DESCRIPTION OF SERVICES/GENERAL INFORMATION

1.1 SCOPE: The contractor shall provide all personnel, equipment, tools, Laundry tickets, materials, supervision and other items and services necessary to perform Laundry Service. The contractor shall perform to the standards in this contract.

1.2

SPECIFIC TASKS

1.2.1 Pick-up. The contractor shall pick-up soiled laundry and deliver clean laundry to WPMC BLDG 830; Area A. Twenty-four (24) hour turn-around service is required on all bulk laundry items. Forty-eight (48) hour turn-around service is required for all piece items (i.e. cubicle curtains, uniforms). The Medical Center's linen shall not be mixed with other facilities' linen at any time during the processing cycle. Soiled linen items only shall be picked up from Bldg 675 Area B on Wednesdays only after pick up of soiled linen from Bldg 830.

1.2.2 Weighing. Weighing of all bulk laundry shall be done by contractor personnel in the presence of a Government representative prior to pick-up. The contractor shall verify weight by signing a pick-up ticket for the items being picked up and such signature constitutes agreement to the Government's weight unless specific exception is taken. The pickup slip will be retained by the Government for computing variance in weight when clean laundry is delivered.

1.2.3 Delivery. The contractor shall record the number of pounds/pieces for all items delivered on the delivery ticket and the Contracting Officer Representative (COR) shall verify this amount. The contractor shall provide the tickets and furnish the Government with the original and one copy on a daily basis. Difference in bulk weight for laundry returned shall not exceed 7% from bulk weight recorded on the prior day pick-up. No percentage variance shall be allowed for piece items.

1.3

PACKAGING

1.3.1 Bulk Laundry Items. Packaging of finished items shall be placed in a clean linen cart lined with unused plastic liner or a clean reusable liner. Bulk linen shall be measured in weight. All items shall be shrink-wrapped in clear plastic as bundles, marked by type of item, amounts, and size as specified in Appendix A, Packaging Requirements.

1.3.2 Piece Items. All piece items returned from the laundry service shall have the correct laundry ticket number on a stain proof hanger under a protective covering. Uniforms with the same laundry/ticket number may be placed in a single protective covering.

1.4 UNSERVICEABLE ARTICLES: After laundering, all unserviceable articles shall be separated by the contractor. These articles shall be bundled and marked unserviceable. Final determination of whether these articles stay in the system or go to salvage shall be made by the COR.

1.5

STATE OR LOCAL DEPARTMENT OF HEALTH REQUIREMENTS:

Laundry services will be inspected annually or more often in the event that there are issues or concerns with the linen services being provided. Annual inspection will be completed by Infection Prevention and Control Program Manager to ensure that current healthcare laundry guidelines are being followed.

1.6 VEHICLE/CARTS INTERIOR AND EXTERIOR SANITATION REQUIREMENTS: Sanitation of contractor-provided vehicle and carts shall include interior and exterior cleaning. Vehicle interior must be metal including floor to allow for proper sanitation. After transporting dirty/soiled laundry, the vehicle and carts interiors shall be vacuumed and sanitized. The aseptic technique required daily for sanitizing is a pressurized steam apparatus or manual cleaning with an EPA registered and approved tuberculocidal detergent germicidal. After vehicles and carts have been sanitized, they shall be free from dirt, dust, litter, debris, dry soil and odor before loading the clean articles. Soiled items and clean items shall not be transported in the same vehicle at the same time. All clean items shall be covered during transit to prevent contamination.

1.7

WORKMANSHIP AND SANITATION

1.7.1 Laundry. The contractor shall ensure that proper cleaning procedures are followed; including personnel wearing protective clothing (etc., lab coat) during finish process, proper supplies are used and washing formulas are selected to enable extraction of various stains. Delivered finished articles shall be clean, free of stains and odor, free of lint and soap residue and other foreign matter. All work performed shall conform to industry standards, under dust and vermin free conditions. Failure to perform work under these conditions could result in harm to hospitalized military members by increasing the chance for hospital born infection. The COR shall be notified of any linen held for rewash as it will affect the 7% allowable weight variance as stated in 1.2.3. Notification shall be annotated by the contractor on the daily delivery sheet.

