clearance conducted by the USSS and U.S. citizenship is required. USSS will provide Contractor personnel access application forms to be completed by all USSS contract employees and returned to USSS within one week prior to working at the on-site and off-site locations. All such employees must be cleared for access prior to admittance to the location. Updated clearances will be required every January. Cost of the clearances will be borne by the USSS.

11. Insurance

Insurance of the following kinds and minimum amounts shall be provided and maintained during the period of performance of this contract:

(a) Worker's compensation and employer's liability. The contractor shall, as a minimum, meet the following requirements: Contractors are required to comply with applicable Federal and State workers’ compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer’s liability section of the insurance policy, except when contract operations are so commingled with a contractor’s commercial operations that it would not be practical to require this coverage. Employer’s liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers.

(b) General liability. The contractor shall, as a minimum, meet the following requirements: The contractor shall maintain bodily injury liability insurance coverage written on the comprehensive form of policy of at least $500,000 per occurrence.

(c) Automobile liability. The contractor shall, as a minimum, meet the following requirements: The contractor shall maintain automobile liability insurance written on the comprehensive form of policy. The policy shall provide for bodily injury and property damage liability covering the operation of all automobiles used in connection with performing the contract. Policies covering automobiles operated in the United States shall provide coverage of at least $200,000 per person and $500,000 per occurrence for bodily injury and $20,000 per occurrence for property damage. The amount of liability coverage on other policies shall be commensurate with any legal requirements of the locality and sufficient to meet normal and customary claims.

12. Additional Information to be Provided

Contractor shall provide and maintain a list of employees who are authorized to perform work under this contract. All such employees must provide valid state and/or federal government identification. All contractor personnel proposed to work under this contract must be U.S. Citizens. They will be subject to and must successfully pass a police background investigation prior to being admitted into the facility. Upon contract award the contractor will be provided with U.S. Secret Service Contractor Personnel Access forms.

13. Applicable Wage Determination The Occupation Codes 23470 from Wage Determination Number 05-2104, Revision Number 13, dated 6/17/11 and the rates associated with it are incorporated by this reference. In accordance with the Service Contract Act, the contractor must pay at least the minimum wage determination incorporated into this purchase order.
