STATEMENT OF WORK

FOR

EGRESS TESTING AT UTAS HURRICANE MESA TEST FACILITY

AFLCMC/EBHJ, HILL AFB, UT

1.0 GENERAL

1.1 Introduction

The Hill AFB Cartridge Actuated Device/Propellant Actuated Device (CAD/PAD) office requires a contract to support system level sled testing and egress component testing with Government Furnished Property (GFP) such as aircrew flight equipment and CAD/PAD assets at the UTAS Hurricane Mesa Test Facility (HMTF), Virgin, UT. The purpose of this testing will be to obtain performance data and comply with USAF OSS&E requirements to ensure there will be no system safety, suitability and effectiveness deficiencies due to aging and/or service environments.

1.2 Background (For Information Purposes Only)

The CAD/PAD office functions as the USAF system manager for energetic egress devices. In order to meet OSS&E requirements and be compliant with regulations, periodic testing of components is required. Component level testing is accomplished through routine ordnance assessment through the Aging and Surveillance program. The ability to obtain system level testing is limited do to availability of acceptable test facilities and cost of testing. By providing GFP to HMTF for testing, the CAD/PAD office is able to obtain critical performance data used to ensure the reliability of USAF egress systems. When the CAD/PAD office selects GFP consideration is given to the length of time the assets have been in the field, current issues reported/observed, validation of new components and which assets will provide the most data. Typical components used for testing are already beyond their shelf/service life and are of no longer suitable for use on aircraft or are new items that must undergo testing and qualification to meet airworthiness requirements.

1.3 Scope

Hill AFB CAD/PAD office has a requirement to test and evaluate explosive egress components. Testing will be accomplished at the HMTF. CAD/PAD assets and other GFP associated with egress testing will be provided as requested to support testing. All transfer of GFP shall be accounted for and managed in accordance with Federal Acquisition Regulation.

2.0 REQUIREMENTS

The Contractor shall use provided CAD/PAD for egress testing as outlined in the requirements below.

2.1 All GFP shall be properly tracked and accounted for IAW with terms and conditions located in this contract and DFARS 252.211-7007 and DFARS 252.245-7001 through 7004. Requests for additional or replacement CAD/PAD shall be submitted via email to Hill AFB. All requests and shipments of GFP shall include the record requirements of FAR 52.245-1(f) (1) (iii) (A) (1) through (5) in table format. Items that are deemed by contractor as unacceptable or no longer required for test shall have an SF 120 form submitted to USAF via hard copy at the below address or emailed to thomas.schmidt.14@us.af.mil. Disposition instructions shall be provided to the contractor. Costs associated with shipment to demilitarization location shall be covered by contractor.

2.2With written permission from Hill AFB contracting or local DCMA, the contractor may consume GFP at no cost to USAF in lieu of submitting SF 120 to the government.
2.3Any GFP at HMTF that was provided by the CAD/PAD office that is to be used by another government agency shall be transferred to that agencies contract. A written request, including items to be transferred and which contract the items will be transferred to, shall be submitted to Hill AFB and receiving agency’s contracting offices.
2.4USAF is not responsible for proper function of the CAD/PAD items. The contractor assumes all risk associated with safety, handling, storage and performance of the CAD/PAD device upon transfer. The contractor has the right to request an alternate item for testing if the item is deemed not suitable or the contractor is unwilling to accept the risk provided they are available. Items received shall be stored IAW contractor’s safety site plan.
2.5Hill AFB engineering shall be notified via email at least 15 days prior to testing and have the opportunity to witness testing as deemed necessary.
2.6CAD/PAD inventory shall be evaluated 60 days prior to exercise of option year to include NSN, P/N, lot number and quantity. Hill AFB reserves the right to inspect any GFP assets at HMTF with proper contractor coordination under mutually agreeable terms and schedule.
2.7The contractor shall provide a report (CDRL A001, DI-NDTI-80809B) of the expended items (NSN, part number, serial number and quantity) and whether any anomalies or performance issues were observed during testing. The report shall be submitted via Wide Area Work Flow (WAWF) under DoDAAC FA8213 and submitted yearly. The contractor shall notify Hill AFB engineering of any anomalies and suspected CAD/PAD performance issues identified within 3 working days.
2.8The contract shall be a one year Firm Fixed Price contract with four, one year option periods.

3.0. SAFETY REQUIREMENTS.

In performing work under this contract, the contractor shall:

3.1 Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

3.2 Take such additional immediate precautions as the contracting officer may reasonably require for safety and mishap prevention purposes.

3.3 Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) an Explosive Site Plan (ESP) IAW DoD Standard 4145.26M and a Safety Training Plan for the protection of Government facilities and property and to provide a safe work environment for contractor personnel.

3.4 Provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

3.5 Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

3.6 Record and report promptly (within one hour) to the contracting officer or designated Government representative all available facts relating to each instance of damage to Government property or injury to either contractor or Government personnel.

3.7 In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

3.8 If the Government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.

3.9 Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.

3.10 Comply with safety provisions listed in the technical publications within the SOW and other appropriate DoD safety publications.
