PERFORMANCE WORK STATEMENT FOR

JANITORIAL/MAID SERVICES FOR AIR FORCE CREW REST FACILITY

BLDG 5218, DUGWAY PROVING GROUND, UT

1. GENERAL INFORMATION FOR BUILDING 5218

a. The UTTR-Q is a Crew Rest Facility for Air Force personnel and its test and training customers, operated by the UTTR, Detachment 1 and located on Dugway Proving Ground. The building is a two-story hotel type facility. The main floor consists of the following:

b. The dayroom is fully carpeted and is approximately 20 feet by 30 feet. There is a pool table, a seating area, large television and a DVD/VCR player.

c. The conference room is fully carpeted and is approximately 11 feet by 11 feet. There is a table and 12 chairs.

d. The kitchen has linoleum tile floor, it has a sink, refrigerator, two stoves, two microwaves, two dishwashers, and wood cabinets, laminate counter tops and a television. It is approximately 20 feet by 20 feet. There is also a dining area in the kitchen that is approximately 20 feet by 20 feet that consists of 6 small square tables with 24 stacking chairs.

e. There are six (6) suites. They are two rooms, a bedroom and a living room with a bathroom between.

The rooms are approximately 11 feet by 11 feet and fully carpeted. The bedroom has a queen size bed with a headboard, four-drawer dresser, mirror, nightstand, desk and chair, television, two lamps, telephone and clock radio. The living room is furnished with a sofa sleeper, love seat, recliner, two end tables, a coffee table, two lamps, activity table and chair, television, refrigerator, microwave, 4-cup coffee maker and a telephone. The bathrooms are approximately 5 feet by 8 feet and consist of a sink, toilet, mirror and shower. The walls and floor have ceramic tile. The shower is approximately 3 feet square, the walls and floor are ceramic tile.

f. There are two (2) single rooms. They are furnished with a queen size bed with a headboard, a dresser, nightstand, lamp, television, refrigerator, activity table and chair, clock radio and a telephone. There is a shared bathroom is approximately 5 feet by 8 feet and consists of a sink, toilet, mirror and shower. The walls and floor have ceramic tile. The shower is approximately 3 feet square, the walls and floor are ceramic tile.

g. A storage room that is approximately 11 feet by 11 feet and a janitor and linen closet.

2. THE SECOND FLOOR CONSISTS OF THE FOLLOWING:

a. There are nineteen (19) rooms. They are approximately 11 feet by 11 feet and are fully carpeted.

Eighteen of the rooms share a bathroom. One room has a private bathroom. (Ten (10) total bathrooms). The bathrooms are the same as the main floor. The rooms are furnished with a queen size bed with a headboard, dresser, mirror, nightstand, two lamps, activity table and chair, recliner, television, refrigerator, coffee maker, clock radio and telephone.

b. A storage room approximately 6 feet by 6 feet and a janitor and linen closet.

c. There is a laundry room located in the basement of the building. It is approximately 20 feet by 40 feet. There are three washers and three dryers. There are three entrances to the building. Each entrance has a stairway.

3. REQUIRED JANITORIAL/MAID SERVICE DUTIES FOR COMMON AREAS:

a. The Common Areas are defined as the dayroom, conference room, kitchen and dining area, laundry room, storage rooms, janitor and linen closets, hallways, stairways and entrance foyers.

b. Twice weekly dusting of all furniture, pictures, windowsills, window tracks, P-TAC units (AC/Heating

Units), window blinds, televisions and doorframes in the dayroom, conference room, hallways, stairways and entrance foyers. All horizontal surfaces, such as windowsills, window tracks, window blinds, hand rails, wood strips, door frames, exposed piping, light fixtures, covers and diffusers, ceiling and walls within eight feet from the top of the finished floor shall be free of lint, dust, dirt, cobwebs, marks, finger prints, smudges, and other accumulated soils. Items on furniture tops are to be dusted and replaced. Upholstered furniture shall be free of dust, dirt, lint, other stains and discoloration and shall be kept free of all visible lint, liter, soil and imbedded grit.

