SECTION 1

DESCRIPTION OF SERVICES

GENERAL SCOPE: The Contractor shall provide all personnel, labor, equipment, materials, supervision, and tools necessary to perform drayage of government-owned furnishings (to include recreation (foosball, ping pong tables, etc.) and exercise equipment) to/from warehouses, consolidated dormitories (CD) on Eielson AFB, quarters on Eielson AFB, and quarters off-base. The contractor shall comply with commercial and industry standards, and all applicable federal, state, and local laws, regulations, and procedures to include OSHA Guidance 29 CFR part 1910 General Industry. At contract start, the contractor shall complete any work left in progress by the previous contractor.

1. FURNISHINGS SET-UP: The contractor shall assemble/disassemble & setup all government-owned furnishings, as well as setup and connect all appliances. Contractor shall maintain the correct type and amount of hand tools needed to assemble /disassemble furnishings.

1.1. MOVEMENT OF FURNISHINGS: When furnishings are uploaded from the warehouse to the truck, Furnishings Management Section (FMS) personnel shall place palletized material directly behind the truck for loading. When items are not palletized, contractor shall be required to pick up individual items at their designated location within the warehouse. When furnishings are off-loaded at the warehouse, the contractor shall place furnishings at the location designated by FMS. When FMS requires a truck to be off-loaded onto pallets, FMS shall place pallets directly behind truck. The contractor shall be required to move furnishings from truck to the designated location. Items will be inspected by FMS or Consolidated Dormitory Manager (CDM) personnel as they are off-loaded from truck. The contractor liability for damage ends when contractor finishes the placement of furnishings at the designated location and items are verified by FMS or CDM personnel. When loading and off-loading more than one shipment at warehouse, the contractor shall unpack, and separate shipments to provide for an accurate inventory of each shipment. After initial preparation of Bill of Lading (BoL), any changes to condition(s) shall be initialed by the contractor and the customer, FMS or CDM personnel (when applicable). When required, the contractor shall be responsible for removing and reinstalling doors that affect movement of government furnishings in and out of dorms, quarters on Eielson AFB, or residence off-base. Prior to large dormitory moves, the contractor, CDM and/or FMS representatives shall conduct a joint and concurrent inspection of government facility/facilities in addition to property inspection as required. Existing damage to facility/facilities shall be documented as an added item on the BoL. Damaged items (other than normal wear and tear), shall be treated as exceptions and documented as added items on the BoL. An attachment shall be used if this is required. The government may occasionally perform movement of minimum sized furnishing kits or individual CDM furniture pieces for residents when needed. Residents are authorized to pick-up and deliver furnishings as needed from government warehouse.

1.1.1. REPLACEMENT OF ITEMS: Any furnishings broken or damaged by the contractor during delivery shall be returned to the FMS warehouse and exchanged for serviceable items which shall be delivered the same day on original order at no additional cost to government or occupant.

1.1.2. MISSED APPOINTMENTS: The contractor is required to wait fifteen minutes after initial arrival at delivery/pick-up location. If resident cannot be contacted, the contractor shall attempt to contact occupant by means of telephone, ringing front door bell, and knocking on front and back door. If occupant cannot be located, the contractor shall contact FMS personnel or CDM for guidance. FMS or CDM personnel will determine if this is a missed appointment. In the event residents cannot be located after all attempts, the contractor shall leave a NO SHOW notice and annotate arrival and departure time on BoL. The move shall be considered a missed appointment. The contractor shall be paid eighty percent (80%) of BoL total for moves considered missed appointments. If the contractor has been properly notified by FMS or CDM personnel, of the date and time of delivery or turn-in and misses the appointment, the contractor will have twenty percent (20%) deducted from the BoL when move is rescheduled and accomplished.

1.1.3. VEHICLES: Each contractor vehicle shall have the name of the contractor neatly exhibited on each side of the cab. The vehicle shall be in satisfactory mechanical condition and shall present a neat and clean appearance. Vehicles shall be supplied with sufficient clean padding, covers, and other protective equipment necessary to ensure safe transit, shipment, and delivery of items in an undamaged condition.

1.1.4. TRAFFIC MANAGEMENT OFFICE (TMO) CARRIERS: FMS or CDM personnel will not schedule a drayage move for the contractor if a TMO carrier will be performing work at occupied quarters. The contractor shall not enter an occupant’s quarters if, upon arrival, a TMO carrier is working on the premises. If the contractor does not perform for this reason, the move shall be considered a missed appointment in accordance with (IAW) paragraph “Missed Appointments”.

