1. DESCRIPTION OF SERVICES
The contractor shall provide heavy equipment identified in the specific requirements and also provide logistical support for that equipment during the entire duration of this project. The project is to install a satellite earth terminal located on Fort Buckner, Okinawa, Japan. Heavy equipment should include cranes, an all-terrain forklift and either man lifts or bucket trucks. The
U.S. Government will operate all rental equipment with the exception of the crane to accomplish this project. Crane support shall include a crane operator. This project is expected to last for approximately 106 days from start to finish. Projected start date is no later than 10 days after award.
Dimensions of satellite earth terminal:
1.1. Pedestal Assembly
a. Height: 4.86 meter
b. Diameter (Top): 1.77 meter
c. Diameter (Bottom): 3.2 meter
d. Weight: 9,071.9kg
1.2. Reflector Assembly
a. Height: 2.57 meter
b. Diameter: 12.25 meter
c. Weight: 21,000kg
2. SERVICES SUMMARY
This project has a total duration of approximately 106 days with limited flexibility to adjust dates depending on work accomplished ahead of schedule and possible delays due to weather or other unforeseen circumstances.
2.1. Provide with a licensed operator a: 100 Ton Crane with articulating boom 43 meters in length with 16 meter auxiliary cable / rooster sheave. This crane will be required on day 2 - 15 of the project schedule for approximately total 14 days. The crane is also required a spreader bar for the first days of off load – 4 meters wide, with a load capacity of 18,000 kg.
a. The crane is required to be capable of lifting an object that weighs 9,071.9kg, 6.1 meters high with a working radius of 13.7 meters.
b. The crane is also required to be capable of lifting an object that weighs 10,227.3kg, 9.1 meters high with a working radius of 13.7 meters.
2.2. Provide with a licensed operator a: 200 Ton Crane with boom capable of reaching 33.6 meters. This crane will be required on day 43 - 45 of project schedule for approximately total 4 days.
a. The crane is required to be capable of lifting an object that weighs 21,000kg, 12.19 meters high with a working radius of 15.24 Meters.
2.3. Provide a 19-meters man lift with a maximum load of 200kg. The man-lift will be required on days 2-64 of the project.
2.4. Provide one all-terrain 4 Ton forklift that has 4-wheel drive and leveling system. The forklift also needs to have a telescopic boom cable of 17-meter lift height with a reach of 15-meters. The all-terrain 4 Ton forklift is also required an extension fork (length 120mm).
a. These equipment will be required beginning on day 2 of project schedule and required for
88 continuous days until day 90.
2.5. Provide dumpster, minimum 8 cubic meter capacity from day 2 throughout 90 days completion of the project. Provide service contract emptying of dumpster as needed by U.S.
Government installers. The contractor is also responsible for any necessary sorting of garbage to comply with Japanese standards for Recycling.
2.6. Contractor shall provide fuel for the following heavy equipment during the duration of the project. This will require that the contractor has a means of delivering the fuel to the worksite.
Fuel is reimbursable for actual consumed amount only and will not include tax. Contractor will complete the USFJ Form 327EJ for fuel tax exemption consumed by the Government for all equipment identified in each LINE item. USFJ Form 327EJ shall be submitted to 18 CONS
Squadron for Contracting Officer’s approval.
a. 19 meter man lift: Diesel Fuel
b. Retractable boom forklift: Diesel Fuel
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES
The U.S. Government will provide all government installed satellite earth terminal equipment.
U.S. Government will provide all manpower to operate man lifts and forklift with the exception of the crane operator.
4. GENERAL INFORMATION
4.1. Security and Physical Requirement
Fort Buckner is a designated restricted facility and requires screening process for all contractor personnel and vehicles. A listing of contractor personnel with photo ID, such as Driver’s
License, address on Company letterhead will be required prior to access approval. A listing of contractor vehicles, make, model and license plate is also required prior to the start of work.
4.2. Hours of Operation
Monday through Saturday, 0800 – 1700 include Federal Holidays and Japanese Holiday in the projected work schedule.
First Friday of July Independence Day
First Monday of September Labor Day
4.3. Operation during Typhoon
When Tropical Cyclone Condition Readiness 2(TCCOR 2) is declared, all work will cease immediately. The contractor will clean up and dispose of all loose debris and waste as directed by COR to secure the job site against damage that may result from anticipated weather conditions. Contractor responsibility shall remain the same during tropical cyclone declaration
“Tropical Cyclone Condition of Readiness 1 Caution (TCCOR1C)”. Upon the declaration of
TCCOR1C, the contractor employees shall be excused from all duties. Upon declaration
“Tropical Cyclone Condition of All Clear (TCCOR ALL Clear)”, the contractor will return to duty.
4.4. Safety Requirments
In performing work under this contract, the contractor shall:
4.4.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.
4.4.2. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel.
4.4.3. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.
4.4.4. Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.
4.4.5. Record and report promptly (within one hour) to the contracting officer or designate government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.
4.4.6. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
4.4.7. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.
4.4.8. Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.
