PERFORMANCE WORK STATEMENT

PHYSICAL THERAPIST SERVICES

DDESS-PR RAMEY SCHOOL, 201 ARCH ROAD, AGUADILLA, PR 00603

1.0 PURPOSE: The Department of Defense Education Activity (DODEA) requires Physical Therapy (PT) services for two (2) special education students (severally handicapped) for the period of performance for School Year (SY) 2015-

16 through the end of the Extended Year (ESY) – and four option periods, in support of the Special Education

Program for the DoD Domestic Dependent Elementary and Secondary Schools (DDESS) located at 201 Arch Road, Ramey Base, Aguadilla, PR 00603.

2.0 BACKGROUND: PT services are one of the services mandated for provision of special education services to

DODEA students with disabilities under the DoD Instruction 1342.12 “Provision of Early Intervention and Special

Education Services to Eligible DoD Dependents,” April 11, 2005.

The Case Study Committee (CSC) is responsible for determining if a student requires PT services in accordance with the Individualized Education Plans (IEP) goals. An IEP is the document written by the CSC that establishes the goals and services that must be provided to a student with disability in order to provide a Free Appropriate Public

Education (FAPE). The required services shall be provided with linkage to the educational needs of the student rather than to the medical needs of the student.

3.0 TECHNICAL QUALIFICATIONS: The contractor shall:

3.1 To be a Certified/Credentialed Physical Therapist

3.2 Have pediatric experience

3.3 Have experience working in educational settings

3.4 Have experience with the implementation of IEPs

3.5 Be proficient in English

3.6 Possess basic computer skills

3.7 Be able to work a flexible work schedule of varying days and hours (See 4.10 below)

4.0 REQUIREMENT: The contractor shall:

4.1 Provide direct PT services in accordance with IEP goals for students in grades Pre-K through 12 in accordance with school calendar (TE-2) for school year 2015-16. The school calendar will be updated for the options years at the time the option is exercised. The school calendar is subject to change based on weather, facility, security, health or safety issues.

4.2 Provide consultative PT services to staff, parents, and students in accordance with IEP goals.

4.3 Attend meetings and assist in the development of IEPs for students found eligible for PT services.

4.4 Perform observations, screenings, and evaluations at school.

4.5 Write evaluation reports to include diagnostic findings and educational implications.

4.6 Perform case management duties to include tracking of timeliness and review dates.

4.7 Maintain confidential special education files to ensure compliance with the Privacy Act 10 U.S.C. 2164 and 20

U.S.C. 921-932 authorizing DoD Directive 1342.20 “DoD Education Activity (1992) authoring DoD Education

Activity Administrative Instruction 6600.1 (2004) and DODEA 2500.13G September 2005.

4.8 Plan and assemble appropriate specialized equipment and accommodations.

4.9 Coordinate work schedule hours/day with the DDESS-PR ISS Special Education . Approximate numbers of hours per week are five(5). (TE-1 below). Services are generally performed between the hours of 0800-1430, five days per week. Actual hours and days shall be based upon the school calendar and the number of students requiring services. Student enrollment for services fluctuates and there may be periods during which no services shall be performed because there are no students requiring these services in the current enrollment. The contractor shall not exceed the hours for the performance period without written permission of the Contracting Officer.

5.0 PERSONNEL REQUIREMENT:

5.1 Standards of Conduct: Contractor personnel performing under this requirement shall comply with the Standards of Conduct of DDESS-PR Ramey School. The contractor agrees that inappropriate conduct shall not be tolerated and may be grounds for suspension or dismissal of a contractor employee from performance under this contract.

5.2 Security: The contractor shall comply with the security requirements of Ramey School and installation.

6.0 GOVERNMENT SUPPLIED MATERIALS AND SERVICES: The Government will provide:

6.1 Assessment tools and protocols as required for compliance with completion of assessments as deemed appropriate by the Case Study Committee (CSC).

6.2 Access to printer and copier for completion of work tasks related to assessment, report writing, preparation for meetings and service delivery to students as per their Individualized Education Plans (IEP).

TE-1: BREAKDOWN: RAMEY SCHOOL

One (1) of the students will require a three times weekly for 60 minutes (3x weeklyx60min) service and One (1) student will require a two times weekly for 60 minutes each (2xweeklyx60 minutes). This request is also to contract to include a total 8 hours per student for evaluation and report writing equivalent to 16 additional hours as a separate line item.