1.1.5. PROPERTY INVENTORY AND INSPECTION: The contractor shall provide FMS or CDM personnel (when applicable) with pre-printed BoL invoices, in quadruplicate, to schedule items for delivery/pickup. FMS or CDM personnel (when applicable) will inventory and inspect items prior to loading and immediately after Contractor personnel unload shipment. Contractor personnel shall inventory and inspect items prior to loading. The contractor shall jointly and concurrently inventory and inspect property with occupants for each delivery/pickup and both will sign BoL. Inventory and inspection condition(s) other than normal wear and tear will be recorded. If damage is noted (e.g., item is deemed unserviceable), movement of item is stopped and the contractor shall contact FMS or CDM personnel (when applicable) immediately. FMS or CDM personnel (when applicable) will respond immediately to inspect the property. The following condition codes shall be used and shall be recorded on pre-printed BoL:

The contractor shall provide condition codes with a definition to customer on BoL or by attachment of BoL. The contractor must obtain signatures from customer and FMS or CDM personnel, (when applicable) upon completion of delivery/pickup. The contractor shall retain copy one of BoL, provide copy two to FMS or CDM personnel (when applicable), and copy three to customer. BoL shall be used to determine responsibility for damage.

1.1.6. DELIVERY & SETUP: The contractor shall deliver, assemble/disassemble and setup all furnishings, washers/dryers & appliances. The contractor shall jointly and concurrently inventory and inspect property with occupants for each delivery/pickup and both will sign BoL.

1.1.7. CONTRACTOR WASHER/DRYER HOOK-UP: The contractor shall deliver washers and dryers to on base, as well as off base housing. The contractor shall be responsible for supplying a certified installer to insure both water and electrical connections are correctly installed and the unit is setup correctly. This includes installing appropriate power cord to the dryer or stove, as necessary. Contractor shall assume all liability for damages that result from their negligence when hooking up connections to washers and dryers whether home is located on base or off base.

1.1.8. CONTRACTOR CLEANUP: The contractor shall remove, in or near residences and warehouse, any packing materials used with each move. Upon completion of the job, the contractor shall leave the work site and premises in a clean, neat condition.

1.1.9. NOTIFICATION OF SERVICES: Notification of pickup/delivery orders written against this contract will be issued by FMS or CDM personnel. FMS or CDM personnel (as applicable), will notify the contractor prior to 4:30 P.M., one work day prior to date services are required. FMS or CDM personnel will designate whether drayage moves will be in the morning or afternoon. Morning moves shall be accomplished between 7:30 A.M. and 11:30A.M., except Saturdays, Sundays, and Federal Holidays. Afternoon moves shall be accomplished between 12:30 P.M. and 4:30 P.M. except Saturdays, Sundays, and Federal Holidays.

1.1.10. WEATHER CONDITIONS: The contractor shall perform work in variety of weather conditions. The contractor is not required to perform work if weather temperature conditions are reported to be 40 degrees below zero, or colder (no wind chill factor will be used in this temperature). The contractor shall use the National Weather Service (NWS) Forecast Office, Fairbanks, Alaska Office for weather forecasts and advisories.

1.1.11. DAMAGE LIABILITY: The contractor shall be responsible for all government furnishings while in his/her possession. The contractor shall be liable for any damages to government or privately owned quarters and for personal items damaged during performance of the contract. The contractor or customer shall immediately notify government personnel when damages are incurred. The contractor will be given the opportunity to inspect all reported damage claims.

1.1.12. REIMBURSEMENT: The contractor shall reimburse the government the invoice value for loss or damage to government owned furnishings and/or government/privately owned quarters while performing services under this contract unless, in the opinion of the designated government representative, said damages can be satisfactorily repaired by the contractor to meet government standards. Any applicable reimbursement to the government will be processed through reduction of the invoice monthly by the contractor and agreed to by FMS or CDM personnel.

2.1. SECURITY REQUIREMENTS:

2.1.1. The contractor shall be responsible for obtaining all necessary clearances, cards, passes, decals, and other items required for access of contractor personnel and vehicles to the base or other areas where service will be performed. The contractor shall be required to apply for vehicle and personnel identification at the Pass and Registration Office, 354 SFS, Eielson AFB, AK. Upon completion or termination of this contract, the contractor shall ensure all vehicle and personnel identification are surrendered to office of issue.

2.1.2. Contractor employees shall wear distinctive clothing bearing the name of the company. Employees shall wear a picture badge that contains the company name, and employee’s name. Picture identification shall be worn and displayed at all times.

2.1.3. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population. Contractor personnel shall be courteous and professional, present a clean, neat appearance and by easily recognized.

