Statement of Work San Carlos Irrigation Project

Flame Resistant Clothing and Boots Program

1.0 SCIP’s Flame Resistant Program Specification

1.1 Overview of Services Requested by SCIP

a. Direct billed, commercial PPE program for flame resistant (FR) protective work apparel and boots

b. Approximately 25 employees to make purchase of flame resistant (FR) shirt/jean and boots with initial allowance of $2000 and $750 per year thereafter.

1.2 Garments Specifications & Pricing:

a. Work Shirt: Shirts to be picked by SCIP personnel must be rated at a minimum of CAT2.

Shirts can have front pockets and nonmetallic buttons but must have area free for embroidery over left chest.

b. Work Pant: Pants to be picked by SCIP personnel must be rated at a minimum of CAT2.

Five jean style pockets, two-needle felled seam construction, soft pre-wash comfort look, and/or equal or better.

c. Work Boot: Boots to be picked by SCIP personnel must be rated at a minimum of safety toe.

d. Price: Initial allowance not to exceed $2000.00 per employee. Additional annual allowance not to exceed $750. Remaining balance can not be rolled over to following year and entire allowance should be exhausted within the year.

f. The garment shall be marked for easy identification for FR approved clothing.

1.3 Service Requirements by Supplier:

The supplier shall include the following services in the FR Clothing and Boot program:

a. Initial sizing and outfitting for employees.

b. Supplier shall take individual measurements of all employees.

c. Sizing session shall be conducted within first week of program, either on site at SCIP or a retail site of SCIP’s choosing.

d. Any adjustments required in sizing or outfitting needs to be completed in 7-day workdays.

e. Create and maintain an employee-sizing database that is accessible to SCIP employees via their individual login to web-based maintenance account.

f. New employees shall be sized and outfitted within 7 calendar days.

1.4 Supplier Requirements:

a. Maintain an online catalog of FR clothes and boots that can be shipped within 7 calendar days of purchase.

b. All purchases will be reconciled monthly, and an invoice sent to SCIP for payment, through’s Department of Interior’s Invoice Processing Platform (IPP).

c. Returns should be processed through the online ordering website, with a printable pre-paid return shipping label to return any unwashed, unworn, and un-embellished item within 60 days for exchange or refund at no charge.