1.7.2 Polyester/Cotton Uniforms and Drapery. The contractor shall identify and remove spots and stains from uniforms and provide extra starching as requested. Polyester/cotton uniforms and cubicle curtains shall be free of wrinkles and shrinking of fabric. All uniform pants require a straight centered crease. Uniforms shall not appear yellow or damaged from cleaning.

1.8 REPERFORMANCE OF LAUNDERING: Any items found to have been unacceptably cleaned shall be re-cleaned at no additional cost to the Government including clean items that were dropped during processing. All items identified by the COR for re-performance shall be picked up on the next scheduled pick-up day and returned within 24 hours. The contractor shall identify and pack these items separately as required in Attachment A after re-performed.

1.9

CONTAMINATED LINENS

1.9.1 The contractor must comply with all local health regulations, state laws, and Occupational Safety and Health Administration (OSHA) requirements pertaining to the transporting and handling of all linens.

1.9.2 The contractor shall treat all used linens as if they are contaminated.

1.9.3 The contractor shall ensure all employees who handle soiled linen (including those involved in collecting, transporting, sorting and washing) receiving adequate training regarding risk of exposure. The contractor shall also ensure all employees follow frequent hand washing procedures. The contractor is responsible for the health requirements for protection of his employees.

1.10 LAUNDERING INFANT'S CLOTHING: Infant's clothing, infant shirts, cotton infant blankets, and children's pajamas shall be laundered separately from all other items. The finished products shall be evaluated by the COR to ensure articles are fresh smelling, soft in touch, and free of lent. These items shall not be laundered with any soap, detergent, or cleaning agent containing the chemical "Trichlorcarbanilide". The contractor shall furnish a chemical analysis from the manufacturer to the COR for all soaps, detergents, or cleaning agents that do not have the chemical contents on the label prior to use.

1.11

LABORATORY TESTING

1.11.1 The contractor shall obtain washroom test services from an approved laboratory at no additional cost to the Government.

1.11.2 Once each quarter in the months of Oct, Jan, Apr and Jul, the contractor shall perform washroom tests for tensile strength, whiteness retention, yellowness, blood stain removal, soil removal and bleach effectiveness, in accordance with the laboratory instructions for light, moderate and heavy soil loads. The tests shall be conducted on contractor-provided TEST SWATCHES that have been placed in the same wash cycle as the items picked up at the Medical Center. Upon completion, the swatch shall then be sent to the lab for evaluation. Two (2) copies of the test results shall be provided to the COR not later than five (5) workdays from the date of receipt by the contractor from the laboratory.

1.11.3 In the event laboratory test results indicate tensile strength losses and appearance ratings in excess of the values shown in the standards chart in paragraph 1.11.4 below, the contractor shall take appropriate corrective action within five (5) working days and retest. This process shall be repeated until all values are within prescribed tolerances. The Government and the contractor will receive a copy of all test results.

SERVICES SUMMARY

The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

III.

GOVERNMENT FURNISHED FACILITY, UTILITIES AND SERVICES

3.1 Facilities: None.

3.2 Equipment: The Government will make available scales for weighing linen on the back loading dock of Bldg 830, located in Area A at WPAFB.

3.3 Emergency Medical Service: The Government will provide emergency medical treatment and emergency patient transportation service for contractor personnel who are injured or become critically ill during the performance of this contract while on the WPAFB installation grounds. The contractor shall reimburse the Government for the cost of medical treatment and patient transportation service at the current inpatient or outpatient treatment rate, as appropriate.

3.4 Record, and report promptly (within one (1) hour), to the Contracting Officer or designated Government representative, all available facts relating to each instance of damage to Government property or injury to either contractor or Government personnel.

IV.

CONTRACTOR FURNISHED FACILITY, UTILITIES AND SERVICES

4.1 The contractor shall provide all vehicles and carts required. Approximately 30 or more carts with a 400-800 pound capacity are required. Each cart must be marked with its empty weight and also be identified for WPAFB MEDICAL CENTER USE ONLY. The contractor shall provide a linen cart for each linen collection area of the Medical Center.

