TERMS AND CONDITIONS

1) Orders:

a) Agencies will be required to execute the following Agreements prior to ordering services: Master Services Agreement, End-User Agreement, applicable Addendums or Exhibits based on specific services and upon receipt of all FCRA required notices. A copy of all such documents are available from primary contact for review and execution. These are requirements of the FCRA, CFPB, State, local and Credit Bureau providers.

b) Agencies may use EDI Orders, Blanket Purchase Agreements, individual purchase orders or task orders for ordering services under this contract. Blanket Purchase Agreements shall not extend beyond the end of the contract period; all services and delivery shall be made, and the contract terms and conditions shall continue in effect until the completion of the order. The purchase order shall specify the availability of funds and the period for which funds are available. For GSA reporting purposes only, an order should include the SIN(s) covered. BPI’s task order proposal will reflect the applicable SIN(s).

PLEASE NOTE BELOW PRICING INFORMATION:

· All plans can be customized to meet your needs.

· The prices identified above do not include applicable Country, federal, county, state, verification, court, third party fees, and certain other costs that Pre-Employ is required to pay in order to gain access to records requested by End- User. Unless otherwise noted on this Exhibit, End-User is responsible for payment of these costs. (As an example, some N.Y. counties up to $100 per county per name searched. Actual costs incurred will be added to the base price of service).

· The prices listed above only reflect the rate charged for searching for one name in one state, county, federal jurisdiction or Country. An additional charge will be made for each additional name searched, and each additional county or country searched. Unless otherwise specified by the End-User, Pre-employ will search for Best names and U.S. counties provided by the consumer (e.g., the job applicant or employee), as well as Best names and Best counties that are encountered in other searches performed by Pre-employ regarding the consumer.

· Additional court access fees may apply to criminal searches, and additional names searched, depending on the court.

· Additional International fees may apply.

· Additional service fees may apply to employment and education verifications.

· Additional state fees may apply to driving history searches depending on the state. Specific list is available upon request.

· Consistent with industry best practices, all records found in large databases, such as the “U.S. Criminal” database, will be re-verified at the local level. The End-User will be separately charged for such verification at the rate shown plus applicable fees.
