ATTACHMENT 1.
Each Offeror is required to submit their proposal as specified in RFP provision 52.212-1, Instructions to Offerors-Commercial Items, along with the following addendum. Failure to provide any required information may result in an offer being determined non-responsive.
ADDENDUM - 52.212-1, Instructions to Offerors – Commercial Items
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. In accordance with FAR 15.102(b)(1), (2), and (3) the evaluation factors and significant subfactors that establish the requirements of acceptability shall be set forth in the solicitation. Solicitations shall specify that award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.
Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must submit a proposal that reflects the offerors clear understanding of the requirement. Offerors shall not submit a proposal that simply restates the Performance Work Statement, Evaluation Factors, and the solicitation. Contractors are to provide information that reflects the solicitation and how the contractor intends to provide services in compliance with the terms and conditions of the PWS, Evaluation Factors, and the Solicitation.
Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two (2) separate parts, Part I – Technical Capability Proposal, and Part II – Price Proposal.
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.
C. SPECIFIC INSTRUCTIONS:
1. PART I – TECHNICAL CAPABILITY PROPOSAL – Submit one copy electronically in
Microsoft Word or PDF format WITHOUT company logos. Limited to no more than 30 pages.
a. FACTOR 1: Contract Management Experience:
1. The contractor shall have at least five years of experience managing contracts involving the government with at least two of those five years managing Department of Defense (DOD) military medical services contracts.
2. Proposal must show that the contractor understands the government’s requirement and the contractor shall have at least five years of experience managing issues that arise throughout the life of a typical services contract.
b. FACTOR 2: Personnel Management Experience:
1. The contractor shall have at least five years of experience managing personnel.
2. Proposal must show that the contractor is capable of recruiting, hiring, and replacing qualified personnel – as delineated in the labor categories for each position in accordance with the Performance Work Statement (PWS) paragraphs listed below:
a. Administrative Assistant Level I (PWS paragraph 2.1.1)
b. Administrative Assistant Level II (PWS Paragraph 2.1.2)
c. Administrative Assistant Level III (PWS Paragraph 2.1.3)
d. Program Manager Level III(Paragraph 2.1.4)
e. Analyst Level III (PWS Paragraph 2.1.5)
c. Proposal must show that the contractor is capable of effectively managing their employees and addressing issues involving their personnel and their work as they come up during the life of the contract.
2. PART II – PRICING PROPOSAL – Submit one (1) copy electronically in Microsoft Excel format. No page limitation.
a. Complete blocks 17a, 17b, 30a, 30b, and 30c of the Request for Proposal. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP. These sections constitute the model contract.
b. Insert the proposed prices for all full time equivalents (FTE) on the attached Pricing Sheet (attachment 4). The pricing sheet will be used to evaluate all offeror’s pricing.
c. Complete the necessary fill-ins and certifications for FAR provision 52.212-3, Representations and Certification- Commercial Items and return in its entirety.
3. PROPOSAL INFORMATION
a. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
b. Requirements of the RFP, CLINs, PWS, and government standards and regulations pertaining to the PWS.
c. Evaluation Factors for Award in Addendum to Provision 52.212-2 – Evaluation-Commercial Items of this RFP.
d. Technical Capability Proposals shall be submitted in 12 pitch font and shall not exceed thirty
(30) pages double spaced. Pages exceeding the page limitations set forth in this provision will not be read or evaluated and will be removed from the proposal.
e. Format for proposal:
f. Tables and figures may be single spaced.
g. Except for reproduced sections of RFP document, text shall be no smaller than 12 point.
h. Margins on all four sides will be at least one inch.
i. Special consideration will not be given for colors, pictures or unnecessary graphics. NO
COMPANY LOGOS.
j. Cross Referencing:
k. Each part shall be written on a stand-alone basis so that its contents may be evaluated without cross-referencing to other parts of the proposal.
l. Information required for proposal evaluation, which is not found in its designated part, will be assumed to have been omitted from the proposal.
m. Proposal Due Dates and Number/Type of Copies Required.

4. Method of Technical Capability Proposal Submission – Offerors must submit Part I – Technical
Capability Proposal and Part II – Price Proposal as follows:

Note: It is the responsibility of each offeror to ensure that their proposals reach one of the designated email addresses above on or before the request for proposal due date/time established in this RFP. 773 ESS/PKB is not responsible for lost proposals, proposals delivered to locations other than the locations outlined in paragraph 1a, or email delivery problems associated with servers.
5. QUESTIONS AND ANSWERS
a. All questions must be submitted via email to maurice.ross.1.ctr@us.af.mil and lynette.parson.2@us.af.mil not later than Monday, 31 October 2011 at 12:00 PM CST.
Answers will be posted to www.fbo.gov no later than Wednesday, 2 November 2011.
Questions will not be accepted, nor will a response be provided after Monday, 31 October 2011.
