Computed Radiography Equipment
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in
Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
Solicitation # FA5606-17-Q-2023 will be issued as an RFQ.
The solicitation document and incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 2005-95.

Include all technical specifications and applicable product information with your quote.
Salient Characheristics include:
o For CLIN 0001: Carestream Industrex HPX-1 Plus Digital System or Equal, must accept both rigid cassettes and flexible plates, simultaneous multiple plate scanning capability, Ethernet connection, filtered air flow, 3MP (color) or 5MP (monochrome), 100-240 VAC, must weigh less than 135lbs, and Spacial Resolution must be 50 microns or better.
o For CLIN 0002: Carestream Industrex HPX-PRO Portable Digital System or Equal, weighs no more than 45lbs, filtered air flow, must be able to process plates up to 6” wide, and up to 90” in length, 100-240 VAC, minumum 3.5 hour standby/40 minute active scanning battery life, Dimensions shall not exceed 18”W X 18”H X 18” L.
o For CLINS 0003 through 0014: Offered products shall meet industry standards.

The below ADDENDA is added to FAR 52.212-1, Instructions to Offerors:
This a BRAND NAME or EQUAL requirement.
Quoted amounts shall be net prices without tax.
Quote shall be valid until 30 September 2017.
The Government reserves the right to award on an “All or None” basis.

All questions are to be submitted in writing by 17 August 2017, 1600 hrs., Central European
Summer Time (CEST).
Quotes are due NO LATER THAN 4 September 2017 1600 hrs., CEST.
Quotes and all other correspondence as listed in this solicitation must be emailed to both points of contact (POCs) the contracting officer Casey Hamann at casey.hamann@us.af.mil, and the contract specialist TSgt Aaron Farris at aaron.farris@us.af.mil.
The provision at FAR 52.212-2, Evaluation of Offerors – Commercial Items, applies to this acquisition in the following manner:
(a) The Government reserves the right to award a contract resulting from this solicitation to the responsible contractor whose quote conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. The following factors shall be used to evaluate quotes:
1. Price.
2. Technical Acceptability according to the above item descriptions and salient characteristics, has to meet or exceed all item specifications as listed therein and all offered items must be
Carestream Brand Name or Equal products.

3. Contractor Responsibility Determination:
A contractor is deemed responsible if IT has no negative entries in the following systems:
a. System for Award Management (SAM).
b. Excluded Parties List System (EPLS).
The award will be made to the lowest priced, technically acceptable quote which is deemed responsible.
The Government will evaluate quotes as follows:
The offeror's quote will be considered technically acceptable if it meets the requirements detailed in the above item descriptions and the listed Salient Characteristics. Quotations that do not meet these requirements will be considered unacceptable and will not be considered for award.
(b) A written notice of award will be mailed or otherwise furnished to the successful contractor after evaluation is complete and a successful quote has been identified. The successful contractor must confirm by signing the SF 1449 in blocks 30a, 30b, and 30c and e-mail the document back to the contract specialist TSgt Aaron Farris at aaron.farris@us.af.mil.
The provision at FAR 52.212-3, Offeror Representations and Certifications – Commercial
Items, requires contractors complete the applicable portions and submit with their quote.

(c) Changes. Changes in the terms and conditions of this contract may only be by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.

52.211-6 -- Brand Name or Equal (Aug 1999)
(a) If an item in this solicitation is identified as “brand name or equal,” the purchase description reflects the characteristics and level of quality that will satisfy the Government’s needs. The salient physical, functional, or performance characteristics that “equal” products must meet are specified in the solicitation.
(b) To be considered for award, offers of “equal” products, including “equal” products of the brand name manufacturer, must—
(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;
(2) Clearly identify the item by—
(i) Brand name, if any; and
(ii) Make or model number;
(3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer;
and
(4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification.
(c) The Contracting Officer will evaluate “equal” products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The
Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.
(d) Unless the offeror clearly indicates in its offer that the product being offered is an “equal” product, the offeror shall provide the brand name product referenced in the solicitation.

EFFORT. NO AWARD WILL BE MADE UNDER THIS SOLICITATION UNTIL FUNDS ARE
AVAILABLE. THE GOVERNMENT RESERVES THE RIGHT TO CANCEL THE SOLICITATION,
EITHER BEFORE OR AFTER THE CLOSING DATE. IN THE EVENT THE GOVERNMENT
CANCELS THE SOLICITATION, THE GOVERNMENT HAS NO OBLIGATION TO REIMBURSE
AN OFFEROR FOR ANY COSTS. PLEASE ENSURE QUOTES WILL BE VALID UNTIL 30
SEPTEMBER 2017.
