STATEMENT OF WORK
DEMOLISH BIRCH LAKE BATH HOUSE, BUILDING 4040
BRAQ 08-2201
08 FEBUARY 2010
1. PURPOSE: The purpose of this Statement of Work (SOW) is to describe the requirements to demolish building 4040-Bath House on Birch Lake Recreational Ground. This project also includes site restoration of the area.
2. SCOPE
2.1. Provide services for the demolition of building 4040 (Birch Lake Bath House).
2.2. The contractor shall provide all necessary personnel, facilities, equipment, transportation, and materials to complete the tasks identified in this SOW.
3. DESCRIPTION OF WORK
3.1. The contractor shall provide all labor, materials, transportation, and equipment to accomplish the following work.
3.1.1. Contractor shall provide a detailed work plan for the demolition of the facility and utilities to include, but not limited to:
3.1.1.1. Outline of intended demolition and disposal procedures;
3.1.1.2. Plan to protect existing buildings and utilities;
3.1.1.3. Site grading and restoration plan;
3.1.1.4. A description of temporary utilities and outages required. An estimation of these time lines will be included with in the work proposed time line.
3.1.1.5. Estimated time line of completion.
3.1.2. Contractor shall demolish building 4040 in its entirety. The demolition area is shown in attachment 1 included in this SOW.
3.1.3. Contractor shall demolish existing slab on grade foundation.
3.1.4. Contractor shall demolish all existing utilities excluding diesel tank and diesel lines attached to building 4040.
3.1.5. Contractor shall demolish existing overhead electric lines. Electric lines have already been cut by 354 CES.
3.1.6. Contractor shall demolish all existing concrete building foundations, sidewalks, footers, and pads to include below grade footings.
3.1.7. Contractor to provide a description of temporary utilities and outages required.
3.1.8. Create a uniform grade after demolition completion. Fill material shall be provided by the Government at a location on base at no cost to the Contractor.
3.1.9. Plant grass in graded area according to the following specifications. The contractor may choose to plant grass by hand according to paragraph 3.1.10.10 or to hydroseed according to paragraph 3.1.10.13.
3.1.9.1. Work includes spreading topsoil and furnishing all seeding as required.
3.1.9.2. Fertilizer shall meet Federal Specification O-F-241C(1).
3.1.9.3. No planting shall be done in freezing weather and grass cover shall be planted prior to 15 August.
3.1.9.4. Substitutions will only be considered if written proof of unavailability is provided.
3.1.9.5. Seeding Material
3.1.9.5.1. Lawn seed shall meet the following standard.

Rate of application: 4 pounds per 1000 SF.
3.1.9.6. Nugget fertilizer shall be delivered in manufacturer's unopened containers with label stating guaranteed analysis. Fertilizer shall be dry, free of lumps and free flowing with a guaranteed analysis of 20-20-10S. Fertilizer shall be spread at a rate of 3/4 lb. per 100 SF.
3.1.9.7. Mulch for seeding shall be screened topsoil, free of large woody material.
3.1.9.8. Seeded areas shall have a minimum 6” of topsoil.
3.1.9.9. Spread topsoil over area to be seeded evenly, smooth to an even surface and fertilize as required above. Remove rocks and other deleterious material, roll to a smooth surface.
3.1.9.10. Broadcast seed by hand. Rake seed into bed and roll with a 200 pound roller.
3.1.9.11. Cover planted seed with 1/4" to 1/2" screened topsoil for top mulch and roll lightly.
3.1.9.12. Irrigate as required to assume uniform germination, water shall not be excessive or cause erosion. Reseed any areas that fail to produce a uniform growth.
3.1.9.13. Install hydroseed according to manufacturer’s specifications with sufficient mulch as required to support plant growth. Seed mixture shall be identical to that described in paragraph 3.1.2.8.5. applied at a rate of 4 pounds per 1,000 square feet. Provide water and perform general maintenance for a minimum of 30 days. If grass does not show substantial growth at that time, reseeding shall be done in those areas at contractor expense.
