RESTOCKING CHARGE
There is a 25% restocking charge for all returns, with the following exceptions:
PRODUCT THAT WE HAVE SHIPPED IN ERROR
Return requests must be made within 30 days of our original ship date. Product must be unused, unopened and in resalable condition. Please DO NOT write address or RA # on the outside of the original boxes, as the product needs to be in resalable condition in order to receive credit.
DEFECTIVE SUPPLIES and PERIPHERALS
Defective supplies and Peripherals must be returned to Cannon IV within 30 days of our original shipment.
We test all products returned as defective and issue a credit if the product does not meet manufacturer specifications. Laser, fax and inkjet cartridges which test "empty" are not considered defective.
Cannon IV Printer / Fax Machine Return Policy
Open Box/Non-Defective
Within 30 days of Ship Date: No open box, non-defective returns will be permitted for return unless pre-authorization is received from Cannon IV. Cannon IV will work with manufacturer for a return authorization. Returns are at the sole discretion of the Manufacturer. All return freight will be at the expense of the end user. This includes printers ordered in error.
After 30 days of Ship Date: Cannon IV will not accept any returns. This is the manufacturers return Policy.

Factory/Sealed
Within 30 days of Ship Date: Cannon IV will allow factory sealed returns within 30 days of ship date. Exterior of all boxes must be in resalable condition. All return freight will be at the expense of the end user.
After 30 days of Ship Date: Cannon IV will not accept any returns. Based on manufacturer return policy.
DOA/Dead on Arrival
Within 30 days of Ship Date: End user is to contact Cannon IV Customer Service for all DOA printers / fax machines not installed by Cannon IV. Many manufacturers now handle the returns directly. Cannon IV will advise the customer on the specific manufacturers policy depending on the brand at the time of call and will assist in helping facilitate the repair or return. End user will receive a Case number initiated from manufacturer once the call has been logged, this is not a return authorization, and it is only a reference number to keep track of the DOA unit. If manufacturer states the unit can be replaced by Cannon IV, end user will need to contact Cannon IV Customer Support and provide the manufacturer Case number if working directly with the manufacturer. Cannon IV will issue an exception with manufacturer and issue a return authorization to the end user. Cannon IV will not ship out replacement unit(s) until the return authorization is received from manufacturer.

All returns are subject to the limitations placed on us by our suppliers' specific return policies and warranties. Some items may not be returnable because of suppliers' restrictions.
INSTRUCTIONS FOR PRODUCT RETURNS
Please call our Customer Support Department at 888-284-4844 ext 399 for approval prior to the return of any product. They will be happy to provide you with a Return Authorization number (RA #) and shipping instructions. Please DO NOT write the address or RA # on the outside of the original boxes, as the product needs to be in resalable condition in order to receive credit. Any packages without an RA # will be refused.
All returns must be sent freight prepaid. Freight will only be credited for Cannon IV errors. Our drivers cannot pick up returns without prior return authorization.
When calling our Customer Support Department for a return, please have the following information available to expedite your request:

