PERFORMANCE WORK STATEMENT (PWS)

FOR

Teradyne Application Support

Prepared by

Section One

1. Description of Services

1.1 Scope: The Contractor shall provide all labor, materials, transportation, travel and manuals necessary to perform Applications Engineering Support at the Oklahoma City–Air Logistics

Complex (OC-ALC), Tinker Air Force Base (TAFB).

1.2 Required Services: Contractor shall provide on‐site Applications Engineers to assist with software wrappers, programming, applications, bus model development and hardware, firmware and driver support for Teradyne instrumentation. Instruments include the Teradyne Bi‐411 serial bus test instrument and the Teradyne M9 and Di series digital test instruments. Contractor shall provide engineers to assist for a minimum of five (5) day increments, up to twenty-six (26) weeks over a one year period.

1.2.1 The Contractor shall contact the Government Subject Matter Expert (SME) and Contracting

Officer Representative (COR) to schedule the assessment delivery dates fourteen (14) business days before arriving to TAFB. The Contractor shall notify the COR of any changes to the assessment delivery schedule/dates.

1.3 Service Report: Immediately upon completion of the support services and before leaving the

OC-ALC, the Contractor shall provide a written Service Report to the SME. When necessary the

Government SME will forward the service report to the COR within two (2) business days of receipt. The Contractor shall provide, by fax or email, the final service report within five (5) business days to the COR. As a minimum, the final service report shall contain the following information:

name of the company requested service date contract number description of services provided additional services recommended (if any) printed name and signature of engineer who performed the services printed name and signature of on-site Government SME or COR start and completion dates complete breakdown of the labor cost (hours and rate) details of any other charges

1.4 Hours of Operation: The Contractor shall perform the work required under this contract during the following days: Monday through Friday, for 8 hours between the hours of, 0700-1600.

1.5 Contractor Personnel: The Contractor shall provide the name of the Contract Manager (CM) and any alternates who shall act for the Contractor when the CM is absent. They shall be designated to the CO and COR, in writing, within five (5) business days after the contract award date. The Contractor shall provide telephone numbers and email addresses where the CM and alternates may be contacted at any time. The CM or alternate shall have full authority to act for the Contractor on all matters relating to operation of this contract at the OC-ALC on TAFB. The

CM or alternate shall be available to meet on the installation with Government personnel designated by the CO to discuss problem areas. The Contractor shall provide a CM who can read, write, speak, and understand English fluently.

1.5.1 The Contractor and Contractor personnel shall be required to identify themselves as

Contractor personnel by introducing themselves or being introduced as Contractor personnel. The

Contractor personnel shall also display distinguishing badges or other visible identification for meetings with Government personnel or while performing work for the OC-ALC on TAFB. The

Contractor and Contractor personnel shall identify themselves as Contractor employees in telephone conversations and in formal and informal written correspondence.

1.6 Listing of Contractor Employees: The Contractor shall provide an initial listing of names and position titles of employees who will come to TAFB to the COR within five (5) business days after the contract award date. Within five (5) business days, the Contractor shall also provide an updated listing of personnel to the COR when employees affecting the work of this contract change status or position.

1.6.1 The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security, and/or general well-being, or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States (US) Government if the employment of the person would create a conflict of interest. The Contractor shall not employ any person who is an employee of the Department of the Air Force (AF), either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the

Contractor shall not employ any person who is an employee of the Department of the AF if such employment would be contrary to the policies contained in Air Force Instruction (AFI) 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibited from employing off-duty CORs who are providing surveillance on any contracts/sub-contracts awarded to the Contractor. All Contract personnel, depending on particular condition, occasion, or place or job assignment shall be required to wear professional and suitable attire. All Contractor personnel shall conduct themselves in a professional and courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor personnel in performance of this PWS shall be the responsibility and prerogative of the Contractor; however, the Contractor shall comply with the requirements set forth in this PWS and TAFB regulations concerning the conduct of employees as referenced herein. The Contractor shall employ and maintain a technically trained and experienced work force.

1.7 Housekeeping: The Contractor shall keep the work areas clean and neat IAW industry standards, OSHA, fire and safety standards, and this PWS.

1.8 Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Tinker AFB per AFMAN 31-116, in its entirety, “Motor Vehicle Traffic Supervision”. The Contractor shall comply with Tinker AFBI 24-302, in its entirety for the operation of POVs within Tinker AFB.