4.2 The contractor’s facility shall be equipped and ventilated so as to prevent dissemination of contaminants. The ventilation system shall meet state and federal requirements. The contractor's facility shall be subject to inspection of sanitary conditions at any time by a representative of the Government.

4.3 The contractor shall provide all racks and hangers for piece items; the contractor shall provide heavy duty hangers to hold the cubicle curtains during delivery and storage at the Medical Center. The contractor shall also provide protective covering for laundry/piece items and any other material required to perform the services of this contract including laundry/piece items delivery tickets.

4.4 The contractor shall provide a person (Point of Contact) who shall be responsible for the performance of the work. The name of this person, and an alternate or alternates, who shall act on behalf of the contractor when the manager is absent, shall be designated in writing to the contracting officer.

V.

CONTRACTOR EMPLOYEES:

5.1 The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the contracting officer as a potential threat to the health, safety, security, general well being or operational mission of the installation and its population.

5.2 Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee name. The contractor shall specify that all employees shall wear a clean uniform every day without rips or tears. The contractor shall specify that all employees must be clean by daily bathing to ensure clean area is not contaminated by employees.

5.3 The contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with Department of Defense, Joint Ethics Publications.

5.4 The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform; however, their absence at any time shall not constitute an excuse for nonperformance under this contract.

5.5 The contractor is prohibited from employing off-duty COR(s) who are surveying any contracts/subcontracts awarded to the contractor.

VI.

SECURITY REQUIREMENTS: The contractor shall complete a "Request for Identification Credential (AFMC Form 1440) for each employee of the contractor requiring access to Wright-Patterson Air Force Base. The requests shall be submitted to Pass and Registration (Building 10286 WPAFB, OH). The Government shall provide a completed "Identification Credential (AFMC Form 87)", which shall be issued, displayed and surrendered as directed in AFI 31-209.

VII.

QUALITY CONTROL PLAN

7.1 The contractor shall establish and maintain a complete Quality Control Plan to ensure the requirements of this contract are met.

7.2 The program shall include, but not be limited to the following:

7.2.1 Inspection System. A description of the inspection system to cover all services in the PWS. This will include the frequency of inspections and the title of the individual(s) who shall perform the inspection.

7.2.2 Deficiency Prevention. A method for identifying and preventing defects in the quality of service performed before the level of performance becomes unacceptable.

7.2.3 Inspection File Records of all inspections conducted by the contractor and necessary corrective action taken. Documentation shall be made available to the Government during the term of this contract. The inspection and corrective action records shall be maintained at a location designated in the Quality Control Plan. The inspection records format shall include; date, time and location of inspection, signature block of the person who performed the inspection, rating of acceptable or unacceptable, area designated for deficiencies noted corrective action take, and total number of inspections.

7.3 The contractor shall be required to revise his Quality Control Plan (at no cost to the Government) should the plan fail to control the quality of services provided.

VIII.

MEETINGS: The contractor shall attend all meetings convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The CO, COR, and other Government personnel, periodically will meet with the contractor to discuss the contractor’s performance, and the contractor shall apprise the Government of problems. Appropriate action shall be taken by the contractor to resolve outstanding issues. The meetings shall be at no additional cost to the Government.

IX.

HOURS OF OPERATION:

9.1 The contractor shall perform the services required under this contract during the following hours: Normal Hours. Pick up and delivery of bulk laundry will be made Monday through Saturday Bldg 830, Area A, between 0700 hours and 1000 hours. Service will be daily with a 24 hour turn around; bulk weight, e.g., items picked up on Monday shall be returned Tuesday.

9.2 The contractor is not required to provide services on the following days: New Year's Day, Martin Luther King Day, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving and Christmas Day. NOTE: In the event a holiday occurs on a scheduled pick up or delivery day, pickup and delivery shall be accomplished on the first workday following the holiday.

9.3 Unforeseen base closures because of adverse weather do not apply to the Medical Center. Medical Center personnel are mission essential and patient care is 24 hours. Therefore, there can be no delay in pick-up or delivery of laundry due to inclement weather.

X.