TERMS AND CONDITIONS APPLICABLE TO CANNON IV SERVICES
RENTAL PLAN FOR COPIERS (SIN 51-55)
Cannon IV provides Rental Plans for Lexmark and HP copiers and multifunction printers. Rental rates cover unrestricted normal use of the rented equipment up to the maximum number of copies (e.g., number of copies per month), as identified in the rental agreement. An excess charge per copy is usually assessed for copies over the maximum number of copies provided for under the rental agreement. The rental rate includes full-maintenance service (repairs, adjustments, replacements) by authorized service technicians at no additional cost to the Ordering Agency. Rental rates do not include the cost of consumable supplies. Rental Plans are provided in terms of twelve months.
REPAIR OF GOVERNMENT-OWNED EQUIPMENT (SIN 51-56)
Cannon IV provides Repair of any type of copier equipment offered under this Schedule not covered under a Maintenance Service Agreement. Repair includes: directly related attachments, supplies and accessories. Rates are $125 per hour with a minimum one-hour agreement. Work performed will be billed in 30 minute increments thereafter. This rate applies to Indiana, Ohio, Kentucky and Texas only.
LEASE TO OWNERSHIP PLANS FOR COPIERS (SIN 51-58)
Cannon IV offers Lease-to-Ownership Plans (LTOP) which are intended for Ordering Agencies that prefer to lease all types of copiers to secure ownership. At the end of the lease period, and upon final payment of all charges due, the title and the leased equipment is immediately transferred from the contractor to the Ordering Agency. The lease rate does not include the cost of maintenance or consumable supplies. Contract terms for LTOP options are available in twelve to sixty month periods.
OPERATING LEASE PLAN FOR COPIERS (SIN 51-58A)
Cannon IV offers Operating Lease Plans applicable to copiers and multifunction printers. An Operating Lease Plan allows Ordering Agencies to lease the equipment for a monthly rate over a period of time, with NO intent to purchase the equipment at the end of the lease agreement period. Ownership of the asset remains with Cannon IV.
Contract terms for Operating Lease Plan options are available in twelve to sixty month periods.
OPERATING LEASE PLAN – ALL OTHER PRODUCTS (SIN 51-58B)
Cannon IV offers Operating Lease Plans applicable to all other products (laboratory imaging systems, mailroom equipment, typewriters, darkroom w/equipment and supplies, network optical systems) under this Schedule with the exception of copiers. Operating Lease Plans allow Ordering Agencies to lease the equipment for a monthly rate over a period of time, with NO intent to purchase the equipment at the end of the lease agreement period.
Ownership of the asset remains with Cannon IV. Contract terms for Operating Lease Plan options are available in twelve to sixty month periods.
COST-PER-COPY PLAN FOR COPIERS (SIN 51-100C)
Cannon IV offers Cost-Per-Copy Plans for Copiers which allows the Ordering Agency to identify the guaranteed minimum number of copies required per month and the required functional capability of the copier equipment. As necessary to satisfy the agency's requirements, Cannon IV provides the required copier equipment including:
accessories, attachments, copy-control devices and systems, all consumable supplies (except copying paper and transparency films) and full-service maintenance including repair parts. The Ordering Agency pays a fixed monthly fee based on the agency’s guaranteed minimum number of copies per month. For copies over the guaranteed minimum, the agency pays an additional fee for each copy made.

MAINTENANCE SERVICE AGREEMENT FOR ALL OTHER EQUIPMENT & REPAIR (SIN 51-208)
Cannon IV provides Ordering Agencies Maintenance Service Agreement options and training on selected software items.
PaperCut Software: PaperCut Software is a powerful cross-platform, cross-manufacturer, cross-industry solution that helps organizations understand and control costs related to printing by tracking and reporting user-level activity. PaperCut Software determines how users are utilizing printing devices, provides automated reports and can be used to implement document workflows.
Capella Technologies (MegaTrak & MIPA PIN): Capella Technologies’ products and solutions are designed to enhance, track and secure document output without requiring major changes to existing business systems or hardware. These solutions allow agencies to improve the cost-effectiveness of document output while leveraging existing investments in technology. Options include server and device-based tracking technologies for HP multifunction devices.
MANAGED PRINT SERVICES (SIN 51-500)
Cannon IV evaluates the client’s current printing and imaging environment and works with them to design an approach that is tailored to fit their needs. The Cannon IV MPS program encompasses people, processes and technology to help the client assess, design, implement, and manage their fleet of printing, scanning, copying/multifunction and fax devices. We simplify the management of the fleet of devices by providing professional services, supplies, maintenance, on-site service and help desk support. These services are typically invoiced based upon a cost per page.
With professional services, Cannon IV helps the client understand what devices they have deployed. Then, we design and implement a balanced deployment of printers and multifunction systems to optimize productivity and minimize costs. This may include repositioning existing fleet to high-demand locations and supplementing with newer printers or multifunction systems where appropriate.
Scanning features on new multifunction systems provide a powerful platform to move information on and off the client network. Cannon IV integrates multifunction systems and printers with powerful solutions to help clients manage, route, store and control their documents.
Our goal is to design the best solution for the client from our comprehensive portfolio of hardware, software and professional services. Whether the client has new devices or a mix of new and legacy printers and multifunction devices from multiple manufacturers, Cannon IV constructs a program that meets the client’s specific needs. If there are obsolete or high costs of ownership devices, printers or MFPs, Cannon IV will replace those devices as a component of the MPS program solution.
MPS Services Provided
Document Systems and Workflow Assessment Services The Document Systems and Workflow Assessment service is comprised of three main elements: Inventory of current output devices (printers, copiers, scanners, and faxes), print usage volumes by device and workflow inventory and analysis. Following completion of assessment activities for each client location, Cannon IV and the client begin the design phase. Cannon IV assessment responsibilities may include:
Work with the client to validate the scope of the discovery effort including sites and device types to be discovered. Discovery scope includes the types of devices to be discovered – printers, faxes, scanners, copiers, large format printers, etc; and specified configuration details or data points to be gathered for each device type.
This includes identification of network and non-network print devices.
Work with the client to develop a schedule for discovery and design.
Work with the client to plan a communication message that communicates the discovery efforts to all affected client employees.