1.9 Common Access Card (CAC) Holders: IAW OC-ALCI 31-1, Chapter 6, paragraph 6.5.2 –

6.5.3., Contractor personnel requiring access to Tinker AFB and the Tinker Domain (computer and network access) shall possess a Common Access Card (CAC). The CAC shall be obtained through the Trusted Associate Sponsorship System (TASS)

Trusted Agent (TA). The Contractor shall contact the COR within five (5) business days after the contract award date for details and forms. Contractors whose personnel require CAC cards shall have initiated a National Agency Check with Inquiries (NACI) as described below and shall have completed (with favorable results) a Federal Bureau of Investigation (FBI) fingerprint check, or a

DoD determined equivalent investigation or greater before the CAC can be issued.

The investigation requirement for a NACI is IAW AFI 31-601, “Industrial Security”. In order to receive access to the Tinker Domain and access to any Government computer system, the

Contractor shall have at least initiated a NACI for personnel requiring access. The Contractor is responsible for vetting their employees. If an unfavorable NACI is received, access to the Tinker

Domain will be revoked. The Contractor shall ensure that Contractor personnel requiring access to the Tinker Domain shall have a civilian Local Files Check (LFC) with no adverse results, as required by the TA.

1.10 Foreign Nationals: The Contractor shall notify the COR before sending a Foreign National representative to perform services. Special rules apply for foreign nationals visiting a US

Government facility.

1.11 Safety and Health on Government Installations: While performing work under this contract on a Government installation, the Contractor shall comply with federal, state and local regulations, and AFI 91-203, “Air Force Consolidated Occupational Safety Instruction”, as established by this contract and Appendix C. The Contractor shall notify the CO and COR within one (1) business day of any injuries that occur to Contractor personnel while they are on the installation. The Contractor shall notify the CO and COR within one (1) business day of damage to Government property or equipment during the execution of the contract.

1.12 Emergency Procedures: The Contractor personnel shall follow the direction of the

Government personnel with regard to emergency procedures, such as fire, tornado, active shooter, bomb threats, or other emergency. It is the responsibility of the Contractor to provide guidance and to establish procedures for its employees to respond to an emergency. It is the Contractor’s responsibility to account for their employees following an emergency evacuation. The Contractor personnel shall participate with the Government personnel in all fire and tornado drills at no additional cost to the Government.

1.13. Training – General: The COR will provide training via slides, video, or classroom prior to the Contractor performing work at the OC-ALC. The Government will be responsible for all approved costs associated with the training identified below. The COR will document training on and maintain copies of the completed AF IMT 1151, Training Attendance and Rating, as part of the contract surveillance file. For purposes of identification and compliance with Personally

Identifiable Information (PII) requirements, only the last four (4) of the Contractor employee’s

Social Security Number (SSN) will be input on the form.

1.13.1 The Contractor shall contact the COR prior to work being performed in order to accomplish the required training or to verify the required training as listed below has been completed:

Section Three

Government Furnished Property (GFP) and Services

3.1. Government Furnished Property.

3.1.1. Government Property Repair/Maintenance. Not applicable

3.2. Government Furnished Services (GFS) are applicable during the performance of this contract. The Government will furnish the following services at TAFB, OK:

3.2.1. Security Forces. The Government will provide general on-base security forces’ service.

The security forces’ telephone numbers are 911 for emergencies, 405-734-2000 for crimes in progress and 405-734-3737 for non-emergency calls. The off-base 911 center receives 911 cell phone calls. Cell phone callers need to advise the 911 off-base center they are on TAFB in order to be connected with the on-base 911 center.

3.2.2. Fire Emergency and Routine Services. The Government will provide general on-base fire protection services. The Contractor shall comply with fire prevention rules and regulations enforced by the TAFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters call 405-734-3981 or 405-734-3982 and 405-734-7964 for after hours and weekends. For routine calls to dispatch call 405-734-7964.

3.2.3. Refuse Collection. The Government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract(s) for the Government provided facilities.

The Government will only allow non-hazardous refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Do not use Government dumpsters to dispose of construction debris.

3.2.4. Workspace. The Government will provide access to the equipment and adequate workspace around the equipment.
