| INSTRUCTIONS: In order to be found responsive to the RFP, all of the following must be met: |
SECTION A: Type Yes or No in Column D. If the Offeror indicates a "No" response(s) in Column D of Section A, the Government will assign a determination of non-responsiveness to the RFP and no further research or evaluation will be conducted.
SECTION B: Offeror will submit no more than five (5) contracts/examples total on the Scorecard to evidence past performance for the Prime and Teaming Partner/Major Subcontractor (henceforth known as "Offeror Team"). The five (5) total referenced contracts/examples may be used multiple times throughout the Scorecard to evidence relevancy. Each contract/example submitted on the Scorecard must have had three (3) months of performance within the last five (5) years. Contracts may only be submitted for the cage code pertaining to the Offeror Team who will be performing this contract (i.e. contracts submitted for cage codes for sister/parent companies will not be considered as meeting the Scorecard criteria unless that sister/parent company is actually proposed to be performing under this contract). For each line, indicate that the Offeror Team meets the minimum identified requirement by typing "Yes" under Column D. Typing "Yes" in Column D will auto-populate a score for that row in Column E. In Column H provide the contract number, applicable PWS/contract page(s)/paragraph number(s), contracting agency, and point of contact information for the contract. For non-government examples, in Column H, offeror must provide contract number, applicable project description, company name, and customer points of contact. Information in Column H will be used by the Government to independently verify Examples listed on the Scorecard. Failure to complete all fill-in information in the Scorecard (e.g. contract numbers, CO and program management contact information for contracts cited, etc.) will result in zero points for the applicable row. NOTE: Scoring in Section B is cumulative. You should answer "YES" to each line where your contract/exmple meets that criteria.
SUBMISSION OF SUPPORTING DOCUMENTATION: Provide a hard copy of the applicable PWS/contract/technical exhibit pages, Facility Clearance Letter (FCL) and SF 328, or any other document that evidences responsiveness, scope, and magnitude, along with this completed worksheet. The Offeror shall only provide applicable pages within substantiating documents ( i.e. do not submit pages of a document that do not clearly support evidence of responsiveness, scope, and magnitude) and shall highlight applicable sections within the pages for ease of identification. The Government will not search through documents to locate the applicable information. Substantiating documentation provided shall clearly transmit knowledge, so that the Government understands how your experience meets the referenced PWS requirement. If documents are not provided, not accurate or verifiable, or the Government cannot validate or clearly connect the information provided so no additional research will be performed and the Government will determine the proposal nonresponsive to the RFP or will adjust points for the applicable row to zero.
DEFINITIONS:
Contracts/Task Orders: One contract/task order constitutes a "C" type contract or a task order under a "D" type contract. Each task order issued against another basic document shall constitute as one (I) contract (i.e. Offerors may not submit multiple task orders under a "D" type contract as one contractual action) for purposes of the five (5) submitted contracts.
