3.3. Government Furnished Services (GFS) are applicable during the performance of this contract.
The Government will furnish the following services at Tinker AFB (TAFB), OK:

1. DESCRIPTION OF DELIVERABLES
The purpose of this Statement of Work (SOW) is to define the work which is to be provided for the repair of the Carleton Life Support Systems Inc. Test Stands.
1.1. Project Management
1.1.1. The Contractor shall host a Post Award Conference within 2 weeks of contract award to provide a management plan with points of contact and schedule of delivery of goods and services. All goods and services to be provided within a maximum period of 12 months.
1.1.2. The Contractor shall provide all required tools, supplies, and labor necessary to complete necessary installations of components in the Concentrator and Monitor test stands.
1.1.3. Installation personnel shall have at least an Associate Degrees in technical specialty and at least 2 years of experience with Carleton’s Oxygen Test Stands.
1.1.4. Contractor personnel shall be United States citizens.
1.1.5. Contractor personnel shall be escorted by production engineer or designee while in maintenance facility.
1.1.6. Contractor personnel shall comply with local and base safety requirements including wearing designated personal protective equipment and clean room behavior.
1.2. MSOGS Concentrator Test Stands - PN 3301270-6101
1.2.1. The Contractor shall if necessary manufacture, supply, and install the components listed in
Table 1.1, in the quantities shown, to return concentrator test stands SN 709002 and
109142 to full operational capability. Test Stand SN 709002 requires a replacement Timer and Oxygen Circuit Card Assembly. Test Stand SN 109142 requires a replacement Wet Meter and Oxygen Circuit Card Assembly. Remaining Oxygen Circuit Card Assemblies will be kept by the government as spares.

1.2.2. The Contractor shall provide updated technical data for the Wet Meter for inclusion into
Technical Order 33D2-10-75-1.

1.3.1. The Contractor shall if necessary manufacture, supply, and install as needed the components listed in Table 1.2, in the quantities shown, to return Monitor test stands SN
1337 and 109419 to full operational capability.

1.4. Other Equipment
1.4.1. The Contractor shall provide one each Leak Chamber Assembly PN 3300271-6101.
1.5. Acceptance Testing
1.5.1. The Contractor shall demonstrate the functionality of the test stands at the completion of the installations. The contractor will not be held responsible for functional/operation test if it is determined that non-operation is due to issues or components other than newly installed component parts from the vendor.
1.5.2. The Contractor shall warranty all work for twelve (12) months after acceptance of final invoice in WAWF. Warranty to be inclusive of all repaired items and spare components.
All warranty complaints will be addressed by the Contractor within two (2) business days.
Contractor shall provide a physical address, telephone and an e-mail address for government notification.
1.5.3. Products found to be defective within thirty (30) days of receipt by the original purchaser will be returned to the manufacturer. The manufacturer will absorb all ground freight charges for the repair. Products found defective within the warranty period, but beyond the initial thirty-day period, should be returned prepaid to the manufacturer for repair. The manufacturer will repair the unit and return it by ground freight pre-paid.
2. OTHER REQUIREMENTS
2.1. Contractor Information
The Contractor shall establish a management system and develop and maintain a Management Plan.
The Contractor shall provide to the Government a POC and a schedule for delivery.
2.2. Security Requirements

If work is required at the Tinker installation, the Contractor shall obtain AFMC Form 496, Application for AFMC Identification Card, from their 76 CMXG\QIM Engineering point of contact (POC).
2.2.1. Contractor Identification Badges and Vehicle Badges
The Contractor shall ensure all Contractor employees obtain identification badges and vehicle passes as follows:
2.2.1.1. Tinker Installation Identification Badges and Vehicle Passes
The Contractor shall comply with local procedures provided at post award conference for obtaining
Tinker installation identification badges and vehicle passes. Contractor personnel shall comply with directives pertaining to operation of privately owned vehicles on Tinker AFB per AFI 31-204, Motor
Vehicle Traffic Supervision.
2.3. Environmental Requirements
2.3.1. Environmental Protection & Safety
The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions or regulation changes during the term of this contract.
Contractors shall be compliant with Tinker Air Force Base (TAFB) Supplement to Air Force Instruction
(AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per Tinker Air Force Base
Instruction (TAFBI) 32-7004, “Hazardous Waste Management”, and Complex procedures per
Oklahoma City Air Logistics Complex (OC-ALC) Supplement to AFI 32-7086, “Hazardous Material
Management” and OC-ALCI 32-101 “Air Quality Record Keeping Procedure”. Publications are available digitally on e-Publishing website at www.e-publishing.af.mil for downloading or ordering.
Spills
Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident. When possible, the Contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the Contractor shall request Tinker AFB 911. The Contractor shall immediately report all environmental violations to the Government POC and UEC. The Contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:
One (1) copy to the CO, two (2) copies to the Government POC, a copy for UEC and one (1) copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:
1. Name and contact number of person reporting the spill
2. Date and time of incident

