PERFORMANCE WORK STATEMENT

For

673 Force Support Squadron Civilian Personnel Furniture

Joint Base Elmendorf-Richardson (JBER) Alaska

1. DESCRIPTION OF SERVICES: Vendor shall provide all tools, equipment, and manpower. Vendor shall remove the current office furniture in building 8517 room 208, suite 200, suite 200 rooms A, B, C, D, E, F, and G. Vendor will relocate and assemble current office furniture at various locations on Joint Base Elmendorf-Richardson (JBER) as directed by Civilian Personnel Office. Remaining current office furniture will be placed into storage or disposed of properly as directed by Civilian Personnel Office. Vendor will deliver and assemble new office furniture on date specified by Civilian Personnel Office. Vendor will dispose of any trash associated with furniture. Civilian Personnel Office reserves the right of final approval of products selected.

2. GENERAL CHARACTERISTICS: The new office furniture shall have the following:

a. Furniture will be placed in locations as directed by Civilian Personnel staff.

b. Furniture will be assembled by Vendor and in working order.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES

a. None.

4. GENERAL

a. Hours of Operation. Normal hours of operation are Monday through Friday 0730-1630. Office will be closed on all Federal Holidays and scheduled down days. (See attachment of scheduled down days)

b. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

c. Security Requirements. The Vendor and all employees will comply with all JBER security requirements imposed by the Installation Commander at all times while on the premises. Vendor will obtain all necessary passes, decals, badges or other items required for access to perform furniture installation services at JBER.

d. Hazardous Material Identification. Safety Data Sheet (SDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. SDS must be submitted by the Vendor upon contract award and updated as inventory is added or deleted. Failure to provide SDS or certificate when requested could result in the Vendor being considered non-responsive and may result in termination of the contract. Vendor shall provide an inventory of materials being brought onto JBER. If applicable, SDSs will be readily available to personnel working with hazardous materials.

e. HAZMART. Not applicable.
