1.0 Description of Service/General Information
The contractor shall construct, install, operate, and maintain both MDTA and MiniMDTA Multiplexor and Set Top Box(es) product types of basic cable and high-speed internet services for 377 Force Support
Squadron and 377th Civil Engineer Division, Kirtland Air Force Base (AFB) in accordance with the terms and standards set forth in this Performance Work Statement (PWS). The services shall be capable of providing service to any building owned by the base. The provider shall be capable of providing service to any office/locations within Kirtland AFB. In instances where there is no existing infrastructure at a facility requiring cable and internet services, the contractor shall provide all equipment, material, and parts associated with the installation. The intent of the Government is to have cable and internet services for more than one facility on Kirtland AFB. Herein as Appendix A lists a breakdown of required buildings, number of connections, account number and product type (e.g. MDTA or MiniMDTA
Multiplexor and Set Top Box(es))
1.1 Basic Services
The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to provide commercially acceptable cable and high-speed internet services, comparable to services offered in the local community, to identified government-owned facilities herein as Appendix A. This may include more than one cable and high-speed internet outlet per building, if needed.
1.1.1 Service Details
The contractor will be expected to provide cable and high-speed internet services to official users using cable through existing technologies of equal quality and speed. The cable internet provided shall be of a minimum speed of 50 Mbps download and 10 Mbps upload. The basic cable TV provided shall include a minimum of 70 channels and include, but is not limited to, ESPN, ESPN 2, TNT, The History Channel, TBS, CNN, FOX, AMC, and Discovery.
1.1.2 Cable/Internet Service Hookup
The contractor shall furnish all parts and supplies for official drops to include the cable/internet hookup, if required. Any broken or damaged cable/internet lines used for official drops shall be replaced at no additional costs to the Government, if determined to never have been properly installed and/or a malfunction of the product/material itself.
1.2 Maintenance Service
The contractor shall maintain, measure, and test the service system in accordance with industry standards.
If trouble is determined to be caused by factors not attributed to the provider’s equipment or service, the contractor shall notify the Government of the nature of the outage and estimated time of correction.
1.3 Termination of Service
The Government may add or terminate services in any building, outlet, or drop with no associated cost anytime during the contract. There shall be no “term” or minimum length of service required for services at any location. There shall be no termination fee for terminating services. If the Government has prepaid for any service, the contractor shall reimburse the Government the appropriate pro rata share of that cost.
1.4 Additional Capabilities
The contractor and the Government recognize that over the term of the PWS, cable and internet technology may change, and the technology, equipment, and services required by this PWS may become obsolete, uneconomical, or otherwise disadvantageous. The contractor and the Government therefore, agree to consider, in good faith, proposals submitted by either party for modification of the PWS to permit the introduction of new equipment and technology or to otherwise improve connectivity to the bases.
1.5 Customer Service
The contractor shall provide quality customer support services to 377th Force Support Squadron and 377th
Civil Engineer personnel. The service includes proactive maintenance and repair management, within 24 hour response time, identification of facility needs and property management skills, and responsible fiscal administration.
1.6 Normal Hours of Operation
The contractor shall perform the services required under this contract during the following hours:
7:00AM to 4:00PM, Monday through Friday, except government holidays. The transition period may require the contractor to perform on weekends. The contractor shall respond to official service outages when reported regardless of whether on a weekend or holiday.
1.6.1 Business Office
The contractor shall staff a Business Office (BO) to handle all complaints and requests for repairs during regular business hours (7:00AM to 4:00PM Monday through Friday except government holidays). After normal duty hours, the contractor shall be available to receive complaints and requests for repairs. A 24 hour contact number shall be provided in writing to the Contracting Officer, 377 Force Support
Squadron, and 377 Civil Engineer Division at the start of the contract and for the duration of the contract for both commercial and residential services. In addition, a point of contact (POC) and phone number will be provided in writing to the Contracting Officer for the government to reach during normal business hours (7:00AM to 4:00PM).
1.6.2 Government Holidays
The following are government holidays: New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and
Christmas.
1.7 Services and Repairs
1.7.1 Work Order Classification
Work orders will be classified by the 377th Force Support Squadron and 377th Civil Engineer Division as
Routine, Priority, or Emergency. The 377th Force Support Squadron and 377th Civil Engineer Division are authorized to reclassify/re-prioritize work orders. All cable/internet line cut repairs shall be classified as Emergency unless determined otherwise by 377th Force Support Squadron or 377th Civil Engineer
Division. The contractor shall comply with the following time limits from the time of receipt of the work order. Work orders will normally be accomplished during duty hours (7:00AM to 4:00PM Monday through Friday). The contractor is responsible for covering such situations with adequate personnel and completing the work within the stipulated time. However, occasions may arise that require work to be accomplished outside of these hours. The 377th Force Support Squadron or 377th Civil Engineer Division will coordinate these occurrences with the contractor through 377 MSG Contracting Division. Please reference herein as Appendix B for list of authorized POCs for work orders.