3. Location and source of spill
4. Substance or pollutant spilled
5. Amount spilled and rate of discharge
6. Any damages or injuries involved
7. Extent of area impacted
8. Potential hazards
9. Actions taken
10. Organizations contacted
11. Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.)
and whether 911 was called and which agencies responded.
2.3.2. Hazardous Material (HAZMAT)
If no HAZMAT will be brought on base or used in performance on this contract, the contractor shall certify “No HAZMAT will be brought on base” or “No HAZMAT Usage” in writing on company letterhead. This certification shall be provided to the Government POC within ten (10) days after award.
Upon Government POC receipt of the certification, the requirements of this section no longer applies.
Appropriate penalties for the contractor, or administrative and disciplinary action for the later shall be assessed if found in violation of above certification.
The Contractor shall not store chemicals or hazardous material (HAZMAT) on Tinker AFB.
Contractors shall be compliant with HAZMAT management procedures per Oklahoma City Air
Logistics Complex (OC-ALC) Supplement to AFI 32-7086, “Hazardous Materials Management”. The
Contractor shall maintain a log of all HAZMAT that is used on base. This usage log shall be provided to the Government POC monthly, the last business day of each month, or when the required work is completed. The Government POC will forward the records to the Unit Environmental Coordinator
(UEC) no later than the fourth (4th) day of the following month or the first (1st) subsequent business day.
Material Safety Data Sheet/Safety Data Sheet (MSDS/SDS)
Upon Government POC receipt of the “No HAZMAT Usage” certification, the requirements of this section no longer applies.
The Contractor shall not transport, ship or use any chemical, material or substance used in the performance of this SOW on Tinker AFB before written approval is provided by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The Contractor shall submit to the
Government POC, ten (10) calendar days after contract award date all Material Safety Data Sheets
(MSDSs) or Safety Data Sheets (SDSs) and a completed OC-ALC Form 152, MSDS/SDS Information
Sheet. The Contractor shall receive approval from the Government POC prior to shipping any chemical, material or substance to Tinker AFB. A copy of the manufacturer MSDS/SDS shall accompany the shipment. Contractor shall comply with the effective dates for SDS requirements IAW the Occupational
Safety and Health Administrations (OSHA) Hazardous Communication Standard, Title 29, Code of
Federal Regulation (CFR) Part 1910.1200. The Government POC will forward all MSDS/SDS submittals to 72 ABW/CEIE for approval. A current manufacturer MSDS/SDS shall be submitted to the
Government POC if the Contractor changes manufacturer or vendor for any chemicals that were prior approved. The Contractor shall maintain copies of manufacturer MSDSs/SDSs for all chemicals and