c. Twice weekly vacuuming of all common areas – this includes, regardless of occupancy, dayroom, conference room, hallways, stairways and entrance foyers. All carpeted areas shall be vacuumed and free of all visible lint, litter, soil and imbedded grit. Any spots shall be removed by carpet manufacturer’s approved methods as soon as noticed. Area and throw rugs are to receive this service also. Vacuum/clean interior and exterior walk-off mats. After vacuuming or cleaning, mats shall be free of all visible lint, litter, and soil. Carpet style entrance mats shall be vacuumed to remove soil and dirt and to restore resiliency of the carpet pile. Rubber or polyester entrance mats shall be swept, vacuumed, or hosed-down and then dried to remove soil and grit. Soil and moisture underneath entrance mats shall be removed and mats returned to their normal location.

d. Entrances and porches shall be swept, scraped and/or mopped to remove debris on a daily basis. Work shall include removal of all debris within ten (10) feet of all entranceways. Clean-up area shall be measured from building foundations to ten (10) feet outside perimeter.

e. Weekly cleaning of the interior and exterior glass in all doors. After cleaning there shall be no traces of dust, dirt, smudges, film, tape, streaks, watermarks, or other foreign matter.

f. Twice weekly sweeping and mopping of kitchen and dining area. The kitchen and dining floor areas shall be damp and/or wet mopped. Chairs, trash receptacles and easily movable items shall be moved to maintain floors underneath these items. All moved items would be returned to their proper position when all operations have been completed. After being mopped, the floor shall have a uniform appearance with no streaks, swirl marks, detergent residue, or any evidence of soil, stains, film debris, or standing water. There shall be no splash marks or mop streaks on furniture, walls, baseboards, etc., or mop strands remaining in the area.

g. Quarterly waxing of kitchen and dining area floor. Strip, scrub, seal, and wax floors as necessary to maintain a uniform glossy appearance. A non-skid wax is required. A uniform glossy appearance is free of scuffmarks, heel marks, wax build-up, and other stains and discoloration.

h. Twice weekly cleaning and sanitizing of all kitchen and dining facilities and appliances (stoves, refrigerator, microwaves, countertops, tables and chairs, sink and dishwashers). All appliances, counter tops, tables, chairs, sink and dishwashers will be free of streaks, swirl marks, detergent residue, soil, stains and finger prints. Roll type paper towel dispenser and liquid soap dispenser will be kept stocked.

i. Dishwashers shall be loaded and operated and then unloaded daily.

j. All bedspreads will be removed and laundered quarterly.

k. All shower curtains will be removed and laundered quarterly.

l. Daily trash removal from all common areas. All trash receptacles shall be emptied and returned to their initial location with contractor provided plastic liners. All debris or liquids remaining in a trash receptacle due to a leaky plastic trash bag shall be removed. Trash shall be disposed of in a secured plastic bag. The contractor shall pick up any trash that may fall onto the facility or grounds during the removal of such trash collection. All refuse collected shall be disposed of in the trash dumpster located in the parking lot outside the building. Dirty trash receptacles shall be washed inside and out and shall be odor free.

m. Weekly cleaning (sweeping, mopping, dusting, organizing) of laundry room, storage rooms, linen and janitor closets. The storage rooms, janitor and linen closets are located on each floor. The contractor will keep storage rooms, janitor and linen closets neat, clean and orderly.

(1) Laundry facilities are located in the basement of building 5218. The stairway leading to the basement needs to be swept weekly. The laundry room needs to be swept and mopped weekly. The laundry tables should be wiped with a wet cloth weekly and kept free of debris. The washers and dryers will be wiped with a wet cloth weekly. Laundry soap and other laundry supplies may be stored in this location but must be kept neat and orderly.

