STATEMENT OF WORK
FOR
REPLACEMENT OF BATTERIES, COOLING FANS, AND
ALARM RELAYS FOR UNINTERRUPTIBLE POWER
SUPPLIES (UPS) OF RAPCON AND DASR
AT
YOKOTA AIR BASE JAPAN

1. DESCRIPTION OF SERVICES.
1.1. GENERAL. The contractor shall provide all necessary personnel, supervision, management, tools, equipment, transportation, materials, and any other items or services necessary to perform replacement of the batteries, cooling fans, and alarm relays for the UPS at the RAPCON, building (Bldg) No. 1371, and the DASR, Bldg 1345, in Yokota AB. The alarm relays apply only to the UPS in Bldg 1371. The contractor shall perform replacement services in accordance with this SOW, manufacturer standards, commercial practice and local standards.
1.1.1. INSTALLATION PLAN. The Contractor shall arrange an installation plan and schedule with the
Site POC, and obtain an approval from the Site POC prior to the commencement of the work.
1.1.2. ARRANGEMENTS OF THE PERFORMANCE TIME. Should the contractor decide to perform any of the required services before or after normal duty hours, the contractor will request approval prior to performance from the Site POC and shall do so at no additional cost to the Government.
1.1.3. HOURS OF SERVICE. The contractor’s personnel will perform scheduled work on weekdays, Monday through Friday (exclusive of the U.S. legal and other designated holidays), during normal duty hours, 0800 - 1630 hours unless prior arrangements have been made with the Site POC.
1.1.4. SITE POC. The authorized primary Site POC for this contract is SSgt Joshua Pogioli, 374
OSS/OSAM, DSN: 225-2272. The alternate is SrA Dennis Decker, DSN: 225-2272. The contractor shall coordinate all aspects of this project with the Site POC. Completed work shall be subject to inspection and acceptance by the Site POC.
1.1.5. QUALIFICATION. The contractor is required to provide employees who have at least 1-years of experience in work similar to this requirement. The contractor shall be trained and certified for this replacement work.
1.1.6. COMMERCIAL WARRANTY. The contractor shall provide the Government with their commercial warranty. The Government shall receive the same warranty service as the contractor’s most favored customers. If the services fail to be performed or otherwise fail due to workmanship or materials, the contractor shall correct the affected area(s) at no additional cost to the U.S. Government. The
Government reserves the right to reject any unserviceable or substandard item and to require replacement with a serviceable like item or acceptable quality item or to require full credit for the item. The period of warranty shall be no less than three (3) months from the date of the acceptance by the Site POC.
1.2. BASIC SERVICES. The contractor shall perform replacement of the batteries, cooling fans, and alarm fuse relays of the UPS at Bldg No.1371 and 1345 at Yokota AB. The major equipment is listed in
Appendix A.
1.2.1. UPS OPERATION PROCEDURES. The contractor shall comply with manufacturer instructions and/or available technical data. Replacement work shall include the following:
1.2.1.1. Configure the system to bypass mode so as to bypass power to the batteries and also supply power to the facility.
1.2.1.2. Remove the currently installed batteries, cooling fans, and alarm relays from the UPS.
1.2.1.3. Contractor shall purchase new batteries, new cooling fans and new alarm fuse relays for this replacement and install the new batteries, cooling fans, and alarm fuse relays in the UPS.
1.2.1.4. Return the system to normal configuration.
1.2.1.5. Run test for operation of the batteries, cooling fans, and alarm relays.
1.3. DISPOSAL OF DISCARDED BATTERIES. The batteries removed from the UPS at each location shall be properly disposed of at Bldg 955, HAZMAT Pharmacy, by the contractor in accordance with (IAW) the Japan Environmental Governing Standard (JEGS). The contractor will call 225-7739 one day in advance for disposal. The contractor shall cover each terminal with insulating tape prior to disposal. No documentation is needed to dispose of batteries at HAZMAT Pharmacy.
1.3.1. DISPOSAL OF MATERIALS OTHER THAN DISCARDED BATTERIES. All debris and unserviceable materials generated under this contract such as old cooling fans and alarm fuse relays removed from the UPS, industrial waste, non-recyclable materials and equipment shall be disposed of off-base by the contractor IAW the Japanese regulation, Article 3 of “Law of Disposal and Clearing of Waste
(Haikibutsu no shori oyobi seiwou ni kansuru horitsu)” and local regulations.
1.3.2. REMOVED PARTS AND MATERIALS. The contractor shall obtain approval from the Site
POC prior to final disposition of any replacement item removed from equipment or facilities.
