SECTION 1

1. DESCRIPTION OF SERVICES

The PROTECTIVE & CLEAN ROOM CLOTHING PROGRAM provides services for the lease and cleaning of protective clothing and clean room protective clothing for the 309th Maintenance Wing (MXW) at Hill AFB, UT and Davis-Monthan AFB, AZ.

1.1. PROTECTIVE /CLEAN ROOM CLOTHING

The contractor shall provide all personnel, equipment, tools, material, supervision, facilities, transportation, and other items and services necessary to provide and maintain protective clothing. The contractor will provide and maintain contractor owned clothing in quantities listed in Appendix F, to designated delivery stations in a clean and serviceable condition. The contractor shall designate a mutually agreed life expectancy to each type of clothing in writing to the Contracting Officer (CO).

1.2. IDENTIFICATION OF LAUNDRY ITEMS

The contractor shall establish and implement a method of identifying the clothing by delivery station, size of garment, service start date, and inventory control number that is legible at all times, including clean room clothing plastic packaging.

1.2.1. Clean Room garments shall be packaged separately, with label showing, grouped and bagged by delivery station prior to delivery.

1.2.2. Labels shall be a minimum of 1” H X 3” L, with a minimum of 8 pitch, times new roman font (or similar). Labels shall be written in English. Labels shall be attached to garment in such a manner to remain with the garment for the serviceable life of the garment.

1.3. PICK UP AND DELIVERY

The contractor shall assume responsibility for pickup of soiled laundry and delivery of clean laundry. The contractor shall furnish in writing to the Contracting Officer (CO) a schedule for pickup and delivery times to each designated delivery station. The schedule shall be submitted 10 days prior to contract start date and shall be updated within 5 days of change.

1.3.1. Article Count (PICKUP/DELIVERY) The contractor shall establish and implement a process for inventory, accounting, pickup and delivery of clothing. The process must include:

1.3.1.1. Clothing count and exceptions document with signature blocks for the designated government delivery stations representatives and the contractor representative.

1.3.1.2. A turnaround time of 7 days.

1.3.1.3. After the seven day turnaround: Articles missing, or incorrect sizes, shall be returned by the next business day.

1.4. WORKMANSHIP AND SANITATION

The contractor shall ensure that cleanliness, finish and appearance will meet industry standards.

1.4.1 Clean room clothing shall meet Air Force standard; TO 00-25-203 sec. VIII, paragraph F, and, each pick up point batch of clean room clothing shall be cleaned separately as a batch. The Government has the right to sample and test any batch, to confirm that garments meet AF standards. When batch testing finds articles that are not in compliance with the above standard, entire batch may be rejected for re-cleaning at no additional expense to the government.

1.5 INVENTORY

The contractor shall inventory all articles of clothing annually. The contractor shall furnish a letter report of lost garments within 15 days after the completion of inventory. Contractor shall replace and be reimbursed for missing clothing unless the government can demonstrate the garments were not returned by the contractor.

1.6 REPAIR / REPLACEMENT OF ITEMS

The contractor shall repair or replace IAW the following guidance:

Unserviceable garments shall be replaced with new garments. The government assumes no liability, and no payment will be made, for any items replaced after garment has reached its life expectancy.

If the government causes a garment to be unserviceable or lost, the replacement cost will be calculated by multiplying the remaining expected service life of the garment by the contract unit price of that garment (Appendix F of the contract). Leased garments that remain serviceable may be kept in service indefinitely.

Expected serviceable life of garment is listed in Appendix F.

1.6.1 The contractor shall repair all rips, tears, open seams and holes at no additional cost to the Government.

1.7 CHANGES IN QUANTITIES OR GARMENT TYPE

Any changes in garment types or quantities leased/cleaned must be approved by the Contracting Officer via contract modification. These changes will be coordinated through the Government Contract Manager, who will coordinate with the Contracting Officer.

