QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
1. Contract or Task Order Title; Shelter Rental Contract, JITC Ft. Huachuca
2. Work Requirements:
· Task 1 - Deliver and pick-up temporary office spaces
· Task 2 - Provide repair and replacement
· Task 3 - Inspect temporary office spaces at delivery and pick-up
3. Primary Method of Surveillance:
100 Percent Inspection. The TM will receive, request repair or replacement, monitor pick-up, and inspect units with the driver to validate compliance with contract terms. Full functional check of heading, ventilation, and air-conditioning systems and power connections will take place within three days. JITC requires this time to make the necessary connections to power systems.
4. Scope of Performance:
4.1 Deliver the requested number of 10’ x 30’ temporary offices as scheduled, with bars on the windows and lockable door, and 2 phase volts alternating current, 100 amps power capability as needed and coordinated
4.2 Contractor will deliver clean and serviceable shelters on or before the date required on site.
4.3 Contractor will complete an on-site inspection of the shelter with the TM and annotate any damages prior to drop-off departure.
4.4 Contractor will pick up the shelter on the scheduled date as coordinated with the TM.
4.5 Contractor will complete an on-site inspect the shelter with the TM prior to pick-up and annotate any damages prior to removing the unit.
4.6 Contractor will repair or replace any shelter within 72 hours should a problem with climate control, security features, power, or structural integrity of the unit develops while in use. Urgent repair or replacement will be conducted within 24 hours.
4.7 Pick-up units not later than five days after scheduled use.
5. Performance Standards:
5.1 Complete temporary office space delivery on the scheduled delivery date.
5.2 Complete repairs or replacement within 72 hours. Urgent repair or replacement will be conducted within 24 hours.
5.3 Conduct delivery and pick-up inspections with the TM to verify the condition and determine any liability prior to driver departure.
5.4 Pick-up units not later than five (5) days after scheduled use.
6. Acceptable Quality Level (AQL):
6.1 Complete 100% delivery of units on time or early
6.2 Complete repairs or replacement as specified in section 5
7. Evaluation Method:
7.1 The TM will coordinate delivery and document actual delivery date. Early delivery may be acceptable with prior coordination.
7.2 The TM will conduct and document inspection of the unit with the driver upon delivery. Damages or discrepancies will be annotated on company documents at that time by the driver and the TM personnel. The document will be signed by both parties and a signed copy provided to the TM.
7.3 The TM will document the date and time the contractor was contacted regarding the need for repairs or replacement and the time of problem resolution.
7.4 The TM will coordinate the date and time for unit pick-up an document any pick-up that occurs after five business days.
7.5 The TM will conduct an inspection of the unit with the driver at pick-up. Damages or discrepancies will be annotated on company documents at that time by the driver and the TM noting any new damages that occurred during use. The document will be signed by both parties and a signed copy provided to the TM.
8. Incentives (Positive and/or Negative):
This is an ongoing effort and options to the base contract will be exercised upon successful completion of all contract requirements. The options years will not be exercised for missed schedules, continued problems with the quality of the office spaces provided, or failure to conduct repairs or replacements in a timely manor. Repeated problems could result in cancellation of the contract. Meeting or exceeding the requirements spelled out above will also be considered in determining if the options are exercised.