(2) The janitor closets will be swept and mopped weekly. The sink in the janitor closet will be damp cleaned. After cleaning, the sink will be free of deposits, dirt, smudges and streaks, soap film, dust, soils, scum, and odors. All bright metal finishes such as faucets, pipes, fittings and hardware shall be kept in a bright and clean condition. Janitorial supplies and equipment may be stored in this location but must be kept neat and orderly.

(3) The storage rooms will be vacuumed or swept and mopped weekly. The shelving will be kept free of dust and debris. All supplies and equipment may be stored in these locations but must be kept neat and orderly.

(4) The linen closets will be swept and mopped weekly. The shelves will be kept free of dust and debris, and linens will be stored in an orderly fashion. Amenities may be stored in this location but must be kept neat and orderly.

n. Monthly cleaning of P-TAC heating and cooling unit filters. There are 37 total hotel type heating and cooling units in building 5218. They are located in every room. The acceptable ways of cleaning the filters are as follows:

(1) The filters may be vacuumed.

(2) The filters may be run under warm water and lightly tapped dry.

(3) The filters may be tapped lightly into a trash receptacle.

o. Contractor will provide all equipment this includes vacuums, vacuum bags, vacuum belts, carpet cleaners, dusters, mops, buckets and any other cleaning equipment used to accomplish the job. Before procuring any hazardous materials, other than normal household cleaning compounds, prior approval must be obtained from the UTTR Environmental Coordinator. The contractor will provide light bulbs (the contractor is responsible for the replacement of burned out bulbs in the lamps in the suites/rooms and common areas and all the bathroom light fixtures). The Base Ops Contractor will replace fluorescent tubes. All cleaning products and supplies, laundry soap and sanitizing products, trash bags, and amenities are to be provided by the contractor. The following is a list of required amenities:

(1) Four cup coffee pouches and condiment kits (2 sugar packets, 2 sugar substitute packets, 2 creamers and 2 stir sticks).

(2) Two each hot/cold disposable 8 ounce cups.

(3) Two each paper napkins.

(4) One-ounce shampoo bottles.

(5) A minimum of a one-ounce bar soap.

(6) The Government will furnish the bedspreads, blankets, flat sheets, fitted sheets, mattress pads, pillowcases and pillows.

p. One master key will be provided. An additional key can be requested and issued based on written justification. Lost master key will necessitate a core lock replacement for the entire building and will be at the cost of the contractor. In no instance can the contractor copy a master key. Broken keys will be replaced if broken parts are provided as proof of breakage.

q. The contractor will report discrepancies, such as water leaks, broken windows, fixtures, problems, strange occurrences, or other repeated similarities to Alison Dunn, (435) 831-5343. A list of high value items (TVs, VCRs, etc) will be provided. Contractor must notify the above POC’s immediately of missing items.

r. Access to this facility is restricted to Facility Management, occupants, occupant guests, cleaning service and repair contractors. Under no condition will the cleaning contractor use any facility item for personal use or permit access to any unauthorized party at any time.

4. REQUIRED SERVICES FOR THE SUITES/ROOMS

a. Janitorial/maid services are based on the following calculation: 52 weeks X 4 days per week = 208 days minus 10 holiday days = 198 normal working days. The maximum occupancy rate = 26 rooms per day x 198 normal working days for a total amount of 5148 rooms per year (could be a combination of suites and single rooms), Monday through Thursday, excluding holidays. The building is not fully occupied at all times, therefore, based on historical information the average occupancy is estimated at 12 rooms per day. A guaranteed minimum up to 12 rooms is for Suites/Rooms cleaned, will be a bi-monthly (24 each) cost. Suites/Rooms cleaned, above 12 rooms, will be a per room rate.

b. A daily sheet or phone call will be provided to the contractor to inform him/her of all rooms to be serviced.

