PERFORMANCE WORK STATEMENT (PWS)

FOR

COURIER SERVICES

15 MEDICAL GROUP

JOINT BASE PEARL HARBOR-HICKAM, HI

13 July 2012

1. Scope: The Contractor shall provide all labor, equipment, and transportation necessary to pick-up and deliver medical records between the Hickam Clinic and Tripler Army Medical Center (TAMC), Hawaii.

2. Specific Tasks:

2.1 Morning Pick-Up and Delivery: The Contractor shall pick-up medical records from the Hickam Primary Care Clinic at Building 559, C-Wing, First Floor Reception Desk, between 0845 - 0930 hrs, Mon. – Fri., except for Federal Holidays, and any previously announced times when the Medical Clinic will be closed. The Contractor shall log-in his/her name, date & time when the records are being picked-up. The records shall be delivered to Tripler Army Medical Center’s Fourth Floor, Mountainside, Medical Records Section, no later than 1015 hrs the same day.

2.2 Afternoon Pick-Up and Delivery: The Contractor shall pick-up medical records from the Tripler Army Medical Center’s Fourth Floor, Mountainside, medical Records Section, between 1000

– 1115 hrs, Mon. – Fri., except for Federal Holidays, and any previously announced times when the Medical Clinic will be closed. The records shall be delivered to the Hickam Primary Care Clinic no later than 1230 hrs the same day. The Contractor shall log-in his/her name, date & time when the records are delivered.

2.3 Tuesday Pick-Up and Delivery: The Contractor shall pick-up medical records from Hickam Primary Care Clinic at Building 559, C-Wing, First Floor Reception Desk and deliver to the Hawaii Air National Guard on Hickam, 360 Mamala Bay Dr, Bldg 3417, en-route to Tripler Army Medical Center.

3. Workload: It is estimated that, on average, two (2) bags, weighing approximately fifty (50) pounds each are currently picked-up and delivered daily. This is only an estimate. The Contractor is responsible for all weights and measures.

4. Training: The Contractor shall assign personnel who are trained on the importance of the Privacy Act and for securing and safeguarding medical records from release to anyone other than the designated pick-up and delivery contacts at the Hickam Clinic and Tripler AMC. The Contractor shall not open the secured mailbags and should they be found unsecured, the Contractor shall ensure that the bags are locked and secured before departing. The Contractor shall provide written instructions to the Government describing the pick-up and delivery process that the Contractor will follow in fulfilling the specified tasks.
