SUMMARY OF CHANGES
SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
QUOTE SUBMISSION INSTRUCTIONS:
1. General: Quotes shall be submitted electronically via email to shawna.l.mitchell2.civ@mail.mil and ashley.d.maddox.civ@mail.mil. Quotes must be received no later than the date and time specified on Page 1 of the solicitation (Standard Form 1449, block 8). Any response received after the due date will be considered “late” and will not be evaluated by the Government. All inquiries shall be submitted in writing via email to the aforementioned email addresses no later than twenty-four hours prior to the due date. The Government reserves the right not to answer questions received after the designated deadline for submission of questions.
2. Content of Quote: All of the following information shall be submitted by the due date to be considered a complete quote (refer to Section M for the evaluation factors for award):
a. Technical Capability – The following items shall be submitted with the vendor’s quote for evaluation:
i. Documentation to ensure the specifications provided in Sections B and C of this solicitation are being provided.
ii. If offering a quote for an “equal” service, the quote shall clearly describe how the
“equal” meets each of the salient characteristics provided in Section C.
b. Price: The following items shall be submitted with the vendor’s quote for price evaluation:
i. One (1) signed copy of Page 1 (Standard Form 1449), blocks 30a-c completed.
ii. One (1) completed copy of Section B the solicitation, to include the Unit Price and Amount for Items 0001 through 4001.
c. Submit completed copies of the following provisions, which are included in full text within
Section K of the solicitation:
i. FAR 52.209-7 Information Regarding Responsibility Matters (JULY 2013);
ii. FAR 52.212-3, Paragraph (b) (complete and submit only paragraph (b) if the annual representations and certifications have been completed electronically via the System for Award Management (SAM)); otherwise, if representations and certifications have not been completed electronically via SAM, complete and submit sections (c) through (r) in accordance with (IAW) this provision;
iii. DFARS 252.209-7999 (DEV) Representation By Corporations Regarding An Unpaid Delinquent Tax Liability Or A Felony Conviction Under Any Federal Law (DEVIATION 2012-O0004); and
iv. DFARS 252.209-7991 Representation by Corporations Regarding An Unpaid
Delinquent Tax Liability Or A Felony Conviction Under Any Federal Law—Fiscal Year 2016 Appropriations (DEVIATION 2016-O0002) (OCT 2015).
3. By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the System for Award Management (SAM) database (https://www.sam.gov) prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation (reference FAR 52.204-7(b)(1)).
SECTION M - EVALUATION FACTORS FOR AWARD
The following have been modified:
1. Basis for Award The Government intends to award a single contract resulting from this solicitation to the responsible vendor submitting the lowest priced, technically acceptable (LPTA) quote. A vendor’s overall rating for all non-price factors must be “Acceptable” in order to be eligible for award. An “Unacceptable” rating for any non-price factor results in an overall rating of “Unacceptable”, rending the offer ineligible for award. The following factors will be evaluated:
Factor 1: Technical Capability Factor 2: Past Performance Factor 3: Price
2. Technical Capability Evaluation and Rating:
Technical capability shall be evaluated as follows:
a. The service quoted is for the brand name item or an equal item that meets all requirements and salient characteristics identified in Sections B and C of this solicitation.
The assessment Technical Capability will result in an overall rating as described in Table 1.
Table 1 – Technical Ratings Rating Description Acceptable Quote clearly meets the minimum requirements of the solicitation.
Unacceptable Quote does not clearly meet the minimum requirements of the solicitation.
3. Past Performance Evaluation
The Government will review past performance information obtained from various sources (e.g., Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS) and past performance questionnaires) to assess the vendor’s probability of successful performance for this effort based on the vendor’s relevant and recent past performance information. Past performance will be rated on an “acceptable” or “unacceptable” basis using the ratings in Table 2.
Table 2 – Past Performance Evaluation Ratings Rating Description Acceptable Based on the vendor’s performance record, the Government has a reasonable expectation that the vendor will successfully perform the required effort, or the vendor’s performance record is unknown. (See note below.)
Unacceptable Based on the vendor’s performance record, the Government has no reasonable expectation that the vendor will be able to successfully perform the required effort.
Note: In the case of a vendor without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the vendor may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the vendor shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
4. Price Evaluation Price will be evaluated in accordance with FAR 15.404-1(b)(2), to determine price fair and reasonableness.
