PART ONE: INSTRUCTIONS

The company who has provided you with this form is responding to an Immigration and Customs Enforcement (ICE) RFQ No. HSCEMS-09-R-00025 for four (4) deployable tactical Radio Interoperability Systems and one (1) fixed facility system interface that allows for the connectivity between the four deployable systems and the current RIOS and satellite systems in the FPS Mobile Command Vehicles. Past Performance is an extremely important part of the evaluation criteria for this acquisition, so your input is very important. The information is to be provided directly to the Contract Specialist, Jeremy Pierre at the address listed below. This information will not be disclosed to the offeror. Please provide an honest assessment and return the questionnaire to ICE by email or fax (email preferred) to the address shown below, no later than Friday, September 4, 2009 at 12:00 Noon Eastern Daylight Time.

Adjectival ratings are defined below and should be used as a reference in assessing performance:

Exceptional
Contractor’s performance significantly exceeded most or all contract requirements. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.
Very Good
Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the Contractor were effective.
Satisfactory
Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the Contractor appear or were satisfactory.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance.

MARGINAL
Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor’s proposed actions appear only marginally effective or were not fully implemented.
UNSATISFACTORY
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problems(s) for which the Contractor’s corrective actions appear or were ineffective.
