Air Force District of Washington Contracting Directorate (AFDW/PKA) Line Operations Safety Audit (LOSA) Request for Information (RFI)

The Air Force District of Washington (AFDW) Contracting Activity (PK) on behalf of the AFDW/Safety Directorate (AFDW/SE) is issuing this request for information as means of notifying the public of an upcoming requirement to support the requirement for LINE OPERATION SAFETY AUDIT (LOSA). AFDW/SE requires contractor support for threat and error identification during flight crew performance and flight operations. As outlined in the Federal Aviation Administration Advisory Circular Number 120-90 dated 27 April 2006, this process, commercially referred to as a Line Operations Safety Audit (LOSA), and provides a rigorous and methodical assessment of the safety strengths and weaknesses in normal flight operations of US Air Force UH-IN Iroquois aircraft and aircrews. A LOSA is a formal process that requires highly trained experienced observers to ride the jump seat during regularly scheduled flights to collect safety-related data on environmental conditions, operational complexity, and flight crew performance. A LOSA provides a diagnostic snapshot of strengths and weaknesses that AFDW/SE can use to bolster the “health” of its safety margins and prevent degradation. Confidential data collection and non-jeopardy assurance for crewmembers are fundamental to the LOSA process.

The purpose of this request for information is to determine the availability and technical capability of small businesses (including the following subsets, Small Disadvantaged Businesses, HUBZone Small Businesses, Certified 8(a) Small Businesses, Service-Disabled Veteran-Owned Small Businesses and Women Owned Small Businesses) to provide the required services.

DISCLAIMER: This RFI is for informational purposes only is NOT a request for proposal. It does not constitute a solicitation and shall not be construed as a commitment by the Government. Responses in any form are not offers and the Government is under no obligation to award a contract as a result of this announcement. The government will not be responsible for any costs incurred by interested parties in responding to this notice. Any information submitted by respondents is strictly voluntary. Contact with other than AFDW/SE Government personnel by potential offerors or their employees regarding this project is prohibited.

NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM (NAICS) CODE: The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 541690, Other Scientific and Technical Consulting Serives, with the corresponding size standard of $15,000,000.00. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for quotes; submission of any information in response to this synopsis is purely voluntary; and the government assumes no financial responsibility for any costs incurred.

Interested parties are requested to submit their answers addressing all questions including any supporting documentation supporting claims of organizational and staff capability. Companies’ response statement shall include:

1. Company name, mailing address, email address, Web site address (if available), and the name, telephone number, and email address of a point of contact having the authority and knowledge to clarify responses with Government representatives.

2. Name, title, telephone number, and email addresses of individuals who can verify the demonstrated capabilities identified in the response.

3. DUNS Number, CAGE Code, Tax Identification Number, company structure (Corporation, LLC, partnership, joint venture, etc.), Socio-Economic 8(a) Program status, and Business size. Companies must be registered in the System of Award Management (SAM) at www.sam.gov to be considered as potential sources.

In response to this RFI, please provide the following:

1. What past performance qualifications do you have performing Line Operational Safety Audits (LOSA), to include rotary-wing operations?

1. What experience do you have with regards to Threat and Error Management (TEM) as it applies to military flight operations?

1. What experience do you have with training military aircrew personnel to operate as an unbiased observer to document threats, errors, undesired aircraft states, crew resource management, crew coordination and aircrew comments, and effectively communicate their observations in a non-punitive “just culture” manner?

1. What experience do you have with DOD agencies, specifically Air Force agencies, relating to Safety support and Rotary Wing Operations?

1. What experience do you have with developing customized LOSA data collection software? Does the software enable remote access, processing and query functions of collected data, and ensure unlimited access and ownership of the data by the customer?

1. What experience do you have with creating TEM matrices?

1. What experience do you have with developing short and long term software capable of archiving previous aircrew and aircraft analysis for comparison with future LOSA observations and subsequent trending?

1. What experience do you have with development of web-based software and infrastructure that allows observer reports to be received by secure and confidential electronic means directly from observer locations?

1. When have you previously provided a team of analysts to review and analyze raw LOSA data in order to document strengths and weaknesses of that specific weapons system?

1. In addition to LOSA capabilities, do you offer other proactive flight safety services? Please describe these services.
