PERFORMANCE WORK STATEMENT

FOR

CHILD DEVELOPENT CENTER LINEN

CLEANING CONTRACT

SECTION I

1.0 DESCRIPTION OF SERVICES/GENERAL: The contractor shall provide all personnel equipment, tools, materials, supervision, and other items and services necessary to perform the laundry cleaning in accordance with industry standards and as required in this Performance Work Statement.

1.1. SCOPE OF WORK: The contractor shall accomplish all tasks to meet the requirements of the Performance Work Statement (PWS). Laundry service delivery standards include; pick-up and delivery are met, correct quantities of laundry are delivered, laundry is delivered free of all visible dirt, debris, dampness, offensive odors, mildew, cleansing chemical residue, has a uniform appearance, and items are properly finished and packaged.

Exchange of signed receipts accompanying laundry, between the Contractor and the Government is specifically required.

1.2. PERSONNEL

1.2.1 Contract Manager: The contractor shall ensure a full-time contract manager or designated representative is available to ensure performance of the work is conducted in accordance with this statement of work. The contractor shall ensure that the contract manager or a representative. other than the driver. will visit vehicle maintenance on a quarterly basis. The name of the contractor or representative shalI be designated in writing and provided to the contracting officer.

1.2.1.1. Contractor Representative: The contractor will be available by cell phone. phone. e-mail. and fax for routine matters in lieu of the contractor representative being available during normal duty hours to meet on the installation. However, when necessary the contractor or his designated representative will be available .to meet on the installation within 72 hours.

1.3. Pick-up and Delivery: At point of pick-up the contractor shall weigh or count the linens and a Center representative shall sign the original pick-up receipt, signature constitutes an agreement to the count. The Cost Center will retain a copy of the slip and the contractor will retain the original. The contractor shall record the weight or count of articles delivered on the delivery receipt and a Cost Center representative will verify this weight or count. Items delivered must be 100% of items picked up. The items delivered shall be clean dry and bundled. Damaged items shall be returned in a separate bundle labeled "damaged."

1.3.1 Schedule: The contractor shall conduct pickup linens on Monday's or Thursday's before 10:00am and delivery shall be done on Monday's before 10:00am. Workdays exclude Saturday's Sunday's Holidays observed by the Military/Government and or otherwise specified (i.e. down days). A Child Development Center representative will contact the contractor to inform them of scheduled changes.

1.3.2. Locations: The contractor shall conduct pick-up and delivery at the Child Development Center located at 1305 W. Fitzgerald Blvd, Building 6457, at Edwards Af-B. CA 93524.

1.3.3 Accountability and Inspection: The contractor shall conduct I00% count of linens with Child Development Center representative during pick-up and delivery to ensure accurate accountability.

Inspection for stains. discoloration and torn or ripped linens shall be conducted by the contractor and

Child Development Center representative before 1,ick-up; accurate coun� shall be conducted upon return of linens the following scheduled delivery day.

1.3.3. Holiday Procedures: During official scheduled holidays, services required by this contract shall be performed on the immediate following workday and continue the regular schedule thereafter.

1.4. Damaged Items: A II laundered items returned from the laundry plant damaged. to include but not limited to items that are stained, discolored. torn or ripped. will be the responsibility of the contractor to replace at their own cost. Notification will be given to the designated Cost Center representative prior to replacement.

1.5. Contractor Quality Control Plan: Contractor shall submit a quality control plan (QCP) to the Program Manager (PM) and CO within IO calendar clays of award and changes to the plan. The QCP shall ensure that all linen services are performed in accordance with applicable commercial standards.

regulations, laws and codes. The QCP must describe the overall quality approach and quality control inspection system tasks to identify and prevent defective services from occurring and recurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the Services Summary and an internal inspection/surveillance system. The contractor shall maintain all quality control records for the duration of the contract and will make them available to the government PM and CO within 24 hours upon request.

1.6. Quality Assurance: The government will periodically evaluate the Contractor's performance by appointing a representative(s) to monitor performance to ensure services are received. The government may increase or decrease the number of quality assurance inspections as performance dictates. The PM/representative will evaluate the contractor's performance through intermittent on-site inspections. Any discrepancies will be investigated by the PM or representative and a final determination will be made of the validity of the discrepancies.

1.7 Industry Regulatory Health Requirements- The Contractor is responsible for all compliance with any certification or labeling requirements arising from health regulations. The contractor shall furnish laundry bags and containers for the storage and collection of dirty linens.

1.8 Workmanship and Sanitation- Delivered finished articles shall conform to the generally accepted industry standards and/or Department of Health for quality of cleanliness, finish and appearance. The contractor's facility shall be open to inspection of sanitary conditions at any time by the representative of the Government; the right to perform or cause to have performed any test necessary to determine the sanitary condition of articles delivered by the Contractor, is reserved by the Government. The sanitary condition of delivery vehicle and the handling of clean laundry according to regulations are also open to inspection.

2.0 SERVICES SUMMARY: The contractor service requirements are summarized into performance objectives that relate to mission essential items. The performance thresholds briefly describe the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

I. The Contractor shall pick up all dirty items every Monday before 10:00am. clean all the dirty items and shall bring back those dirty items cleaned the following week (For example. if the Contractor picks up I00 dirty items, the Contractor will clean those I00 dirty iten1s and return those I00 items cleaned the following week. etc.). Every item that is picked up dirty must be cleaned and returned the following week.