RECORDS: The contractor shall be responsible for creating, maintaining and disposing of only those Government required records that are specifically cited in this PWS. If requested by the Government, the contractor shall provide the original record or a reproducible copy of any such record within one (1) working day of receipt of the request.

XI.

ENVIRONMENTAL CONTROLS:

11.1 Compliance with Laws and Regulations. The contractor shall comply with all applicable Interstate, Federal, State and Local laws, regulations, and requirements. This includes OSHA regulations regarding the transporting and handling of all linens and regulations regarding environmental protection. Second, compliance with any certification or labeling requirements arising from state or local department of health regulations is the responsibility of the contractor. Third, the contractor shall follow the standards set forth in The Healthcare Laundry Accreditation Council publication, Accreditation Standards for Processing Reusable Textiles for Use in Healthcare Facilities.

11.2 Notification of Spills. If the contractor spills or releases any hazardous substance into the environment within the boundaries of Wright Patterson AFB, the contractor or its agent shall immediately report the incident by calling the Environmental Management Office at (937) 257-7297. The contractor shall cleanup any spill to the satisfaction of the base Environmental Management Office (EM) and in accordance with all applicable federal and state laws and regulations. The liability for the spill and any associated clean up costs shall rest solely with the contractor and/or its agent. In the case of imminent health and safety dangers, the Government reserves the right to take immediate action to contain a spill or to otherwise minimize the threat to public health and safety. The contractor shall reimburse the Government for the costs incurred upon receipt of a bill from the Government. In the event of a spill, the contractor may request support from the base spill team on a cost reimbursable basis.

11.3 Infectious Waste. In the event that any items, defined as infectious waste by Ohio Administrative Code (OAC) 3745-27-01(B)(15), are found commingled with the Medical Center's linen, the contractor shall leave items intact, where found, and immediately contact EM Office of Environmental Management at (937) 257-7152.

11.4 Material Storage and Use. The contractor shall follow manufacturer's guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling. This includes hazardous materials.

VII.

SAFETY REQUIREMENTS:

12.1 The contractor shall conform and comply with safety requirements and provisions contained in the contract for all activities related to the accomplishment of the work.

12.2 The contractor shall take such additional immediate precautions as the contracting officer may reasonable require for safety and mishap prevention purposes.

12.3 The contractor shall include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

12.4 In the event of an accident/mishap, the contractor shall take reasonable and prudent action to establish control to the accident/mishap scene, prevent further damage to persons or property and preserve evidence until released by the accident/mishap investigative authority through the Contracting Officer.

12.5 If the Government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.

12.6 The contractor shall include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident/mishap reporting and investigation.

VIII.

CONTINUATION OF ESSENTIAL DEPARTMENT OF DEFENSE (DOD) CONTRACTOR SERVICE DURING CRISIS: All basic services to be performed under this contract have been determined to be essential for performance during crisis and, according to Department of Defense Instruction (DODI) 1100.22 and Air Force implementation there of, it is determined that the contractor shall be required to perform during crisis. The Contracting Officer or the designated representative shall verbally advise the contractor of the effort required.

VIV.

ENTERPRISE-WIDE CONTRACTOR MANPOWER REPORTING APPLICATION (eCMRA) REPORTING (JAN 2015)

14.1 Section 2330a of title 10, United States Code (10 USC 2330a), requires the Secretary of Defense to submit to Congress an annual inventory of contracts for services performed during the prior fiscal year for or on behalf of the Department of Defense (DoD). The contractor shall ensure ALL contractor labor hours including subcontractor, at all levels/tiers, labor hours required for the performance of services provided under this contract are reported via a secure data collection site.

14.2 The contractor and all subcontractors, at all levels/tiers, providing direct labor under this contract shall report complete and accurate data for the labor executed during the period of performance during each Government fiscal year (FY), which runs from October 1 to September 30. The Contractor shall input the data into the appropriate eCMRA reporting tool, which can be accessed via a secure web site at http://www.ecmra.mil/. There are four separate eCMRA tools: Army, Air Force, Navy and All Other Defense Components. The appropriate eCMRA reporting tool to use is determined by the requiring activity being supported (e.g., if DISA awards a contract for an Air Force requiring activity, the contractor shall load the required reporting data in the “Department of Air Force CMRA” tool). While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. The contractor shall completely fill in all required data fields. The contractor shall enter initial data into the appropriate eCMRA tool to establish the basic contract record no later than 15 working days after receipt of contract award or contract modification incorporating this clause. The contractor shall notify the COR when the basic contract record has been established in the appropriate eCMRA tool.

