1-12-2017 Statement of Work
USCH Main Entrance Bollards
1. Project Location: 401 SE First St., Gainesville, FL. 32601
2. Contractor Working Hours: Normal Federal workdays M-F 7:00AM – 5:00PM
3. Purpose: Provide a bollard system along the sidewalk at the building entrance.
4. Points of Contact:

5. Notice to Proceed:
5.1 The Contracting Officer is the only individual from the Government that can give the contractor a notice to proceed.
5.2 Before any of the work within the scope of the contract is started, the Contractor shall meet with the Contracting Officer Representative(COR) for a pre-construction meeting. City of Gainesville permit requirements shall be in place prior to construction.
6. Period of Performance: Work shall be substantially complete 60 days from the notice to proceed. Project to be 100% completed within 7 days of substantial completion.
7. Submittals:
7.1 As Built Drawings (mark up contract documents for changes/adds/deletions)
7.2 Product Data sheets for all materials provided.
7.3 List of all personnel required to complete the site work
7.4 City of Gainesville ROW permit
8. Description of Work: Contractor shall provide all materials, labor, tools, equipment and supervision to accomplish tasks as outlined in the contract documents. Materials, supplies and equipment used shall be commercially available products of reputable manufacturer or suppliers.
8.1 Remove Sidewalk/railings as noted in the drawing below.
8.2 Provide shallow mount bollards as noted in drawing below. Maximum spacing between bollards: 4’ center to center. Minimum height above grade: 3’. Minimum rating: stop a 4,000 lb. vehicle at 30 mph. Basis of design; Barrier 1 systems, Inc. Model SMB-400.
8.3 Provide concrete as noted in the drawing below. Materials shall be IAW city of Gainesville ROW permit requirements and bollard manufacture.
8.4 Modify railings as noted in the drawing below.

• Permits: Prior to start of work all applicable federal, state and local permits shall be obtained by the permittee from the appropriate governing agency. These include City of Gainesville, Florida Department of Environmental Protection, Water Management District, Florida Department of Transportation, Alachua County, Florida Division of Forestry, etc.
A Building Permit is required for the installation or replacement of driveway aprons, curbs, sidewalks, and stormwater systems.
• Right-of-Way Use: All permitting activities related to work in the public ROW shall adhere to the City of Gainesville Code of Ordinances, Part II, Chapter 23 – Streets, sidewalks and other public places.
Obtaining a Right of Way Permit does not alleviate the requirements for other applicable permits, including but not limited to Driveway Permits and Maintenance of Traffic Permits.
The applicant must move or remove facilities located in the public ROW, regardless of permit status, in accordance with the approved schedule provided to the City.
Work in the public ROW which qualifies for a permit exemption under Section 23-40 and is emergent in nature, shall provide the City with the information required in a permit application once the situation has been stabilized.
City Right-of-Way:
The method and manner of performing the work and the qualities of material for construction within the ROW shall conform to the requirements specified by the Public Works Department.
No work shall be done nor materials used in the ROW, without inspection by the Public Works Department (334-5070), and the Contractor/Developer shall furnish the Department with every reasonable facility for ascertaining whether the work performed and materials used are in accordance with the requirements and intent of the plans and specifications.
The Public Works Department reserves the right to modify the proposed work within the ROW to ensure compatibility with existing improvements.
• Maintenance of Traffic: All permitting activities related to Maintenance of Traffic shall adhere to the City of Gainesville Code of Ordinances, Part II, Chapter 23 – Streets, Sidewalks and Other Public Places.

Obtaining a Maintenance of Traffic Permit does not alleviate the requirements for other applicable permits, including but not limited to Driveway Permits and Right-of- Way Permits.
• Public Sidewalk: Public sidewalks shall be constructed of a minimum 4 inch thick -class NS per FDOT Spec 347 for non-driveway areas and 6 inch thick - 3,000 psi concrete at driveway crossings. Brick pavers and other alternative materials used within the ROW shall include a 4 inch thick minimum concrete subgrade. ADA standards apply to all public sidewalks. See Appendix B for a brick detectable warning detail. Ramps at public sidewalks shall be in accordance with FDOT standards.

9. Liquidated Damages: $150 per calendar day beyond the final completion date.
10. Security: Contractor shall submit a list of all personnel required to complete the work prior to notice to proceed.
11. CLEAN-UP: The contractor shall remove all debris generated in the performance of this contract daily. The contractor shall recycle or reuse any parts and / or material removed from the site, with no cost to the government.
12. Inspection and Acceptance: Upon completion, all items shall be inspected and agreed upon for final payment upon invoice receipt. The contractor and the COR shall meet and discuss prior to the contractor leaving the site
13. Warranty: All materials and labor shall be warranted for a one year period following acceptance by the COR. If a product has a longer standard warranty, the government shall be provided a copy of such.
14. PROTECTION: The contractor shall provide all materials, barriers and safety equipment necessary for the protection of pedestrians and property during the implementation of this contract. Every precaution shall be taken by the contractor to prevent a fire while storing any combustible materials.
15. SAFETY AND HEALTH: All work shall comply with the applicable requirements of 29 CFR
1910/1926 and 40 CFR 761. All work shall comply with applicable state and municipal safety and health requirements. Where there is a conflict between applicable regulations, the most stringent shall apply. The contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of work, and shall hold the government harmless for any action on its part or that of its employees or sub-contractors, which results in illness or death. The contractor shall provide all necessary safety equipment and ensure that such equipment and all relevant safety procedures are adequate for the job being performed and are utilized properly.
16. BUY AMERICAN ACT: Use only domestic construction materials in construction contracts performed in the United States. Reference FAR 52.225-9 – Buy American Act – Construction Materials for further details. The full text of this clause can be located at www.acquisition.gov/far
17. TAXES: The CO will issue a tax exempt certificate for all materials associated with this contract.
18. Closeout: Provide a marked up set of contract documents showing the as built conditions/changes. Provide all manufacturer’s product and maintenance data for items provided. Provide a final invoice for payment.