1.7.1.1 Routine Work Order
The contractor shall complete routine work orders within 5 duty days.
1.7.1.2 Priority Work Order
The contractor shall complete priority work orders within 3 duty days.
1.7.1.3 Emergency Work Order
The contractor shall respond to emergency work order within one hour of notification and shall complete the emergency work order within 24 hours of receipt of the work order.
1.7.2 Exceptions
Exceptions to time limits may be allowed with the written approval of the Contracting Officer. In such cases, the work shall be completed within the agreed upon time.
1.7.3 Declination of work order
The contractor has 24 hours to decline a routine or priority work order and 2 (two) hours to decline an emergency work order. Reasons for rejection must be provided in writing to the Contracting Officer. The
Contracting Officer will provide the work order and reason for rejection to 377th Force Support Squadron and 377th Civil Engineer Division.
1.7.4 Contractor Complete Work Order Form
The contractor will provide forms for the customer to sign after a work order is complete. Complete forms will be submitted to the Designated Force Support Squadron and Civil Engineer Division POC’s .
1.7.5 Administrative Records
All records, documents, and associated papers provided by the Government, generated during the period of this contract, become government property and will remain in place upon contract termination or completion. During the period of the contract, all records, documents, and associated papers shall be available for review by the Government.
1.8 Complaints
The contractor shall respond to user complaints brought to the contractor’s attention by the CO or the
Designated Force Support Squadron and Civil Engineer Division POC’s acting as the common agent for individual users. The contractor shall maintain at the BO, a written record of all complaints received during the performance of the contract showing the identity of the user, the nature of the complaint, and the contractor’s response, including the date of response. All complaints must be answered, although not necessarily resolved within a 24-hour period.
1.9 Interruption of Service
The contractor shall at all times, maintain the system in good condition and repair to provide uninterrupted service in conformance with all standards and requirements. The contractor shall limit to the shortest time possible, any interruption of service for the purpose of maintaining, repairing, or upgrading the system. Planned interruptions shall be preceded by written notice to users and shall occur during periods of lower user viewing. Designated Force Support Squadron and Civil Engineer Division
POC’s shall be notified five (5) working days in advance of interruption of service. For the purposes of computing the duration of an interruption of service, said period shall begin when the contractor receives notice from a user that service has been interrupted, or when the contractor has actual or constructive notice of said interruption, whichever is earlier, and shall cease when the service has been restored to the user.
1.10 Response Time
The contractor shall respond to cable/internet line cuts and cable/internet line damage within one (1) hour during duty hours and two (2) during non-duty hours. If serviced cannot be restored within twenty-four
(24) hours after the response, the contractor shall notify the Contracting Officer and the Designated Force
Support Squadron and Civil Engineer Division POC’s. The Contracting Officer authorizes the repair of the cut. The party (the contractor, other contractors, or the Government) who cut or damaged cable/internet lines shall be liable for the repair costs.
1.11 Contractor Support
The contractor shall provide a Point of Contact (POC) for both commercial and residential that shall be responsible for ensuring that the contract services are provided satisfactorily. The POC and alternate must be able to read, write, speak and understand English and have the authority to act as a binding agent in regards to this contract. The name of this person, and an alternate or alternates, who shall act for the contractor when the POC is absent, shall be designated in writing to the Contracting Officer, 377th Force
Support Squadron, and 377 Civil Engineer Division at the start of the contract and for the duration of the contract.
1.11.1 Contractor Employees
The Government reserves the right to restrict the employment, under this contract, of any contractor employee, or prospective contractor employee who is identified as a potential threat to health, safety, security, general well-being, or the operational mission of the installation and its population. The contractor shall perform background checks, at contractor’s expense, to ensure that said employee has not been convicted or any sexual crime or any crime against children. The contractor shall employ competent and qualified supervisory, administrative and direct labor personnel to accomplish the work performed under this contract. The contractor shall not hire off-duty Air Force personnel whose employment would result in a conflict of interest or would otherwise violate the Joint Ethics Regulation, DOD Directive
5500.7-R. Any worker dealing with the customer for installation or service must speak and understand
English.
1.11.2 Dress and Appearance
Contractor personnel shall present a clean, neat and professional appearance and be easily recognized as contractor personnel. This may be accomplished by wearing distinctive clothing showing the name of the company or by wearing appropriate badges that contain the company name and employee name. No item of military clothing shall be worn by a contractor as an outer garment.
1.12 Physical Security
The contractor shall be responsible for safeguarding all government property provided for contractor use.
At the close of each work period, all government facilities, equipment, and materials shall be secured.
1.13 Performance Meetings
The Contracting Officer shall require the contractor to meet with the Contracting Officer or administrator as deemed necessary by the Government. The contractor may request a meeting with the contracting officer as necessary. Written minutes of any such meetings shall be recorded in the contract and signed by the contractor and the contracting officer or contract administrator. If the contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the contracting officer within 10 calendar days following receipt of the minutes.