3.1.9.14. Maintain all seeded areas for at least 60 days from planting, at which time all material shall show an established healthy condition. Areas that show a germination rate lower than specified or a growth rate less than other seeded areas shall be replaced. The Contracting Officer will make a preliminary inspection after the initial 60 day maintenance period listing any defects to be corrected as a condition of preliminary acceptance and listing a date of such acceptance.
3.1.9.15. All work and material shall be guaranteed for the period of one year from the date of preliminary acceptance.
3.1.9.16. Remove all dead plant material. If damaged material cannot be satisfactorily repaired, remove and replace. Seeded areas which have not shown vigorous growth shall also be replaced with no additional cost to the Government.
3.1.10. The Contractor is responsible for designating at least one individual to apply for and receive a Security Line Badge for access to Quarry Hill. Contractor shall remove and dispose of the following demolition debris to Quarry Hill Landfill on Eielson AFB with a completed AF Form 103.
3.1.10.1. Clean Concrete (no lead based painted surfaces)
3.1.10.2. Excavated unusable soil or gravel
3.1.10.3. Asphalt
3.1.11. All remaining construction debris not meeting the above mentioned conditions shall be disposed of at an approved off base site.
4. GENERAL REQUIREMENTS
4.1. Attend Pre-performance meeting (timing to be coordinated with the Contracting Officer):
4.2. A digging permit is required and must be executed by the Contractor. This must be coordinated with the Civil Engineer Squadron customer service at 377-2100.
4.3. Work is not in a restricted area. Contractor shall perform work in a manner to minimize interruption to the government and facility occupants. Scheduling the work to be done will be coordinated with the base POC. In addition, contractor shall maintain area in a neat and orderly manner. Contractor shall provide 48 hour notice of scheduled demolition date with Contracting Officer to allow for coordination of base personnel.
4.4. The progress schedule shall be in accordance with the successful offeror’s proposed schedule with approval by the Government. Winter Exclusion will not be used for this project.
4.5. The performance period shall be in accordance with the successful offeror’s proposed schedule with approval by the Government. The demolition shall begin during the summer of 2010.
4.6. The Government shall provide a storage/staging area. Contractor shall be responsible for any storage trailers and/or temporary fencing needed and for restoring storage areas to pre-construction state.
4.7. Contractor shall provide portable toilet facility for work crew.
4.8. Permits: Before work is started, obtain all necessary permits required by Federal, State, and local jurisdictions for project completion to include, but not limited to, storm water construction permit and sedimentation and erosion control.
4.9. Character of Work and Mechanics: The work shall be executed in the best and most workmanlike manner possible by qualified, careful, and efficient mechanics skilled in the applicable trade, and in strict accordance with the contract documents and standards of the industry.
4.10. Location: Birch Lake Recreation Area is located approximately 43 miles south of Fairbanks, AK on the Richardson Highway.
4.11. All work shall be coordinated with the Civil Engineering construction inspector and Base Contracting through the Contracting Officer.
4.12. Government Furnished Information and Materials:
4.12.1. Project location plan;
4.12.2. Utility maps of the project site (electrical, water, sewer, and gas);
4.12.3. As-builts and photos.
4.13. Utility Outage: When a utility outage is necessary to perform the contract work, the outage shall be coordinated in advance with the Civil Engineering construction inspector. The contractor shall give written notice 14 days in advance of scheduled outage.
4.14. Salvage: It is recommended that the contractor pursue selling salvaged materials; however, the contractor is prohibited from selling any salvaged materials on government property.
4.15. Any damage to existing building or utilities shall be the responsibility of the contractor to repair at no additional cost to the Government.
5. ENVIRONMENTAL REQUIREMENTS
5.1. Compliance with Laws: The contractor shall comply, and ensure that all Subcontractors comply, with all applicable federal, state, and local laws, regulations, ordinances, and standards related to environmental matters. The contractor shall also comply, and ensure that all subcontractors comply, with all specific instructions or directions given to the contractor by the base regarding environmental matters.
