STATEMENT OF WORK

FOR

CYTEC FROZEN MATERIAL

PREPARED BY:

OC-ALC/OBCA

1.1 Scope: The Contractor shall provide all labor, material, quality control and transportation necessary to provide Cytec frozen material to the Oklahoma City- Air Logistics Complex (OC-ALC) Building 9001, Tinker Air Force Base (TAFB) OK 73145.

1.2 Background: The 76 CMXG Composite shop requires Cytec Frozen Material to complete jobs for multiple aircraft at Tinker Air Force Base. The Boeing Material Specification Qualified Parts List (BMS QPL) require Brand Name Cycom material manufactured by Cytec.

1.3 Requested Services: The 76th Commodities Maintenance Group requires a three year contract for the purchase and delivery of Cytec Frozen Material. The yearly requirement is specified below:

1.3.1 Contractor shall provide 24 Rolls of Cycom 919/120 BMS8-169 Rev Type 120 (24 Roll per year requirement will be broken up into 6 rolls shipped every 3 months with the first shipment starting from the manufacturer required lead time).

1.3.2 Contractor shall provide 24 Rolls of Cycom 4102A BMS8-80 Type 7781 (24 Roll per year requirement will be broken up into 6 rolls shipped every 3 months with the first shipment starting from the manufacturer required lead time).

1.4 Parts and Materials: The Contractor shall provide and use only new parts and material in performance of this Statement of Work (SOW) unless otherwise authorized by the Contracting Officer (CO).

1.5 Shipping/Delivery: The Contractor shall provide to the Government POC, the date material was shipped out, shipment identification information and include contract number on the shipping paper work with a Free On Board (FOB) Destination. The ship to address is listed below.

1.5.1 All Shipments must be delivered to Building 9001 Post C-5 Monday thru Friday between the hours of 11:00am - 1:00pm. These times exclude federal holidays.

1.5.2 All shipments shall be packed with Dry Ice to ensure the product is kept at the proper temperature and must include the following monitors:

a. Two (2) Data Logging Monitors per shipment

b. One (1) 3M Monitor Mark 9860A Time Temperature Indicator (or equal) per shipment

c. One (1) WarmMark 25°C/77°F Ascending Time-Temperature Indicator (or Equal) per shipment

d. One (1) 0° to 80°F Irreversible Temperature Indicator Label per shipment

1.5.3 Contractor shall provide a Certificate of Conformance (CofC) to BMS line item for every batch and provide report for every shipment. All shipments must include product shelf life, & date of manufacture information (all products must have 85% shelf-life remaining upon arrival at Tinker AFB).

1.5.4 The shipment of the Cytec Frozen Material will be broken up into a total of four shipments per year with first delivery starting from the manufacturer required lead time.

The remaining three deliveries shall every 90 Days apart.

a. Each delivery will consist on 6 Rolls of Cycom 4102A BMS8-80 Type 7781 and 6 Rolls of Cycom 919/120 BMS8-169 Rev Type 120.
