STATEMENT OF WORK

FOR

Repair Active

Vehicle Barriers

Gate 19B

WRIGHT-PATTERSON AFB, OH

GENERAL: The Contractor shall provide all supervision, labor, materials, equipment, and transportation to accomplish this project in accordance with this Statement of Work, project drawings, and the requirements contained in the original documents. All maintenance and repair work performed under this shall be in accordance with industry/commercial practices as well as all local, state, and federal regulations. Ensure areas are clean, orderly and free of debris upon completion of each service. If manufacturers’ recommended parts are not available, replacement parts shall be equal to or better than the manufacturer’s recommended part. Technicians performing work under this PWS shall be trained have a minimum of 2 years certifiable experience inspecting, testing and maintaining GRAB, AVB, Swing, Sliding and Drop Arm Gate systems. Contractor shall provide documentation of the technicians training and experience along with their proposal for evaluation.

1.

1.1. The Contractor Officer Representative in charge of this work shall personally contact James

Carpenter, 88 ABW/CEOFP (657-8596) prior to starting work.

1.2. The work areas will remain open to and occupied by U.S. Government personnel and / or equipment during this work.

1.3. “Provide” is defined as furnish new, install and make operational.

1.4. Secure staging areas are not available. Contractor will be responsible for protecting equipment and materials. Prior to the start of any construction operations, a schedule of work or operations in proper sequence shall be submitted by the contractor for approval by the Fire Protection Branch, Security Forces, Safety Officer and the contracting office so as to cause a minimum amount of disruption to the normal flow of traffic on streets, pedestrian travel, base security and facility operations.

1.4.1. The Government retains the right to limit the number and location of all material storage, construction and office trailers, temporary structures of any kind, open storage, and staging areas requested by the contractor to support this project. The contractor shall be responsible to submit a site layout plan identifying the number, purpose, and proposed location of any and all material storage, office and construction trailers, and temporary structures required for accomplishment of this project. The plan shall also identify any and all required open storage and staging areas. This plan must be submitted to 88 ABW/ CEOER for review and approval.

1.5. Contractor shall visit the site to thoroughly familiarize themselves with all details of the work and working conditions, to check and verify all dimensions in the field, and shall advise the Contracting Officer of any discrepancies before performing any work. The Contractor shall be specifically responsible for the coordination and proper relation of all work for the entire project and for the work of all the trades, and for the neat appearance, correct fitting, required quantities, etc., of all work in this project.

1.6. All work shall be done in a professional and craftsman-like manner following applicable codes and standards of the building trades and of the US Air Force. All work shall follow the WPAFB Base Facility Standards (BFS).

1.7. All materials shall be installed per manufacturer's recommendations unless specified otherwise.

All materials shall be utilized within the manufacturers limitations and intended purposes.

1.8. The Contractor shall protect against construction debris, including dust, from entering non-project areas.

1.9. In the event of damages of any nature caused by this work due to improper protection, precaution or safety measures, such damages shall be repaired or such property shall be replaced by the Contractor at no cost to the Government. In the event the Contractor does not satisfactorily repair or replace such damage caused by the work of this contract, the Government will make the necessary corrections and the Contractor shall reimburse the Government for inconveniences, labor, and materials involved.

1.10. It shall be the responsibility of the Contractor with assistance from base personnel to locate all existing utility lines in the field before starting excavation of any kind. The Contractor shall obtain a signed AF Form 103, Base Civil Engineering Work Clearance Request prior to excavating. The Contractor shall notify Government Contracting Officer Representative (COR) 72 hours in advance to schedule an on-site clearance. In order to prevent damage, all digging around utilities shall be done by hand.

1.11. All required utility outages shall be approved through Government Contracting Officer Representative (COR) no less than 7 days advanced notice. Request shall contain date, time and duration of outage.

1.12. Government Furnished Property (GFP) will not be included in this project.

2. COORDINATION AND SCHEDULING

2.1. Contractor shall perform work required under the contract during the following hours: 0700 hours (7 A.M.) to 1600 hours (5 P.M.), Monday through Friday, excluding weekends and U.S.

Government holidays. If the Contractor wishes to work periods other than above, permission must be requested from the Contracting Officer at least 48 hours in advance of the desired work period.

2.2. Coordinate all construction activities with: Project Inspector James Carpenter(657-8596), Contracting Officer, Base Fire Department (257-4075) and security Police (257-6516)

3. PERFORMANCE PERIOD: Project to start April 3, 2017 and be completed no later than May 26, 2017.

3.1. LIQUIDATED DAMAGES (LDs): N/A

4. SCOPE: Refurbish the four Active vehicle Barriers at gat 19B. Install to be in compliance with UFC-4-

2201 and UFC-4-2203.