5.2. Hazardous and Special Wastes Generated by the Contractor:
5.2.1. The contractor shall identify, characterize, containerize, store, and dispose of hazardous wastes in strict accordance with federal guidelines found in the Code of Federal Regulations, Title 40 (40 CFR) parts 260-270, FWI (Fighter Wing Instruction) 32-7005 Hazardous Materials Management, all local guidelines, and as specified.
5.2.2. The Contractor is responsible for properly storing, marking, labeling, securing, and transporting hazardous wastes. All hazardous wastes shall be collected in contractor furnished DOT/UN approved containers and taken to Eielson's Hazardous Waste Facility for disposal. Call the Hazardous Waste Facility at 377-1668 prior to transporting wastes to the facility to coordinate delivery of the waste materials. The Contractor shall not store hazardous waste on base for more than 30 days.
5.2.3. A Uniform Hazardous Waste Manifest shall be used by the contractor to document all parties and locations involved in the transportation, storage, and disposal of all hazardous and special wastes. This form shall be provided to the government by the contractor and signed by the Base Environmental Coordinator before the waste is transported from the limits of government property. A copy of the manifest shall be signed by the receiver of the waste and submitted to the Contracting Officer not later than forty-five days after disposal has taken place.
5.2.4. The contractor shall notify the Contracting Officer upon encountering any material not identified in the contract documents thought to be hazardous that could jeopardize the safety of workers or personnel in the area. Any discovery of potentially contaminated soils or water (odors, free product, sheen, staining, field screening readings > 20 ppm), not previously documented by the base, shall be reported immediately to the 354 CES/CEAN at 377-SPIL. Eielson AFB Reporting Procedures Guidance shall be followed. 354 CES/CEAN shall make all required agency spill reporting notifications. Suspect soil must be tested to determine if it contains any contaminants prior to relocating it. Testing and disposal of soil shall follow Alaska Department of Conservation 18 AAC 75 (Oil and Other Hazardous Substances Pollution Control). If test results determine “other than clean”, the material shall have to be transported to an appropriate ADEC approved disposal facility based on the contaminants identified.
5.3. Spill Reporting/Clean-up:
5.3.1. Contractor spills of hazardous materials, petroleum products, glycols, antifreeze, grease, latex paint, hydraulic fluid, etc. shall be reported immediately upon occurrence to the 354 CES/CEAN at 377-SPIL. This number is manned 24-hours a day. If for some reason, the Contractor is unable to contact the above number, the Base Fire Department should be contacted at 377-4156. Eielson AFB Reporting Procedures Guidance shall be followed. 354 CES/CEAN shall make all required agency spill reporting notifications. Spill reporting includes spills to any surface, including concrete, inside buildings that are not constructed as secondary containment, and to surface water bodies or sewers. The Contractor shall take appropriate actions to correct the spill’s cause and prevent future occurrences.
5.3.2. For Contractor caused spills, the Contractor shall be responsible for all costs and labor required for sampling and analysis, response, containment, clean up, and disposal. The site of the spill shall be documented using latitude and longitude in the Eielson AFB approved digital GIS format. Contractor shall obtain the Approval to Transport from ADEC prior to disposal at an ADEC approved disposal facility. A copy of the approval to transport shall be submitted to the Eielson project manager prior to transport off-base. No material shall be removed from the site or backfill begun without approval from the 354 CES/CEAN. Copies of the weight and/or acceptance tickets shall be submitted to the Eielson project manager within 5 business days of delivery or receipt by contractor. Copies of the certificate of thermal incineration or disposal and confirmatory samples shall be submitted to the Eielson project manager within 5 business days of delivery or receipt by contractor.
5.3.3. The operator shall have sufficient spill response supplies readily available on vehicles and/or at the site to contain any spillage.
5.3.4. The contractor shall display (e.g. job site trailer, bulletin boards, etc.) the Eielson Spill Response placard developed for construction projects (Attachment C)
5.3.5. The name and qualifications of the individual who shall be responsible for implementing and supervising the containment and cleanup will be provided.
5.3.6. Location of temporary fueling facilities, associated spill control measures and secondary containment provisions shall be required for any container or group of containers co-located holding 55 gallons or more of petroleum products.