DAILY ROOM SERVICE

1. If an occupant stays for more than 1 day/night, maid services are required as follows:

2. Restrooms will be cleaned and sanitized daily. (Occupied rooms only). Completely damp clean and disinfect all surfaces of the wash bowl, toilet bowl and seat, shower, dispensers, plumbing fixtures, door, walls, polished exposed piping, mirrors, and other such surfaces, using germicidal detergent. After cleaning, receptacles will be left free of deposits, dirt, smudges and streaks, soap film, dust, soils, scum, and odors. All bright metal finishes such as faucets, pipes, fittings and hardware shall be kept in a bright and clean condition. After sweeping and mopping the floor, the entire surface shall be free from litter, dust, and foreign debris, including grout. Grout on wall and floor tiles shall be free of dirt, scum, mildew, residue, etc. Floors shall have a uniform appearance without streaks, swirls, marks, detergent residue, or any evidence of soil, stain, film, or standing water. Easily moveable items shall be tilted or moved aside to sweep and damp mop underneath.

3. Make beds daily. Soiled bed linen shall be removed, changed and laundered once weekly. (Occupied rooms only).

4. Bath towels, hand towels, face cloths and bath mats shall be changed and laundered daily. (Occupied rooms only).

5. Amenities will be placed in the suites/rooms daily. (Occupied rooms only).

6. Daily trash removal from occupied suites/rooms. All trash receptacles shall be emptied and returned to their initial location with contractor provided plastic liners. All debris or liquids remaining in a trash receptacle due to a leaky plastic trash bag shall be removed. Dirty trash receptacles shall be washed inside and out and shall be odor free.

7. Daily vacuuming of occupied suites/rooms. All carpeted areas shall be vacuumed and free of all visible lint, litter, soil and imbedded grit. Any spots shall be removed by carpet manufacturer’s approved methods as soon as noticed.

CHECKOUT

1. Once an occupant is no longer staying. Maid services are required as follows:

2. Restrooms will be cleaned and sanitized. After descaling, the entire surface shall be free from streaks, stains, scale, scum, urine deposits, mineral deposits, and rust stains. After sweeping and mopping the floor, the entire surface shall be free from litter, dust, and foreign debris, including grout. Grout on wall and floor tiles shall be free of dirt, scum, mildew, residue, etc. Floors shall have a uniform appearance without streaks, swirls, marks, detergent residue, or any evidence of soil, stain, film, or standing water. Easily moveable items shall be tilted or moved aside to sweep and damp mop underneath.

8. All carpeted areas shall be vacuumed and free of all visible lint, litter, soil and imbedded grit. Any spots shall be removed by carpet manufacturer’s approved methods as soon as noticed.

5. JANITORIAL/MAID SERVICES ON WEEKENDS/HOLIDAYS/EMERGENCY & SPECIAL

EVENTS

a. Janitorial/maid services are based on an as needed basis. Based on historical information on Weekends, Holidays, Emergency and Special events, the overall minimum hours used on this requirement was a total of 20 hours for a year. This will be a per hour rate.

b. WEEKENDS/HOLIDAYS SERVICES: Will be required on an as needed basis, based on Air Force mission requirements. Contractor will be notified by Close Of Business on Thursday or as soon as possible in the event of an emergency request, if there is a weekend requirement. Work required will be as outlined in

Section 3 through 4.

c. EMERGENCY CLEANING: Upon notification by the Air Force, the contractor shall respond within 3 hours. The contractor shall perform emergency cleaning required due to broken or leaking pipes, sinks, toilets or other occurrences requiring immediate corrective action to restore an area to its normal state of cleanliness and appearance. Work required will be as outlined in Section 3 through 4.

d. SPECIAL EVENTS CLEANING: These events include VIP visits, Inspector General team visits, and other events determined by the Air Force. Cleaning services include any work identified above. The suites/rooms may need to be refreshed to meet newly serviced room standards. The Air Force will notify the contractor as soon as the requirement is known, but no less than 24 hours prior to the event. Work required will be as outlined in Section 3 through 4.

