(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) The solicitation number is FA5613-15-R-0020, and the solicitation is issued as a request for proposal (RFP).

(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82.

(iv) Since performance of this requirement will take place overseas (Germany), no set-aside applies for the associated North American Industry Classification System (NAICS) code

621111.

(v) The 700 CONS/LGCA Services Flight has a requirement to purchase a Personal Services

Contract for a Medical Standards Management Element Technician Support (MSME)

Services at the United States Air Force (USAF) 86 th

Medical Group’s Separation History and Physical Examinations (SHPE) Program, Ramstein Air Base, Germany. This requirement has a period of performance of 01 September 2015 through 31 August 2016, with the yearly option to extend the performance through 31 August 2020. Offerors shall use the attached Attachment 1, Price Schedule which includes all Contract Line Item

Numbers (CLINs), quantities and unit of issue. The offeror shall fill in all unit prices and total amount blocks for the base period and all option periods.

(vi) Attachment 2, Performance Work Statement (PWS) contains a comprehensive description for the MSME at the USAF SHPE, Ramstein Air Base, Germany.

(vii) The USAF SHPE, Ramstein Air Base, Germany will constitute as the place of performance and acceptance. All offerors must propose Free On Board (FOB) destination.

Required period of performance: Base Period: 15 Sep 2015 – 14 Sep 2016 st Option Period: 15 Sep 2016 – 14 Sep 2017 nd

Option Period: 15 Sep 2017 – 14 Sep 2018 rd

Option Period: 15 Sep 2018 – 14 Sep 2019 th

Option Period: 15 Sep 2019 – 14 Sep 2020

(viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Apr

2014), applies to this acquisition. Addenda to this provision are included in Attachment

3, Applicable Clauses and Provisions.

(ix) The provision FAR 52.212-2, Evaluation – Commercial Items (Jan 1999), applies to this acquisition. Specific evaluation criteria contained in paragraph (a) of that provision are included in Attachment 3, Applicable Clauses and Provisions.

(x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror

Representations and Certifications -- Commercial Items (May 2014), Alternate I (May

2014), with their offer, or complete paragraph (b) of 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov.

(xi) The provision FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May

2014), applies to this acquisition. Addenda to this provision are included in Attachment

3, Applicable Clauses and Provisions

(xii) The provision FAR 52.212-5, Contract Terms and Conditions Required to Implement

(xiii) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition.

In accordance with FAR 52.212-1(f)(2)(i)(B), proposals will not be considered late if evidence establishes that the proposal was received at the Government installation designated and it was under the Government’s control prior to the time set for receipt of offers for delivered hardcopy proposals.