5.4. Hazardous Materials: The contractor shall provide the Base Environmental Coordinator a list of all hazardous materials that the contractor will bring onto government property and shall further provide a Material Safety Data Sheet (MSDS) for each Hazardous Chemical listed in OSHA Hazard Communication Standard 29 CFR 1910.1200. In addition, the contractor shall observe proper storage practices for hazardous materials stored on base.
5.5. Underground Fuel Tanks: The contractor must adhere to both statutory requirements and generally accepted industry standards for the decommissioning of the tanks. The contractor shall be familiar with and strictly follow guidelines in 18 AAC 78 Underground Storage Tank (UST) regulation to include (but not limited to):
5.5.1. File an intent to close and a post-closure report with the state of Alaska for the tank removals;
5.5.2. Provide a State of Alaska UST certified worker to supervise and sign the applicable paperwork;
5.5.3. Complete a Site Assessment and Release Investigation Summary Form.
5.6. Personnel: The Contractor shall provide an experienced and qualified consultant. The consultant’s qualifications shall be submitted to the Contracting Officer for approval. Acceptability shall be determined on the basis of education, training, experience, and past performance. The Consultant shall be experienced in all phases of the required work. The Consultant shall perform all field screening of suspect contaminated soils. The Consultant shall review the SAP, review test results, and provide recommendations for the Contractor’s testing program. The Consultant shall have a minimum of 2-years experience in the development and preparation of SAPs. The Consultant shall have a minimum 4-year college degree in Chemistry, Environmental Science, Engineering, Geology, Hydrology, or a related field; experience in and knowledge of EPA methods for collecting environmental and hazardous waste samples; and experience in operation of field screening equipment and be ADEC certified as “Qualified Personnel” per 18 AAC 78.995.
5.7. Excavation:
5.7.1. If any potentially contaminated soils or water (odors, free product, sheen, staining, etc) is discovered, the contractor shall then conduct field screen testing every 10 cubic yards to determine the approximate boundaries of any soil contamination within the footprint of the project.
5.7.2. If excavation/construction shall occur at an IRP site, the Contractor shall closely coordinate all excavation/construction activities with 354 CES/CEAN to ensure site access and construction activities are in full compliance with the terms of the Eielson AFB Land Use Control Management Plan and Record of Decision and document for the site.
5.7.3. Dewatering operations at an IRP site are explicitly prohibited without coordination with 354 CES/CEAN and written consent from the EPA Region 10, and ADEC. No exceptions.
5.8. Backfill: If potentially contaminated soil is discovered during the project, the contractor shall obtain approval for backfilling from the Contracting Officer in writing prior to placement of any backfill. Excavated materials conforming to specification requirements for materials to be used for fills, backfilling, grading or top soiling, may be utilized provided there are no indications of contamination as defined herein.
5.9. Field Report:
5.9.1. The contractor shall prepare and submit a draft and final Field Report to the Contracting Officer prior to acceptance of the facility. The contractor shall make an assessment of the site based on the field work and analysis required by assessment of the site based on the field work and analysis required by this contract. As a minimum, each Field Report shall include the following:
5.9.1.1. The owner’s name and address;
5.9.1.2. The operator’s name and address, if different from the owner;
5.9.1.3. The name and business address of each person who supervised the site assessment or site inspection;
5.9.1.4. A scaled site sketch that approximately shows:
5.9.1.4.1. The location and configuration of any tanks and piping;
5.9.1.4.2. The locations and depths of field screening tests;
5.9.1.4.3. The proximity to buildings and residences;
5.9.1.4.4. Any fresh or historic release sites;
5.9.1.4.5. Any free product sites;
5.9.1.4.6. The facility and property boundaries;
5.9.1.4.7. A bar scale and north arrow;
5.9.1.4.8. Any other pertinent information.
5.9.1.5. A photographic history and description of the contract work to include pre- and post-construction photographs. Each print shall show the following information in typewritten format.