Utilize network discovery when necessary and request access to any printer inventory and monthly copier volume data to help facilitate and augment the discovery process.
Systematically walk through the client’s locations (included in the scope of the discovery) and identify the location of all discovered in-scope imaging and printing devices.
Gather information on the capabilities of specific devices to help understand requirements for the future fleet such as color printing, 11x17 paper handling, two-sided printing, and output capabilities (sorting, stapling).
Perform formal interviews with key users (as identified by the client) when necessary in order to capture specific usage and end-user print and imaging requirements or requests. Ad-hoc interviews may also occur as the device discovery is taking place. Based on the observations of the consultants, they may ask questions to nearby employees about the usage of specific equipment. Ad-hoc interviews are optional and may be omitted if required by the client or local regulations.
Any required travel will be invoiced based upon actuals, not to exceed the Joint Travel Regulations in effect at the time the travel is taken. Travel costs will be invoiced based upon Joint Travel Regulations as a separate line item.
The deliverable to the customer resulting from this Assessment Service will include:
o Provide floor maps with location of current state devices identified o An inventory of devices with average monthly page volumes per device and associated cost-per-page per device for the volume of pages printed per device o Carbon Footprint and energy consumption analysis report
MPS Program Design Services MPS Program Design service which includes: Compiling the number and models of devices located within the scope of the Document Systems and Workflow Assessment and provide current-state utilization data. Provide color floor maps with location of proposed future-state devices identified. The Cannon IV professional services team reviews the Document Systems and Workflow Assessment with the client, and in a collaborative process, design an MPS Program that meets the needs of the client. The outcome of the Design service is a Statement of Work that provides a definition of the products and professional services that will be provided to the client in the MPS Program and an MPS Program Implementation schedule. The MPS Program Statement of Work defines the roles and responsibilities of Cannon IV and the client.
MPS Project Management Services A Cannon IV Project Manager is assigned to coordinate the initial implementation of the products and services at each client location set forth in the MPS Program Design Statement of Work. The Project Manager acts as the single point of contact for the client during the deployment of the products and services; communicates and monitors that client requirements for hardware delivery and installation are in place prior to installation activities staring; develops a deployment plan for the delivery and installation of the products; establishes regular communication with client to provide status on the deployment schedule through the completion of delivery and installation of products; manages equipment installation activities; and manages communication with installation service engineers.
Installation Cannon IV will: Unbox/uncrate the products; inventory the product against the packing list; set up the products;
install accessories, including memory, paper-handling, duplex-unit, and EIO devices; install consumables (client must provide paper); connect products to power and network connections; verify products functionality via a standard print test; provide client with up-to-date product device drivers; provide checklist of performed actions;
place excess packaging materials in client-provided trash receptacle. Key operator training will be provided and will include the following:
Cannon IV will provide up to one (1) hour of training Cannon IV will provide a basic overview of how to install consumables, how to print a test page, how to clear paper jams, overview of basic features and functions of the device