3. GENERAL INFORMATION

3.1. CONTRACTOR REPRESENTATIVE (CR)

The contractor shall provide a contractor representative who shall be responsible for the performance of the work, the name of this person and an alternate, who shall act for the contractor when the representative is absent, will be designated in writing to the CO within ten days after contract award. The contractor shall provide telephone numbers of the contractor representative and alternate(s) and where these persons can be contacted outside of normal duty hours. The contractor representative or alternate(s) shall have full authority to act for the contractor on all contract matters relating to the daily operation of this contract and shall have full signatory authority. The contractor representative or alternate(s) shall meet as necessary with government personnel designated by the CO to discuss problem areas. The contractor representative or alternate(s) shall be available Monday through Friday during day shift duty hours. The contractor representative and alternate(s) shall be able to read, write, speak, and understand English.

3.2. CONTRACTOR EMPLOYEES

Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. The contractor shall provide uniforms and ensure that employees wear them at all times. The company name shall appear in large letters, not less than one and one-half inches in height, both on the chest area, and upper back of outer garments, including bad weather gear.

3.3. CONTROL OF CONTRACTOR EMPLOYEES

The selection, assignment, reassignment, transfer, supervision, management, and control of contractor employees in performance of this PWS shall be the responsibility of the contractor. However, the contractor shall comply with the general intent and specific policies set forth in the PWS, concerning conduct of employees as referenced herein. When the government directs, the contractor shall restrict/ remove any employee identified as a potential threat to the health, safety, security, general well being, or operational mission of the installation and its population.

3.4. DRIVING ON BASE

Contractor personnel shall comply with directives pertaining to operation of private and/or contractor owned vehicles on Hill AFB in accordance with; AFI31-204, appendix C.

3.5. HOURS OF OPERATION

Normal hours of operation: The contractor shall deliver laundry as required Monday through Friday between the hours of 0600 and 1530. Schedule adjustments will be approved by the CO.

3.5.1 HOLIDAYS: Recognized Holidays: The contractor will not be required to provide services on the following holidays: New Year’s Day, Martin Luther King Day, Presidents’ Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.

3.6. PERFORMANCE OF SERVICE DURING SPECIAL SITUATIONS

Special or emergency situations (i.e., weather related situations, base closures) may necessitate the contractor operating on a reduced basis. The CM will inform CR or their alternate if a situation warrants.

3.7. SECURITY REQUIREMENTS

Request for Identification Credentials (AFMC Form 496) for each employee of the contractor requiring access to Hill AFB will be the responsibility of the contractor. The completed form will be forwarded to the Contract Administrator for coordination/approval. The signed form will be returned to the contractor. The approved form shall be submitted to Pass and Registration (South or West Gate Visitors Center).

3.7.1. CONTROLLED AREA, ENTRY/BADGES

The Security Manager will complete an "Unescorted Entry Authorization Certificate" (AF Form 2586) for each employee requiring entry into controlled areas. The request shall be submitted to Pass and Registration (South or West Gate Visitors Center), and to the security manager. Badges will be displayed at all times.

3.7.2. ACCESS TO FACILITIES AND PROPERTY The government will provide the contractor access to work areas and facilities as required under this PWS. All contractor personnel shall comply with the Air Force facility requirements for vehicle registrations and personnel identification badges that allow access to facilities to accomplish the tasks defined within this PWS. AFI 91-207, Appendix C.

3.8. QUALITY CONTROL

The contractor shall provide a Quality Control Plan (QCP) to the CO prior to contract start date. The CO will notify the contractor of acceptance or required changes to the QCP.

3.9. QUALITY ASSURANCE

The government will evaluate the contractor's performance by appointing a Quality Assurance Evaluator (QAE) to monitor performance to ensure services are received. A government representative will evaluate the contractor’s performance through Quality Feedback Review, and surveillance of Non-SS items listed in this PWS. The CO will make final determination of the validity of customer complaint(s) in cases of disagreement with the Contractor

3.9.1. INSPECTION FILES

The contractor shall maintain records that document all inspections conducted by the contractor and all corrective and/or preventative actions taken. This documentation shall be available for government review.

3.9.2. SURVEILLANCE

The government QAE will perform all required surveillance in accordance with the PWS. The QAE will inform the contractor representative in person when discrepancies occur. The QAE will make a notation of the discrepancy on their tally/surveillance checklist with the date the discrepancy was noted and will request the contractor representative to initial the entry on the tally checklist.