1.4. DEGRADATION OF SYSTEM CAPABILITY. Except in case of imminent personnel hazards or equipment damage, the contractor shall not degrade system capability without the specific approval of the
Site POC specified in Appendix A. Degradation of system capability is defined as any changes that would adversely affect the system’s capacity, performance, or ability to provide full system capability.
This includes a major functional limitation in system capability caused by any maintenance action. At least fifteen minutes prior to the degradation start time, the contractor shall notify the Inspector/Acceptor to include, but limited to, the following:
a. Purpose of the degradation
b. Equipment involved
c. Period of degradation
d. Estimated recovery time
e. Other information that is required by Inspector/Acceptor
1.5. REPORTS. The contractor shall submit the following reports to the Site POC:
1.5.1. FINAL REPORT. The contractor shall submit the final report in English in a form of Appendix B to the Site POC for approval upon completion of all replacememnt work.
1.5.2. DEFICIENCY REPORT. When any deficiencies are identified with the UPS under this contract, the contractor shall inform the Site POC immediately. The contractor shall submit the deficiency report in both English and Japanese to the Site POC within 1 day of the findings. The deficiency report shall include at a minimum of the following information:
(1) UPS location
(2) Finding date of failure
(3) Description of failure (picture, drawings, etc.)
(4) Justification (what will happen if repair is not accomplished)
(5) Proposed description of repair work (including drawings, pictures, etc.)
(6) Material list (description of article, quantity, catalogue-cut, pictures, etc.) for repair
(7) Man-hour list (type of worker, required man-hour of each worker, etc)
(9) Proposed repair schedule (repairs under this basic contract)
(10) Cost estimate for repair work (in case of additional repair/replacement work)
(11) Submission date of the report
(12) Signature of the Contractor Representative
1.5.3. OTHER REPORTS. Upon request, the contractor shall provide the Site POC with specific data.
The data may pertain to replacement actions, resources expended in specific replacement actions, or administrative information.
1.6. SAFETY REQUIREMENT. The contractor shall identify, control, and eliminate potential safety hazards, and shall initiate an immediate action to correct any deficiencies. The contractor shall inform the
Site POC immediately of any accidents involving injury to contractor personnel or damage to the
Government equipment or property that are a direct result of executing this replacement work.
1.7. GENERAL INFORMATION.
1.7.1. CONTRACT MANAGER. The contractor shall provide a contract manager who shall be responsible and have English capability for the work. The name of this person and an alternate(s) who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer and/or the Site POC. The contract manager or alternate shall have full authority to act for the contractor on all matters relating to this contract.
1.7.2. CONTRACTOR EMPLOYEES. Contractor personnel shall be easily recognizable as contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the contractor’s company name and the employee’s name.
1.7.3. REMOVAL OF CONTRACTOR’S EMPLOYEES. The contractor shall utilize only experienced, responsible and capable employees in the performance of this contract. The contractor may be required to remove any of his/her employees who endanger persons or property, or whose continued employment under this contract is inconsistent with the interests of the U.S. Government.
1.7.4 SECURITY REQUIREMENTS. The contractor shall not make available or disclose technical or administrative information relative to or concerning the sites, systems, publications, records, reports and any other data, to persons who are not certified by the Site POC.
1.7.5. REPORTS OF DAMAGES, DESTRUCTION, INJURY, OR DEATH
(a) The contractor shall report all incidents of damage or destruction to property, or personal injury or death to any person, arising out of and during the performance of this contract. The term “property” includes equipment, freight, or property of any type, whether Government owned or not. The term
“Contractor” includes the contractor, its agents, servants or employees.
(b) The contractor shall submit these reports to the KO through the Site POC as follows:
(1) A preliminary report shall be made as soon as practicable after the time the incident occurs.
Report may be either verbal or written.
(2) A complete written report shall be submitted within 4 days (96 hours) after occurrence of the incident. A copy of this report shall be furnished to the Site POC.
1.7.6. RESPONSIBILITY FOR DAMAGES. Any damages caused to U.S. Government property during the course of performing this contract shall be restored, replaced and/or repaired to its original condition at no additional cost to the U.S. Government.
1.7.7. CLEANLINESS. The contractor is responsible for cleanliness of the service areas. This includes, but is not limited to, washing, sweeping, dusting and other janitorial services necessary to maintain the area in a clean and sanitary condition. The contractor shall furnish such supplies as required.
1.7.8. CONSERVATION OF UTILITIES. The contractor shall instruct employees in utility conservation practices. The contractor shall be responsible for operating under conditions, which preclude the waste of utilities. The contractor shall comply with additional energy policies upon notification by the KO.