5.9.1.7.2. Name of person performing;

5.9.1.7.5. Summary of test results.
5.9.1.8. General Site Inspection: The inspection of the site(s) shall be documented in the Field Report. The inspection shall include:
5.9.1.8.1. Checking for obvious leaks and spills;
5.9.1.8.2. Checking for any obvious soil contamination caused by a release or leakage;
5.9.1.8.3. Recording local climate conditions during inspection.
5.9.1.9. If sampling for laboratory analysis was performed, copies of lab reports shall be included. Laboratory Data Reports shall meet the requirements of Alaska Department of Conservation Technical Memorandum 06-002.
5.9.1.10. Field Notes: The contractor shall maintain field notes in a bound book. Field notes shall be written in ink. Erasures shall not be allowed. The contractor shall document all field activities and any visibly contaminated soil. The Contractor shall include a copy of the field notes as part of the draft Field Report. The original field notes shall be submitted as part of the final Field Report.
5.9.1.11. The contractor shall submit the draft field report to the Contracting Officer prior to facility acceptance. The Contracting Officer shall provide comments on the draft report to the Contractor within 30 calendar days after the draft report has been submitted. The Contractor shall include all Government comments in the final report and state concurrence or non-concurrence with the comments. The Contractor shall submit the final report to the Contracting Officer within 14 calendar days of receipt of comments and prior to facility acceptance.
5.10. Nuisance and Polluting Activity: Polluting, dumping, or discharging of any harmful, nuisance, or regulated materials (such as concrete truck washout, vehicle maintenance fluids, residue from saw cutting operations, Solid Waste and Hazardous Substances) into building drains, site drains, streams, waterways, holding ponds or to the ground surface shall not be permitted and the contractor shall be held responsible for any and all damages which may result. The contractor shall conduct its activities in such a fashion which avoids creating any legal nuisance including, but not limited to, suppression of noise and dust, control of erosion, and implementation of other measures as necessary to minimize off-site impacts of work activities.
5.11. The contractor will not allow fugitive dust to migrate off the jobsite as described in 18 AAC 50.
6. SITE MAINTENANCE AND CLEANUP
6.1. Site Maintenance: The contractor shall protect adjacent property, buildings, and their contents from dust, dirt, or other materials. Contractor is to wet down dry materials and settle debris enough to prevent dust or material from blowing into surrounding parking areas and buildings. Work areas shall be maintained in a neat and safe condition.
6.2. Cleanup: The contractor shall collect all trash, debris, refuse, garbage, etc., which he generates and place it in appropriate containers with lids or approved covers on a daily basis. Disposal shall be outside the limits of government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all local, state, and federal guidelines, criteria, and regulations.
7. STORAGE AND PARKING
7.1. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its offers and agents, free and harmless from liability on any nature occasioned by the Contractor’s performance.
7.2. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor at its expense upon completion of the work. With the written consent of the Contracting Officer, buildings and utilities may be abandoned and need not be removed.
7.3. The Contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer or the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.
7.4. All fuel or petroleum products shall be stored in doubled-walled tanks or within a containment area with an impermeable liner. The storage area shall be marked with appropriate signage (i.e. “Flammable Storage - No Smoking within 50 ft.”). All fuel tanks and petroleum storage containers shall be grounded and bonded in accordance with NFPA. The containment area shall be sized at 110% of the largest storage container with a minimum of one foot of freeboard for earthen berms. Any evidence of fuel or oil spills shall be immediately cleaned up by the contractor at no additional expense to the government. Bermed areas shall be removed at the completion of the job and the area restored to its original condition. All contamination shall be removed from the site and disposed of in conformance with all federal and state regulations. Costs of all soil tests as a result of spills shall be a responsibility of the contractor.
7.5. All other storage areas shall be kept free of debris, leaks, stains, or splashes and kept in a neat, clean, and safe condition. Any hazardous substance which may cause contamination of the storage area shall be stored in a contractor supplied trailer which shall be secured when not in use.
8. SAFETY
8.1. The contractor shall comply with all applicable Air Force Occupational Health and Safety Standards and Regulations. Compliance with the American General Contractors’ Safety Manual and Occupational Safety and Health Act shall also be a requirement of the contractor of this project.