1.14 Cable/Internet Installation
Work orders will be issued by 377th Force Support Squadron and 377th Civil Engineer Division for all new installation, upgrade, and terminations of official cable/internet drops. Site surveys shall be conducted by the contractor and accomplished in a 5-day period. In order to adhere to the 5-day suspense period, the Government encourages use of e-mail as the primary method to providing the required information.
1.14.1 Interior Installation
Cable/internet lines shall be concealed behind ceilings and walls and not be run along interior walls or baseboards, unless approved by the Government. Cable outlet boxes shall be installed in accordance with commercial practices and be set flush with finished walls. The diameter of any holes passing through walls shall not exceed the diameter of the cable/internet line or conduit by more than one-eighth inch. All exterior holes shall be sealed to prevent entry of moisture or insects into the facility.
1.14.2 Exterior Mounting
Cable/internet lines shall be installed parallel or at right angles to walls or structural members. Mounting shall follow building lines where possible (i.e., brick, mortar, siding) in straight, horizontal and vertical lines with bends tied to building corners.
1.14.3 Service Drops
All overhead cable/internet lines should be reserved for remote locations where underground is not feasible or economically justified; if security is not an issue.
1.14.4 Clearances
Cable/internet lines shall not conflict with or cause any other cabling to violate ground or power line clearance criteria unless a waiver has been granted by the base Civil Engineer or designated representative.
1.14.5 Underground Connections
1.14.6 Installation
Cable/internet lines shall be installed in conduit not less than two (2) inches in diameter under paved areas and roadways. Conduit shall be zinc-coated rigid steel or scheduled 40 PVC. Conduit shall be extended not less than two (2) feet beyond pavements and roadways when such roadway is used for vehicular traffic. Under heavily traveled roadways, conduit shall be installed by boring. Pavement may be cut and repaired in an approved manner on lesser traveled roads on approval of the base Civil Engineer.
1.14.7 Trenching
Trenches, in which direct burial cables are placed, shall have thirty six (36) inches as minimum depth with marker at 1’ below grade, shall not be less than six (6) inches wide, and shall generally be in straight lines between cable/internet connections. Bends in trenches shall have a radius of not less than thirty-six
(36) inches. A marker, tape six (6) inches below grade, shall be used above all direct buried cable/internet lines. Tape shall bear the warning “CABLE.”
1.14. Excavation and Backfilling
1.14.9.1 Excavation Permit
The contractor shall obtain an excavation permit from the base Civil Engineer or a designated representative before commencing any digging or excavation on the base. All new cable/internet line drops or installs must be buried underground outside of facilities or use existing underground available conduit. The contractor shall obtain a Base Civil Engineering Work Clearance Request, AF Form 103, prior to any trenching or digging and an AF Form 332, prior to any facility, manhole, or hand hold modification. Trenching, excavation, confined spaces entry, confined spaces atmospheric testing/forced air ventilation, and marking and barricading of open trenches are to be performed within IAW
Occupational Safety and Health Act (OSHA) standards, local procedures, and other provisions of the
PWS as listed.
1.14.9.2 Excavation Methods
Hand digging is required within four (4) feet of existing utilities identified by excavation permit and in areas immediately adjacent to and within approximately one (1) foot of buildings.
1.14.9.3 Backfilling
Backfilling shall be accomplished with well-pulverized soil and each twelve (12) inch layer of soil shall be compacted to a CE 55 density if at least ninety-five (95) percent.
1.14.9.4 Open Excavation
Open excavation shall be barricaded with appropriate safety barricades when contractor personnel are not present in the immediate vicinity of the work site.
1.14.9.5 Pavement Cuts
Pavement cuts shall be made only after the approval of the location and circumstances by the base Civil
Engineer. Traffic barricades and warning lights to mark the excavation shall be provided by the contractor. The restored pavement shall be equal to or better than the original pavement.
1.14.10 Final Cleanup
After the work is completed, the work site shall be returned to its original state.
1.15 Equipment/Supplies/Manpower
The contractor will purchase and provide all equipment and supplies or manpower for all official drops.
2.0 Government Furnished Property and Services
2.1 Entry/Escort Service
When required, the Government POC will arrange for entry and escort service if needed for Kirtland
AFB.
2.2 Points of Contact
Telephone numbers for the 377th Force Support Squadron and 377th Civil Engineer Division POC’s shall be provided to the contractor no more than 30 days after award of contract. As such, the contractor shall also provide telephone numbers for commercial and residential POC to the Contracting Officer no more than 30 days after award of contract.
2.3. Facilities
The Government has no obligation to provide facilities or property on Kirtland AFB.
3.0 DOCUMENTS INCORPORATED BY REFERENCE
The contractor shall comply with the standards and requirements set forth in the regulations and manuals pursuant to the industry they are supplying the services for.