Hardware Support Services Based upon the contracting agency requirements as defined in the MPS Program Design or defined in a contracting agency RFP, one or more of the following services may be included in the Cost-per-Page rate for the Managed Print Services Contract.
a) Onsite hardware support. For technical hardware issues that cannot be resolved remotely, a Cannon IV authorized Service Engineer provides onsite technical support on covered hardware products to return them to operating condition.
b) Technical Phone Support. Cannon IV provides a designated Technical Phone Support Center as well as an online Service Portal for clients to place service requests. Cannon IV will work with the client during coverage hours to isolate the hardware problem. Prior to any onsite assistance, Cannon IV may initiate and perform remote diagnostics using electronic remote support tools (where available) to access covered products, or Cannon IV may use other means available to facilitate remote problem resolution such as telephone, email, text or facsimile communication tools.
Cannon IV provides technical support on all products listed in the MPS Design Statement of Work. The Cannon IV Technical Phone Support Center is accessed by calling a toll free number and providing the Product Identification (PID) number, which has been assigned by Cannon IV. The support staff is able to identify the client based on the PID provided. Once in contact with the support staff, the client is able to receive assistance on technical issues with their Cannon IV products, get answers to usability questions and place requests for supply fulfillment. If on-site service is needed the support agent will make those arrangements for the client.
c) Onsite Service level options The MPS Program Design Statement of Work defines the service coverage level based upon the needs of the client. Standard service coverage is available during the coverage window, 8:00 am to 5:00 pm local time, Monday through Friday excluding Cannon IV holidays – New Years Day, Memorial Day, Independence Day July 4th, Labor Day, Thanksgiving, and Christmas. A Cannon IV authorized Service Engineer will arrive at the client’s site during the coverage window to begin hardware maintenance service. The standard service coverage offerings include:
o Same Business Day, standard business hours (9x5) o Same Business Day, extended hours (There is a 50% premium on standard hourly rates) o Next Business Day, standard business hours (9x5) o Next Business Day, extended hours (There is a 50% premium on standard hourly rates)
Supplies Replenishment Service – Consumables The Supplies Replenishment service provides for the replenishment of toner cartridges for devices defined in the MPS Program Design Statement of Work. This does not include the provision of paper or staple cartridges (collectively, the “Consumables”) for the defined devices. The Supplies Replenishment service minimizes the supply-chain cost exposures and inefficiencies for the client while enhancing en-user productivity.
Based upon the requirements of the contracting agency and defined in the MPS contract Cannon IV will provide OEM toner products or compatible toner products manufactured per ISO 9001 manufacturing processes.
Based upon the contracting agency requirements, as defined in the MPS Program Design or defined in a contracting agency RFP, the Supplies Replenishment Service may be included in the Cost-per-Page rate for the Managed Print Services Contract.
Training Services Cannon IV provides development, documentation and implementation of a training plan designed to educate the client’s end users about the features and full functionality for the specific products which are specified in the MPS Program Design Statement of Work. The client is responsible for describing their current education and communication processes and working with the Cannon IV Solution Architect to jointly develop the training plan.
The resulting training plan may consist of the following training options and are charged on an hourly basis:
Custom onsite end-user training Remote or web-based end-user training

Outline of education solutions such as training, job aids, and resource materials Logistic plans and delivery schedules Outline of communication events, describing the purpose, key points and designated audience; such events may include e-mails, presentations, posters, and other media
The hourly rate will be based upon whether the contracting agency requires a Solution Architect Level 1 or a Senior Solution Architect.
Based upon the contracting agency requirements as defined in the MPS Program Design or defined in a contracting agency RFP, one or more of the Training Services may be included in the Cost-per-Page rate for the Managed Print Services Contract or as a separate line item expense. This service will be invoice on an hourly basis, per Training Service.
Document Management Software Services Document Management Software Services are professional services to address the needs of the client for document security and enterprise content management. Following the completion of a document management and workflow analysis, Cannon IV develops a Document Management Software Services Statement of Work with the client.
Based upon the contracting agency requirements as defined in the MPS Program Design or defined in a contracting agency RFP, one or more of the Document Management Software Services may be included in the Cost-per-Page rate for the Managed Print Services Contract or as a separate line item expense. This service will be invoiced on an hourly basis.
Networking services Cannon IV provides printer network setup, including programming of the IP address and network settings and verification of network printing functionality. Cannon IV also provides: configuration and setup of scan-to-email capability, point MFP to existing SMTP server, link device to existing LDAP server for easy access to company email addresses, setup of scan-to-folder capability for one folder that exists on the client’s network, key operator training on how to use the digital sending features on your device and a checklist of performed actions.
Based upon the contracting agency requirements as defined in the MPS Program Design or defined in a contracting agency RFP, one or more of the Networking Services may be quoted and invoiced as a separate line item expense.
This service will be charged on an hourly basis.
MPS Service Labor Categories
Solution Architect Level 1 $146.10/hr The Solution Architect Level 1 (SA1) designs multi-systems and ensuring architectural integrity across the platform(s). Expertise and work efforts focus primarily on design activities from requirement analysis through design specification. Duties are usually performed at the customer’s site. The SA1 is responsible for functions including but not limited to on-site evaluations and walk-through, manual and software based page count collections and other general assessment services and end-user product training. The SA1 is responsible for processing and analyzing data following an assessment.
Senior Solutions Architect $166.25 The Senior Solutions Architect (SSA) designs multi-systems and ensuring architectural integrity across the platform(s). Expertise and work efforts focus primarily on design activities from requirement analysis through design specification. Duties are usually performed at customer site. The SSA is responsible for the analysis and design of MPS solutions requiring an advanced software strategy. The SSA is responsible for the design, implementation and training of software solutions including but not limited to Enterprise Content Management (ECM), workflow, job tracking and print management solutions. The SSA will also be assigned to conduct end-user product training.