MSDSs/SDSs shall be readily accessible at all times by Contractor personnel as well as government employees.
Hazardous Waste
If no HAZWASTE will be generated during performance of this contract, the contractor shall certify
“NO Hazardous Waste Generation” in writing on company letterhead. This certification shall be provided to the Government POC within ten (10) days after award. Upon Government POC receipt of the “No Hazardous Waste Generation” certification, the requirements of this section no longer applies.
Appropriate penalties for the contractor, or administrative and disciplinary action for the later shall be assessed if found in violation of above certification.
The Contractor shall dispose of contaminated solid waste, spent or processed chemical solutions, used rags and hazardous waste in government furnished waste containers located in the shop or area the work is being performed. The Contractor shall notify the Government POC if the required waste containers are not available on site.
Discharges
Any discharges into Tinker’s Industrial Waste Treatment Plant (IWTP) or sanitary sewer will be approved and done IAW Tinker Industrial Waste Water Discharge Permit as instructed by 72
ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The Government POC will submit the required notice of intent documentation to the UEC for 72 ABW/CEIE approval. The
Contractor shall maintain records of written approval issued by 72 ABW/CEIE for discharges into IWTP and sanitary sewer.
Off-Site Discharges
For discharges outside of Tinker AFB, the Contractor shall ensure waste water discharges, that may contain pollutants resulting from any contracted service, go to a state or federal permitted treatment facility such as a municipal Publicly Owned Treatment Works (POTW). The Contractor shall provide a letter ten (10) calendar days after contract award date to the Government POC from the POTW that states the POTW is permitted for all pollutants that the Contractor may discharge during the contracted service.
Environmental Training
Upon Government POC receipt of the “No Hazardous Waste Generation” certification, the requirements of this section no longer applies.
Contractor personnel performing duties in the service of this SOW shall have training that acquaints the person with the details of Resource Conservation Recovery Act (RCRA) regulations in 40 CFR 260 through 279; applicable State Laws and regulations; DoD, Air Force and local requirements. This training can be obtained through creditable training institutions or companies. All training shall be approved by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). Prior to obtaining training, the Contractor shall provide the name of the training institution or company and course description to the Government POC for approval within ten (10) calendar days after contract
7 | P a g e B1055 Carleton Test Stand Repair award date. Once written approval is received from the Government POC, the Contractor shall provide the environmental training certificates for employees performing duties in the service of this SOW to the
Government POC, within thirty (30) calendar days after contract award date. In addition to the required environmental training, Contractors shall be compliant with Tinker AFB procedures per TAFBI 32-
7004, “Hazardous Waste Management”.
2.3.3. Shipping
Environmental Certification: The Contractor shall ensure and provide written certification that the part or equipment is cleaned to remove all hazardous contaminants, dirt, shavings, oil, or grease, and drained and purged to remove all fluids, oils and liquids before returning to Tinker AFB. Certification shall be on company letterhead with a statement which specifies the part or equipment is certified to be clean, drained and purged. In addition, as a minimum, said letter shall contain description of part or equipment, model and serial number. This certification shall be provided to the Government POC two
(2) business days prior to the shipment of the part or equipment.
3. Government Furnished Property (GFP) And Services
3.1. Government Furnished Property.
3.1.1. Government Property Repair/Maintenance On-Base is not considered Government Furnished
Property (GFP). FAR Parts 45 and 52.245 plus supplements are not applicable to Government Property
Repair/Maintenance On-Base. The responsible Government Organizations (76 CMXG/ENA) for
Government Property Repair/Maintenance On-Base will be responsible for the Government Property inventory, record keeping, reporting requirements (including Report of Survey), and disposal of all
Government Property requiring repair/maintenance on-base during the performance of this contract.
3.2. RESERVED.
3.3. Government Furnished Services (GFS) are applicable during the performance of this contract. The Government will furnish the following services at Tinker AFB (TAFB), OK:
3.3.1. Security Forces. The Government will provide general on-base security forces’ service. The security forces’ telephone numbers are 911 for emergencies, 405-734-2000 for crimes in progress and
405-734-3737 for non-emergency calls. The off-base 911 center receives 911 cell phone calls. Cell phone callers need to advise the 911 off-base center they are on TAFB in order to be connected with the on-base 911 center.
3.3.2. Fire Emergency and Routine Services. The Government will provide general on-base fire protection services. The Contractor shall comply with fire prevention rules and regulations enforced by the TAFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters call

405-734-3981 or 405-734-3982 and 405-734-7964 for after hours and weekends. For routine calls to dispatch call 405-734-7964.
3.3.3. Utilities. The Government will provide utility services. These utilities include natural gas, electricity, water, and sewer for the sole purpose of accomplishing this contract. If the contractor requires additional utility services, it shall be the contractor’s responsibility to provide. Contractor shall use due care and diligence in efforts to conserve utilities to reduce utility costs.
3.3.4. Refuse Collection. The Government will provide refuse collection services limited to the extent provided in the Base Refuse Collection Contract(s) for the Government provided facilities. The
Government will only allow non-hazardous refuse to be emptied into the nearest authorized refuse dumpster for trash collection. Do not use Government dumpsters to dispose of construction debris.
3.3.5. Workspace. The Government will provide access to the equipment and adequate workspace around the equipment.