XV.

Combating Trafficking in Persons (DFARS PGI 222.17) The contractor shall comply with FAR 52.222-50, Combating Trafficking in Persons (CTIP). The COR will monitor contractor’s performance regarding trafficking in persons such that non-compliance with CTIP is brought to the immediate attention of the CO. All DoD military and civilians must receive CTIP awareness training; it is highly recommended that DoD contractors receive CTIP awareness training. The military has zero (0) tolerance for human trafficking. CORs are to report all violations to their CO immediately, and keep a log for annual review of all concerns. The CO shall convey the information to the DoD CTIP Law Enforcement Support Office of Under Secretary of Defense (OUSD) Personnel and Readiness (P&R) Defense Human Resources Activity (DHRA). http://ctip.defense.gov. The Department of State’s Office is to Monitor and Combat Trafficking in Person at http://www.state.gov/j/tip.

ATTACHMENT 2: Definitions Laundering: The process by which soiled fabric items are cleaned and treated. The term includes cleaning by mechanical action or by hand in water solutions of detergents and other additives. For the purpose of this contract, the term includes repellent treating, bleaching, bluing, chemical rinse (sour), starching, ironing and pressing operations as appropriate to the type of fabric and nature of soil.

Clean: Finished articles are dry and free of visible soil, lint, and odors. Spots and stains shall be removed without damaging the fabric.

Piece items: Items include cubicle curtains, doctors’ coats, cooks’ aprons, uniform trousers and shirts, pantsuit tops and bottoms, and white dresses.

Unserviceable Articles: Those articles that are worn beyond economical repair, these items shall be extracted from the system at the discretion of the Government COR.

Contaminated Lines: The linens originating in isolation rooms or other areas of the Medical Treatment Facility (MTF) which are soiled and contaminated with infectious microorganisms.

Medical Treatment Facility (MTF): Those activities providing outpatient and/or inpatient health care for services for authorized personnel.

Major findings are contract non-conformances which are considered critical or major. IAW FAR 46.101 a major nonconformance means a nonconformance, other than critical, that is likely to result in failure of the supplies, or to materially reduce the usability of the supplies or services for their intended purpose. A critical nonconformance means a nonconformance that is likely to result in a hazardous or unsafe condition for individuals using, maintaining, or depending upon the supplies or services; or is likely to prevent performance of a vital agency mission. If at any time COR identifies a condition as having a significant adverse effect on the quality of the activity, such as those stated below, COR shall document their findings and notify the CO immediately in writing. Email is acceptable.

A. Contractor failure to meet a Performance Threshold.

B. Failure to provide adequate corrective action to preclude reoccurrence of Government identified findings.

C. Failure to provide corrective action to deficiencies identified by the contractor within a prescribed suspense period.

D. Any failure to adhere to security and/or safety regulations that results in a security or safety incident.

Minor Finding. IAW FAR 46.101 a minor nonconformance means a nonconformance, that is not likely to materially reduce the usability of the supplies or services for their intended purpose, or is a departure from established standards having little bearing on the effective use or operation of the supplies or services. When COR identifies a minor finding, the COR shall document the findings, but is not required to notify the CO. However, if the same minor finding is repeatedly identified, it may be an indication that a major finding is occurring or has occurred because the Contractor has not taken proper steps to prevent recurrence. In this case, the COR shall notify the CO in writing (email is acceptable).

Point of Contact: This person(s) shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. This individual(s) shall meet as necessary with Government personnel designated by the contracting officer to discuss problem areas. The individual(s) shall be available within one (1) hour to meet on the installation with Government personnel during normal duty hours. After normal duty hours of 0730 to 1630, Monday to Friday, the contract manager or designated alternate shall be available within two (2) hours. The individual(s) must be able to read, write, speak and understand English.
