PERFORMANCE-BASED WORK STATEMENT

COMMUNITY READINESS CONSULTANT

RAF CROUGHTON, UK

18 June 2019

1. Description of Non-personal Services

1.1. The Contractor shall provide a Community Readiness Consultant personnel in support of the

Family Services and Sustainment Flight (422 ABS/FSA) at RAF Croughton, United Kingdom.

All work shall be performed in accordance with federal, local rules and regulation, and this

Performance Work Statement (PWS).

1.2. The main duty of the Community Readiness Consultant is to be a liaison between the 422

ABS/FSA and the Airmen & Family Readiness Center (100 FSS/FSSR) at RAF Mildenhall, UK and RAF Croughton community. The contractor shall communicate and promote all A&FRC programs and core services to the RAF Croughton community. These services include but is not limited to: Loan Closet, Hearts Apart Events, Sponsorship and Sponsorship Level II, Newcomers, Information and Referral, Transition Assistance, Smooth Moves, Pre-deployment and

Reintegration briefings, and other programs considered A&FRC core services. These services are also outlined in Chapter 2 of AFI 36-3009 Airman and Family Readiness Centers.

1.3. The Community Readiness Consultant, along with the 100 FSS/FSSR and 422 ABS/FSA, will assist the 422nd Air Base Group (ABG) Commander’s programs for the health and welfare and mission readiness of RAF Croughton’s military community and their eligible family members.

2. Contractor Qualifications

2.1. Excellent customer service and verbal and written communication skills are essential. The contractor must be capable of communicating effectively in the English language. The contractor personnel must have supervisory skills with a general knowledge of all aspects of A&FRC services. Additional skills in administrative procedures, customer service, and resource and referral process performed in a Community Readiness Consultant or specialist capacity is preferred.

2.2. The contractor must be familiar on how to use a personal computer and have basic knowledge on various Microsoft programs such as MS Word, Excel, Publisher, PowerPoint and

Adobe Acrobat. The contractor must also be familiar in navigating Web sites, social media sites, and perform searches on the Internet.

2.3. The contractor must not be a current employee of the United States Government due to potential conflict of interest.

3. Specific Duties

3.1. The Community Readiness Consultant shall administer and coordinate A&FRC programs in support of the 422nd ABG Commander’s program as outlined in Chapter 1 of AFI 36-3009.

3.2. The contractor shall deliver professional and courteous customer service. All services provided by the Community Readiness Consultant is covered under the Privacy Act and shall be considered confidential, except for some instances which may be excluded by regulation.

3.3. The contractor shall be responsible for the dissemination of information such as local services and resources offered by the A&FRC to the RAF Croughton community. The contractor shall also be responsible for coordinating and facilitating classes, seminars, and workshops as outlined in section 1.2 of this document, and coordinating /facilitating these with 100 FSS/FSSR when applicable.

3.4. The contractor shall be an Ad Hoc member and attend Integrated Delivery System (IDS) meetings. The contractor is expected to offer solutions based from the Community Readiness perspective during the meetings. The contractor shall maintain a record of all services provided to the community under the Community Readiness Program.

3.5. The contractor in coordination with the Relocation Core Compliance Expert staff will maintain the Military Home Front’s Plan My Move database and keep it up to date.

3.6. The contractor shall assist customers in completing the necessary paperwork for the Loan

Closet program. The contractor shall ensure that loaned items are tracked, in a serviceable condition, and returned on-time. The contractor shall check that all loaned items are clean prior to returning to the inventory. The Loan Closet program items must be maintained at adequate quantities based on annual usage patterns.

3.7. In coordination with the A&FRC, conduct pre-deployment and reintegration briefings. The contractor shall facilitate events for the Hearts Apart program for eligible families of deployed members.

3.8. The Community Readiness Consultant shall attend speaking events such as Commander’s calls, Spouses’ groups, etc. and post on the A&FRC’s social media page to promote programs similar to those found in paragraph 1.2.

3.9. The contractor shall work in coordination with the local Command Support Staff with regards to new incoming personnel to RAF Croughton. The contractor shall send invites and host briefings in support of the Newcomers Program. The contractor shall review and maintain the schedule of the said program.

3.10. The contractor shall provide training in support of Sponsorship and Sponsorship Level II to eligible personnel. The contractor shall responsible for maintaining the schedule for this program.

3.11. The contractor shall collect and gather information from books, magazines, and other related materials in the subject of Community Readiness. The information and materials collected will be maintained in the reference library for 422 ABS/FSA volunteers and customers.

3.12. The contractor shall coordinate and route all budget requirements related to Community

Readiness through the Sustainment and Family Service Flight Chief.

3.13. The Community Readiness Consultant shall maintain a neat, clean, and professional image at all times during work performance.

3.14. The contractor shall not accept or collect any moneys from customers in the performance of any work in this PWS.

4. Responsibilities

4.1. Records Management. The contractor shall be responsible for maintaining records in support of the 422 A&FRC in accordance with AFI 33-322 Records Management Program. The training required for this program will be provided by the government.

4.2. Emergency Family Assistance Center. During times of crises or emergencies, the contractor will be notified and shall perform a support role to the 422 ABS/FSA EFAC program.

The contractor shall support the EFAC during regular work hours as specified in paragraph 5.

The contractor shall work at the reception and call center at the designed EFAC location.

5. Work Schedule

5.1. The contractor shall perform work from Monday through Friday at 8:30 AM to 4:30 PM.

The Contractor shall not work more than 8 hours in any 24 hour period. Any work performed outside of the regular work schedule shall be recorded and the regular work schedule adjusted.

Also, the 422 ABS/FSA observes federal holidays, USAFE Family and Goal Days.

6. Government Furnished Items

6.1. The Government shall provide all the necessary facility, equipment, materials, and/or services to the performance of work outlined in this PWS. All Government provided items shall be used only in the performance of work. The government will provide ample office space with a desk within the A&FRC facility. Also, the contractor shall have access to a personal computer, landline telephone, printer, and digital sender and copier.

7. Access to Base Network

7.1. Contract personnel requiring access to the Air Force network shall complete and submit a

DD form 2875 System Authorization Access Request (SAAR). The contractor personnel must also complete the online Annual Information Awareness Training and be entered into the Trusted

Associate Sponsorship System or TASS. The contractor personnel must pass all necessary checks and training to obtain access to the base network.