4.1. General Construction: Sand blast and epoxy coat plates, barrier frame, and chains. Replace all steel hydraulic lines, back hinge blocks, hinge block base plates, 22” hinge shafts, skirts, skirt brackets, and 24v LED lights.

5 HAZARDOUS MATERIAL:

5.1 Transport and dispose all hazardous/non-hazardous contents or materials from the barrier, swing, sliding, or drop arm gate systems to an approved off-base facility.

All waste discharge manifests must be signed by the 88 CEG/CEI office. Bldg. 30022 prior to any off-site waste disposal. Maintain and have available upon request, all documentation associated with the sampling analysis, transportation and destruction of hazardous and non-hazardous materials.

5.2 All work involving the removal or disturbance of ACM or PACM must be in compliance WPAFB Specification 02 08 10 or Specification 02 08 50 for utility lines.

Coordinate all sampling and design reviews through the 88ABW/CEA Toxics Program at 79049. Bioenvironmental Engineering (88 AMDS/SGPB, 56815) must coordinate on all self-help and in-house projects.

6 ENVIRONMENTAL, SAFETY, REGULATIONS, AND PERMITS: All work shall be in accordance with the applicable codes and manufacturer's instructions and recommendations.

6.1.1 Air Force Occupational Safety and Health Administration Standards (AFOSH).

6.1.2 Applicable USAF / AFMC / Wright-Patterson Regulations.

6.1.3 OSHA 1910 Standards.

6.1.4 CFR 1926, Safety and health regulations for Construction.

6.1.5 National Safety Council Data Sheets.

6.1.6 The Ohio Environmental Protection Agency (EPA).

6.1.7 Regional Air Pollution Control Regulation (RAPCA).

6.1.8 National Fire Protection Association (NFPA).

6.1.8.1 NFPA 13 – Standard for the Installation of Sprinkler Systems

6.1.8.2 NFPA 70 – National Electrical Code (NEC).

6.1.8.3 NFPA 70E – Electrical Safety in the Workplace.

NFPA 72 – National Fire Alarm and Signaling Code

6.1.9 AWWA Manual M14 – Recommended Practice for Backflow Prevention and Cross Connection Control.

7. DRAWINGS, SPECIFICATIONS, AND SUBMITTALS: N/A

7.1 WARRANTIES

7.2 The contractor shall provide warranty for all work and material for a period of 1 year from the time of acceptance.

7.3 Contractor must provide manufacture’s standard warranty information for all equipment.

8 SPECIAL CONDITIONS AND/OR INSTRUCTIONS:

8.1 The Contractor shall remove all debris from jobsite on a daily basis. Remove materials from the work site and dispose off-base.

8.2 All removed materials shall become property of the Contractor unless otherwise designated in the Statement of Work or by U.S. Government representative. Remove these materials from the work area as they accumulate and dispose off-base.

8.3 Dispose of combustible wastes at the end of each workday and under no circumstances all them to accumulate.

8.4 The Contractor shall barricade and post the work area to restrict non-project related personnel traffic.

8.5 The Contractor shall conduct his work in such a manner that it would not inhibit or cease operations being conducted in and around the project area without the written approval from the Contracting Officer.

8.6 The Contractor shall protect and preserve the existing vegetation (grass, flowers, shrubs, trees, etc.) and shall be liable for any damages to the same.

8.7 Damages of any nature caused by this work shall be repaired or damaged property replaced at no expense to the U. S. Government. If the Contractor does not satisfactorily repair or replace the damaged property, the U. S. Government reserves the right to make the necessary corrections. The Contractor shall reimburse the U. S. Government for the consequential damages and for all materials and labor used for the corrections.

8.8 Should welding or the use of any flame apparatus occur, approval shall be obtained from the Fire Inspector prior to proceeding. Notification to the Fire Inspector for a clearance requires the request be made at a minimum of 24 hours in advance of the desired "hot" work. The Base Fire Inspector requires that when using flame apparatus, the Contractor is to provide fire protection devices, such as noncombustible coverings and fire extinguishers.

8.9 The Contractor and all sub-contractors shall be in compliance with all applicable safety and security requirements.

8.10 Electrical power may not be available at each location sufficient to operate the Contractor’s power tools and/or work lights. It is the responsibility of the Contractor to provide sufficient electrical power to each work site.

8.11 The Contractor must coordinate with CE Operations Engineering personnel prior to using existing outlets.

8.12 All utilities are to be maintained for the duration of the project. The Contractor shall coordinate any requirement for temporary disconnection of a utility for any period of time through the Contracting Officer.

9 Changes to this contract shall be made only by the Contracting Officer.