8.2. The contractor shall not have or allow glass food or beverage containers on the construction site. Glass lined thermal containers are permitted when contained in an appropriate metal or plastic outer shield.
9. POST AWARD SUBMITTALS
9.1. Demolition Plan to include, but not limited to, an outline of intended demolition and disposal procedures, a plan to protect existing building and utilities, site grading description, and description/design of additional site work to be completed after grading. Submit to Contracting Officer 2 days prior to planned demolition.
9.2. Safety Plan detailing how contractor will comply with provisions of Section 8. Submit to Contracting Officer 2 days prior to start of onsite activity.
9.3. Uniform Hazardous Waste manifests (As required).
10. AS-BUILT DRAWINGS: Contractor will have access to all as-builts.

1. PRINCIPAL FEATURES: The work covered by these specifications and accompanying drawings consists of furnishing all plant, labor, materials, tools, and equipment necessary in performing all operations in connection with Project FTQW 07-2303P2, DEMOLISH SELF SERVICE STATION FUEL TANKS AND UNDERGROUND UTILITIES,
2. SCOPE: In general, the principal features of the work are outlined but not limited to this paragraph. Unless otherwise indicated, it is not the intent of this listing to include technical data for materials or the installation of these materials.
3. FIELD MEASUREMENTS: The Contractor shall verify all measurements at the site and shall be responsible for dimensions and proper installation of all materials.
4. AVAILABILITY OF UTILITIES: These fuel tanks and utilities are on this base and base utilities are present.
5. CONTINUED USE OF FACILITIES: Not used
6. SEQUENCE OF WORK: Work under this contract shall be sequenced and performed in accordance with the schedule provided by the contractor.
7. DISPOSAL OF MATERIALS: Except for excess soil, all materials removed under this contract shall be disposed of off base in such a manner that meets federal, state and local environmental codes.
8. CONTRACTOR SUBMITTALS: Not used.
9. PERMITS
9.1. Digging Permits: The contractor will be responsible for coordinating a government supplied Base Civil Engineer Work Clearance Request (AF Form 103) prior to performing digging of any type. The contractor shall process the digging permit by coordinating with and obtaining signatures from responsible representatives of the organizations listed on the AF Form 103 prior to obtaining final approval from the Air Force Base Civil Engineer or his approved representative The area requested for clearance for each individual permit shall be limited to a maximum of two (2) weeks production for an individual permit. The contractor will be given assistance, by the government, in the execution of the initial Work Clearance Request. Thereafter, government assistance will be limited to an as-needed basis in the event of unusual circumstances. It will be the contractor's responsibility to coordinate the completion of the necessary AF Form 103 and arrange to have existing utilities located as indicated on the completed form prior to the beginning of digging operations in the individual areas. This coordination is anticipated to take approximately three (3) working days to complete per request and may require coordination with as many as twenty (20) individuals located on or near the base. A copy of the AF Form 103 is attached for reference. Any unusual delay in obtaining approval from any particular organization will be reported immediately to the contracting officer’s representative for assistance.
9.2. Utility Staking Requirements: The contractors shall layout and mark his intended area(s) of excavation before calling for field coordination by utility personnel. This shall be done a minimum of five working days in advance of when digging is expected to begin. Once all responsible utility representatives have field located crossover and/or interference points between the proposed work and existing utilities and signed off on the digging permit to signify completion of the field coordination of the digging permit, then digging in the area represented by the digging permit may begin. Any utility service markers or markings established by the utility representatives must be maintained by the contractor through the completion of the digging operations.
9.3. Digging Operations: Digging near established interference or crossover points shall be done by hand, five (5) feet either side of the point along the intended route, in order to prevent disturbing the existing utility. If the existing utility is uncovered in the new excavation, it shall be protected from damage and movement while in the open excavation and during backfill. The contractor shall be responsible for the repairs and associated costs for repairs of any utility damaged by construction, whose location was made known to the contractor.

11. SAMPLING AND TESTING: All quality control sampling and testing shall be the responsibility of the contractor. Sampling and testing shall be performed by an approved commercial testing laboratory. Copies of all test results shall be submitted to the contracting officer.
