Requirement Description

This is a Request for Quote prepared in accordance with Subpart 13.5, as supplemented with additional information included in this notice. This is not to be construed as a commitment by the

Government. All information is to be submitted at no cost to the Government.

The solicitation will be issued under the solicitation number FA5215-19-Q-8046.

This solicitation document and incorporated provisions and clauses are those in effect through the

Federal Acquisition Circular (FAC) 2019-05, Defense Federal Acquisition Regulation

Supplement Publication Notice (DPN) 20190628, and Air Force Acquisition Circular (AFAC)

2018-0525.

Set Aside

This acquisition is open to all qualified parties. The applicable NAICS is 339113, Surgical

Appliance and Supplies Manfacturing; Size Standard is 750 Emp.

Submission Date

All quotes are due on or before 2:00 PM, Hawaii Standard Time (HST), Thursday, 05 September

2019. Submission shall be made via email to the follow:

ADDENDUM TO CLAUSE 52.212-1 Instructions to Offerors – Commercial Items

Paragraphs (E) and (H) are hereby deleted from the referenced provisions.

Paragraph (C) period for acceptance of quotes. The offeror agrees to hold the prices firm for

90 Calendar Days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the solicitation.

ADDENDUM TO CLAUSE 52.212-2

a) This requirement is being solicited in accordance with FAR Subpart 13.5, Simplified

Acquisition Procedures. Award shall be made to a single contractor. Award will be made to the responsible, and responsive offeror that is found to be fair and reasonable and meets the requirements of the provided Requirement Description (Attachment 6). Quotes must include prices for each item listed, in order so that quotes may be properly evaluated. Failure to do so may be cause for the rejection of the entire quote. The Government reserves the right to evaluate, select and fund all, some, or none of the line items submitted under this solicitation. The following factor shall be used to evaluate offers: Price and Technical Compliance.

(c) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the

Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

Basis of Award

The Government intends to award a contract resulting from this solicitation to the responsible offeror whose proposal(s) represent the best value (e.g. warranty considerations or delivery terms) after evaluation in accordance with the aforementioned factors in this solicitation.

Special Delivery Instructions for OCONUS Shipments

Preferred method of shipping is Commercial Door-to-Door. Please see the specific delivery locations in Attachment 2.