6. INSPECTIONS

a. The COR will complete routine inspections of the contractors work at regular intervals. Inspections will ensure that janitorial/maid services are completed according to prescribed standards. An Inspection/Deficiency report will be completed by the COR at the end of each inspection.

b. Inspection/Deficiency reports will be provided to the contractor and the Contract Administrator within 1 day of inspection. If there are any discrepancies they will be noted on the Contract Discrepancy Report.

c. Discrepancies are to be corrected immediately if the contractor is present during the inspection. If the contractor is not present during the inspection, discrepancies are to be corrected within three (3) hours after notification. Discrepancies that are not corrected within the stated time frame will result in a deduction of payment due.

d. Deductions will be calculated as follows:

Monthly charge for service of 6 rooms = A divided by 6 rooms = B divided by 30 days = C. C will be deducted for each room where discrepancies occur. Extra rooms over the normal 6 are charged at D each.

Discrepancies in the extra rooms will be deducted at the rate of D. Monthly charge for service in the common areas = E divided by 30 days = F. F will be deducted for discrepancies in the common areas.

ALL DOLLAR VALUES AND RATES USED IN THIS EXAMPLE ARE FOR ILLUSTRATIVE

PURPOSES ONLY AND ARE NOT MEANT TO BE REPRESENTATIVE OF ACTUAL VALUES TO BE

USED.

An example is as follows:

Monthly charge for 6 rooms = $427.50 (A) divided by 6 rooms = $71.25 (B) divided by 30 days = $2.38 (C).

$2.38 will be deducted per room for discrepancies. Extra rooms over the normal 6 are charged at $6.50 (D) each. Discrepancies in the extra rooms will be deducted at the rate of $6.50 (D) each. Monthly charge for service of the common areas = $935.00 (E) divided by 30 days = $31.17 (F).

$31.17 will be deducted for discrepancies in the common areas.

7. DUGWAY PROVING GROUNG GENERAL REQUIREMENTS

a. LOCATION OF US ARMY DUGWAY PROVING GROUND

U.S. Army Dugway Proving Ground is located in a remote area. The main gate is approximately 90 miles southwest of Salt Lake City, Utah. Contractors may be required to travel up to 150 miles one way from Salt Lake

City to reach some work sites.

b. NORMAL WORK HOURS

U.S. Army Dugway Proving Ground operates on a four (4) day work week, Monday through Thursday, from

7:00 A.M. to 5:30 P.M. (local time) except for Federal Holidays. Deliveries will be accepted on regular scheduled work days between 7:00 A.M. and 4:30 P.M. (local time). The following Federal Holidays are observed:

c. 3rd Monday in February h. November 11th

d. Last Monday in May i. 4th Thursday in November

e. July 4 th

j. December 25th

When any of the above holidays fall on a Sunday, the following Monday will be observed as the holiday; when a holiday falls on a Friday or Saturday the preceding Thursday is observed as the holiday

REQUIRED SERVICES FOR THE COMMON AREAS

The Common Areas are defined as the Dayroom, Conference Room, Kitchen and Dining Area, Laundry Room, Storage Rooms, Janitor and Linen Closets, Hallways, Stairways and Entrance Foyers. The daily tasks will be performed Monday through

Thursday. The twice weekly tasks will be performed as follows: Dayroom, Conference Room, Kitchen and Dining Area

Hallways and Stairways and Entrance Foyers and Porches will be done on Monday and Wednesday. The Laundry Room, Storage Rooms and Linen Closets will be done on Tuesday and Thursday. Descaling will bedone on the last Thursday of the month. The waxing of the kitchen and dining area floor will be done in April and October.

Coordination will be required by the Contractor and the UTTR.

The daily tasks will be performed Monday through Thursday. Descaling will be done on the last Thursday of the month.

Laundering of the bedspreads and shower curtains will be done quarterly. This task may be spread throughout the year so all bedspreads and shower curtains are not done at the same time. Coordination between the contractor and the UTTR will be required.