3.9.3. CONTRACT CHANGES

Any matter concerning a change to the scope, prices, terms or conditions of this contract will be referred to the CO.

3.9.4. PERFORMANCE EVALUATION MEETINGS

The contractor may be required to meet with Government officials, CM, CA QAE and CO annually to discuss contact performance and direction as associated with this PWS. At the discretion of the CO, this meeting may be held more often. This meeting will be hosted by the government. Prior to the meeting an agenda will be established by the CM. The agenda will include topics from both contractor and government representatives. The Contractor will take minutes of the meeting. A copy of the meeting minutes will be sent to all meeting attendees. Meeting minutes will be included in the contract file.

3.10. PHYSICAL SECURITY

The contractor shall be responsible for safeguarding all government property provided for contractor use.

3.10.1. KEY CONTROL

The contractor shall establish and implement methods of ensuring that no keys/key cards issued to the contractor by the government are lost or misplaced and are not used by unauthorized persons. NOTE: All references to keys also include key cards. No keys issued to the contractor by the government shall be duplicated. The contractor shall develop procedures covering key control that shall be included in the QCP. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas.

3.11. ENVIRONMENTAL CONTROL

The contractor shall be knowledgeable of and comply with all applicable interstate, federal, state, and local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to notice requirements (see FAR 52.243-7, 43.104, 43.106) and negotiate a modification to the contract.

3.12. GOVERNMENT OBSERVATION

The Quality Assurance Evaluators (QAEs) are the authorized government representative(s) who will perform assessments of the contractor’s performance. Subsequent to contract award, the identity of the QAE(s), with a letter defining their duties and authority will be promptly furnished to the successful bidder/offeror. Government personnel, other than contracting officers (COs) and quality assurance personnel (QAPs), will from time to time, with Program Officer’s coordination, observe contractor operations. However, these government personnel may not interfere with contractor performance

3.13 SAFETY REQUIREMENTS: In performing work under this contract, the contractor shall:

Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

Take additional, immediate precautions as the contracting officer may reasonably require for safety and mishap prevention purposes. AFI91-202, appendix C.

Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

Record and report promptly (within one hour) to the CO or QAP, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident investigative authority through the CO. AFI91-202, Appendix C.

If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed. AFI 91-202, appendix C.

Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in mishap reporting and investigation.

4.1 ORIENTATION PERIOD

Upon award of a the contract, the Contractor shall have 30 days, or such time as may otherwise be agreed upon in writing, to be fully prepared to perform the contract. This requirement includes whatever is necessary, as provided by the CO at the time of award, to gain access to Hill AFB, Davis Monthan AFB and the controlled areas serviced. The Contractor understands that any Contractor employee may be summarily and immediately removed from or denied access to any government property, facility, and installation at the sole discretion of competent government authority if that authority determines such action is necessary to protect or preserve government property, safety and security, or to maintain good order and discipline.

4.2 CONTRACT PHASE OUT

4.2.1. The incumbent contractor shall provide familiarization to the government or the follow-on contractor, whichever the case may be. During the phase-out familiarization period, the incumbent shall be fully responsible for providing, maintaining, and delivering all clothing in accordance with contract performance requirements.

4.2.2. The government reserves the right to conduct site visits in all contractor-operated facilities in conjunction with the solicitation of offers for the follow-on contract. In the event the follow-on contract is awarded to other than the incumbent, the incumbent contractor shall cooperate to the extent required to permit an orderly change over to the successor contractor.

4.3 MISSION ESSENTIAL SERVICES

4.3.1 In accordance with Air Force Instruction 63-124, paragraph 2.7.1, and Department of Defense Instruction 3020.37 this requirement is not considered mission essential.

5.0. DEFINITIONS

5.1 CONTRACT MANAGER (CM): The individual who is the government’s program representative, who performs the day-to-day administration of the contract. The Contract Manager will be assigned specific duties designated in writing by the Contract Officer.