Project Manager $166.25 The role of the Project Manager is to plan, execute, and finalize the project on-time and within budget. This includes acquiring resources and coordinating the efforts of team members to deliver the project according to plan.
The Project Manager will also define and measure the project’s objectives and oversee quality control. Throughout the life cycle of the project, the Project Manger will continuously communicate and monitor all tasks and act as the single point of contact for both internal and external team members. This includes formal status reports distributed to key stakeholders and timely escalations of critical issues. The Project Manager will also develop a transition strategy to ensure continued support upon completion.
Service Engineer – Entry $136.02/hr The Entry level Service Engineer provides installation and repair, to Cannon IV clients for single function printer devices and some basic multifunction devices. The Service Engineer is responsible for coordinating multiple work orders, setting up new equipment, troubleshooting equipment, performing analysis and providing advice to resolve problems with direction from senior staff.
Service Engineer – Senior $156.17/hr The Senior Service Engineer provides support – installation and repair – to Cannon IV clients and staff for single function and multifunction devices. Duties will be wide ranging, from mentoring staff, setting up new equipment, troubleshooting equipment and participating in implementation of new initiatives. The Service Engineer participates in the development and delivery of a comprehensive program of services that assist and enhance partnerships with our clients. The Service Engineer is competent to advise and resolve problems and perform analysis with some direction from senior staff. The Service Engineer will assist in implementing plans. Demonstrated expertise should include certifications on the equipment supported by Cannon IV.
Managed Print Services Terms and Conditions
Covered Equipment. The Cost per Page is based on the number of pages, as described, above, printed on Equipment covered under this Agreement. The term “Equipment” refers to the printers and multi-function printing devices, identified in the MPS Program Design Statement of Work.
Fleet Management Software License. Cannon IV may install certain hardware and software programs at the Ordering Agency’s premises and on the Ordering Agency’s Equipment, computers, and computer network, to collect output device page usage data or provide other functionality. The software will provide automated page counts to Cannon IV, and can provide notification for the replenishment of printing supplies, routine Equipment maintenance, and need for Equipment repairs.
Service Coverage Window. The coverage window specifies the time during which the described services are delivered onsite or remotely. Service requests received outside this coverage window will be logged the next day for which there exists a coverage window. Coverage window options available for eligible products are specified in the service level options section. All coverage windows are subject to local availability. The service coverage window will be defined in the MPS Program Design Statement of Work.
Service Onsite Response Time. Onsite response time specifies the period of time that begins when the initial service request is received and logged with Cannon IV and ends when the Cannon IV authorized representative arrives at the Ordering Agency’s site, provided the time falls within the specified coverage window. Response times are measured during the coverage window only and may be carried over to the next day for which there exists a coverage window. Response time options available for eligible products are specified in the service-level options section below. All response times are subject to local availability. Cannon IV Hardware Onsite Support coverage applies to the products listed in the MPS Program Design Statement of Work.
Change Management Process. Any changes to MPS Program Design statement of Work or requests by the Ordering Agency for additional Cannon IV Services (including products, support services and supplies management services), must be agreed upon by the parties in writing through a change order, which shall include a description of such additional Cannon IV services and the new pricing.

NEEDS ASSESSMENT & ANALYSIS SERVICES (SIN 51-501)
Cannon IV helps Ordering Agencies thoroughly evaluate their printing and imaging environment to determine the total cost of operation on an annual basis. There are four major components to a Cannon IV Needs Assessment:
Identification of both the Ordering Agency’s printing and imaging needs as well as challenges and strategy of agency departments and users
Inventory of all of agency’s output devices, print volumes, software applications and workflows Documentation of security and compliance vulnerabilities Determination of environmental impact based on energy consumption, paper usage and carbon footprint
Once the assessment is complete, Cannon IV will move into the analysis phase and collaboratively design a solution that best fits the Ordering Agency’s needs based on the assessment findings. Cannon IV will then provide recommendations and work with the agency to design an optimal printing and imaging strategy.