2.1.4. The contractor shall maintain a current listing of all employees. The list will include employee names and date of birth. This list shall be validated by the contract manager and provided to the Contracting Officer and to 354 SFS at Eielson AFB, AK at the contract start date. An updated listing shall be provided upon change of employees.

2.1.5. The Contractor is responsible for protecting all resident Personally Identifiable Information (PII) provided as necessary to complete the furniture move. Examples of PII include name, address, telephone numbers (home and or mobile), etc.

2.1.6. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. During times of crisis or national emergency, the contractor shall continue to provide services as scheduled, with minimal disruption of services.

2.2. SAFETY & VOLUNTARY PROTECTION PROGRAM:

2.2.1. The contractor shall perform work in a safe manner as required by OSHA Safety and Health requirements.

2.2.2. The contractor shall provide a verbal report to the Contracting Officer (CO) as soon as possible after each occurrence of damage to government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact.

Safety, Security, Identification The contractor shall make all the necessary requirements with SFS to ensure contract personnel have the proper security clearance and passes to perform work on installation. Personnel shall wear identifying clothing bearing the company name for identification and present a neat & clean professional appearance and attitude. Contractor shall not employ persons on this contract that present a security, safety, or well-being risk to installation personnel. Contractor vehicle shall present the company name neatly and clearly on the outside of the vehicle.

APPENDIX 1

Abbreviations, Acronyms and Definitions

AFI (Air Force Instruction) - a published instruction for members of the United States Air Force intended for use by active duty, guard, and reserve members and associated civilians.

BoL (Bill of Lading) - a document issued by a carrier to a shipper, acknowledging that specified goods have been received on board as cargo for conveyance to a named place for delivery to the consignee who is usually identified.

CO (Contracting Officer) - a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the CO acting within the limits of their authority as delegated by the CO.

Contractor - Refers to both the prime contractor and subcontractor. The prime Contractor shall ensure that his/her subcontractors comply with the previsions of the contract.

COR (Contracting Officer’s Representative) - an individual, including a contracting officer’s technical representative (COTR), designated and authorized in writing by the contracting officer to perform specific technical or administrative functions.

CDM (Consolidated Dormitory Management) - a system where permanent party enlisted dormitories are managed by a consolidated or centralized office.

CH (Community Housing) - non-government owned housing, typically located off base.

DoD (Department of Defense) - an executive branch department of the federal government of the United States charged with coordinating and supervising all agencies and functions of the government concerned directly with national security and the United States Armed Forces.

FH (Family Housing) - a building, whether located on or off base, used as residential dwelling.

FY (Fiscal Year) - federal fiscal year (1 October-30 September).

FMS (Furnishings Management Services).

Government Quarters - family and unaccompanied housing units that the DoD owns, leases, obtains by permit, or otherwise acquires.

PII (Personally Identifiable Information) - any representation of information that permits the identity of an individual to whom the information applies to be reasonably inferred by either direct or indirect means. Further, PII is defined as information: (i) that directly identifies an individual (e.g., name, address, social security number or other identifying number or code, telephone number, email address, etc.)

3.1. Family Housing (FH) moves shall be delivered by prescribed kits:

3.1.1. FH Moves: “Kits” are the kit composition as described below. FMS personnel will notify the contractor before 4:30 P.M., one work day prior to date services are required. At time of notification, FMS personnel will designate whether drayage moves will be in the morning or afternoon. Morning moves shall be accomplished between 7:30 A.M. and 11:30A.M., except Saturdays, Sundays, and Federal Holidays. Afternoon moves shall be accomplished between 12:30 P.M. and 4:30 P.M. except Saturdays, Sundays, and Federal Holidays. Upon receiving notification, the contractor shall perform drayage of items at prices set forth in bid schedule.

Unaccompanied Housing (Dormitories) Typical UH Furnishing Items

4.1. Unaccompanied housing moves shall be done by item.

4.1.1. BY ITEM MOVES: “Per Item” is defined as any items that are moved individually from rooms or common areas to other rooms or common areas. CDM personnel will notify the contractor before 4:30 P.M., one work day prior to date services are required. At time of notification, CDM personnel will designate the time the moves will be accomplished. Morning moves shall be accomplished between 7:30 A.M. and 11:30A.M., except Saturdays, Sundays, and Federal Holidays. Afternoon moves shall be accomplished between 12:30 P.M. and 4:30 P.M. except Saturdays, Sundays, and Federal Holidays. Upon receiving notification, the contractor shall perform drayage of items at prices set forth in bid schedule.