5.2 CONTRACTING OFFICER (CO): The duly appointed government agent; authorized to award or administer contracts. The contracting officer is the only person authorized to contractually obligate the government.

5.3 QUALITY FEEDBACK REVIEW: Method of contract surveillance used for acceptance of services. Relying on customer satisfaction report where contract compliance or noncompliance can be validated.

5.4 DEFECT: Any non-conformance with requirements specified in the contract.

5.5 DEFECTIVE SERVICE: A service output that does not meet the requirements or standard of performance as specified in the contract.

5.6 FUNCTIONAL AREA: The organization having responsibility for the actual performance of a given service whether performed in-house or by contract.

5.7 FUNCTIONAL COMMANDER OR FUNCTIONAL DIRECTOR (FC/FD): The individual responsible for a functional area.

5.8 PERFORMANCE REQUIREMENT: The point that divides acceptable and unacceptable performance of a task according to the Service Summary, (SS) and the Inspection of Services clause. When the method of surveillance is other than random sampling, the performance requirement is the number of defects in the lot before the government will affect the price computation in accordance with the Inspection of Services clause.

5.9 SERVICE SUMMARY (SS): Captures overall contract requirements and identifies performance objectives and thresholds used in performance analysis.

5.10 QUALITY ASSURANCE: A planned and systematic pattern of all actions necessary to provide confidence to the government that adequate technical requirements are established, products and services conform to established technical requirements, and acceptable performance is achieved.

5.11 QUALITY ASSURANCE EVALUATOR (QAE): A designated and functionally qualified government representative responsible for surveillance of contractor performance.

5.12 PERFORMANCE PLAN (PP): A written document used for quality assurance surveillance. The document contains specific methods to perform surveillance of the contractor.

5.13 CONTROL: Those actions taken by a contractor to control the production of outputs to ensure performance conforms to contract requirements.

5.14 Technical Definitions Peculiar To This Performance Work Statement (PWS).

5.14.1 LAUNDERING: The term includes cleaning by mechanical action or by hand in water solutions or detergents and other additives. For the purpose of this contract, the term includes repellent treating, bleaching, bluing, chemical rinse (sour) starching, ironing, and pressing operations as appropriate to the type of fabric and nature of soil.

5.14.2 CLEAN ARTICLES: Finished articles that are dry and free of visible soil, lint, and objectionable odors. Spots and stains are removed where practical to do so without damaging the fabric.

5.14.3 UNSERVICEABLE ARTICLES: Articles are unserviceable when the government determines that they are not suitable for minor repairs under section 1.6 or for paid repairs.

5.14.4 CORROSIVE AND CAUSTIC RESISTANT TREATMENT: Treatment

by hand or by machine of fabric items with either solvent or aqueous solutions of corrosive or caustic resistant compounds.

5.14.5 FLAME RETARDANT TREATMENT: Treatment by hand or by

machine of fabric items with either solvent or aqueous solutions or flame retardant compounds.

5.14.6 ELECTROSTATIC DISCHARGE CONTROL PROTECTION:

Treatment by hand or by machine that will provide electrostatic discharge control protection. Garments will have a surface charge of less than 50 volts. Static decay will be under 0.1 second to 2.0 second.

APPENDIX D - CLEAN ROOM CLOTHING DESCRIPTIONS

BUNNY SUITS

White woven polyester fabric, no pockets or belts. Zipper front closure. Two-ply reinforced placket. Anti-static knit cuffs. Adjustable snaps at collar and ankles to give snug protective fit without binding. Seams to be double needle stitched with continuous filament synthetic thread to prevent raveling. Strain points to be reinforced and bar tacked. Three vertical snaps on back of legs for adjustable boot attachment.

LAB COAT

White Polyester fabric. Reversible front. Acid and wrinkle resistant. Side slit openings for easy access to trouser pockets. Adjustable snaps at wrists. Back closing belt to be bar stitched.

HOOD

Full head coverage with large shoulder overlap, garment shall meet class 1000 clean room specifications per Air Force T.O. OO-25-203. Garment shall provide static electrical discharge control protection. Surge charge on garments shall be less than fifty volts; static decay shall be under 0.1 second to 0.2 second.