12. IDENTIFICATION OF EMPLOYEES
12.1. Entry Authority List (EAL): These lists are kept at the gates to verify employment. The following guidelines apply:
1. On Base: Original EAL and two (2) copies to the Contracting Office is required
2. The EAL must be in alphabetical order, on company letterhead, containing the complete address and phone number of the company, signed by an authorized company representative, and in the following format:

DO NOT combine your EAL with the subcontractors’ EAL. Each company/subcontractor must have a separate EAL.
3. Additions/Deletions of Personnel: When personnel are to be added or deleted, the entire EAL shall be re-accomplished and submitted to the Contracting Officer.
4. Escorts: An asterisk (*) on the EAL by key personnel such as a foreman or supervisor will allow temporary entry of supply deliveries / pickup / repair. The escorts must accompany these personnel the duration of time on the installation. A maximum of three (3) key personnel per contractor is allowed. A phone number where the escorts can be reached must be included on the EAL.
5. Two (2) days are required for processing of EALs.
12.2. Air Force Form 75, Visitors Pass (AF 75): The AF 75 is required for all employees and vehicles, The AF 75 has two parts, one for placement on the driver's side of the dashboard (must be visible at all times), the other part is issued to the individual and must be on his/her person at all times. Each employee must obtain this pass by presenting his driver’s license at the Visitor Control Center, located outside the main gate. If any of the above identification is not on the employee’s person, he/she will be escorted from the base. It is recommended that a magnetic sign be placed on the left door of the vehicles that do not have a company logo. Periodic spot checks of drivers’ licenses and AF 75's will be conducted on a random basis. It is the responsibility of each employee to turn the pass in the depository (on driver's side of the gate as you depart the base) after completion of the contract(s).
13. CONTRACTORS STORAGE AREA: The contractor will be authorized the use of an area adequate for his materials and equipment storage. The contractor shall set up and maintain his operation within the area in an orderly manner and upon completion of the project, shall restore the premises to its original condition free of all excess materials, debris and excavations or embankments resulting from his operations. The contractor will also be responsible for his own equipment security.
14. TEMPORARY STAGING AREA: Not used
15. CONTRACTOR'S HAUL ROUTES: Access to the project will be by Hursey Gate during the week (Monday through Friday) thence over the haul route indicated on attached documents. The designated routes shall be used by both loaded and unloaded vehicles.
16. TEMPORARY TOILETS: Unless otherwise directed by the contract documents, the contractor shall provide and maintain sanitary temporary toilets for all work accomplished under the project. These toilets shall be located as directed by the contracting officer. The temporary toilets shall be portable, enclosed and weatherproofed and kept in a sanitary condition at all times.
17. CONSTRUCTION SIGNAGE: Signage must be provided by the Contractor.
18. EXISTING CONDITIONS: The plans represent the best available information at the time they were prepared. While every effort has been made to correctly show all major existing items which will affect the work, it is recommended that prospective bidders examine the site prior to submitting a bid. No additional payment will be made for extra work required by obstacles which were in place and clearly visible at the time of bidding.
19. WORK SCHEDULE. Working hours for the contractor will normally be between the hours of 6:30 a.m. and 5:00 p.m. excluding Saturdays, Sundays and federal holidays. If the contractor desires to work during periods other than above, additional government inspection forces may be required. The contractor must make his / her request to the contracting officer three days in advance of his / her intention to work during other periods to allow assignment of additional inspection personnel. The contracting officer shall determine, at his discretion, whether such inspection forces are available and whether work will be allowed on a non-scheduled day. The contracting officer may authorize the contractor to perform work during periods other than normal duty hours / days; however, if inspectors are required to perform in excess of their normal duty hour / days solely for the benefit of the contractor, the actual cost of inspection at overtime rates will be charged to the contractor. These adjustments to the contract price may be made periodically as directed by the contracting officer.
20. DAILY CLEAN-UP: Not used
21. CONTRACTOR DAMAGES: Areas not identified to be removed under this contract that are disturbed or damaged during construction shall be removed and replaced at no additional cost to the Government.
