STATEMENT OF WORK
Concrete work for Buildings 4251 E, 4251 W, 11550, 5250, 5857, 1102, 7377, 8324, 5257 Work Order #’s 34657, 34408, 35592, 32923, 35576, 34815, 33742, 33204, X4518, & 35576
PROJECT INFORMATION
1.1 PROJECT LOCATION: Project 34657 shall be completed as a routine requirement at building 4251, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
PROJECT DESCRIPTION: Remove and replace (R/R) concrete steps and landing at the east entrance of bldg. 4251 “Arctic Issue main entrance” on JBER-E. Repair supports for canopy and replace handrails to meet code compliance for general industry stairs. 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
1.2 PROJECT LOCATION: Project 34408 shall be completed as a routine requirement at building 4251, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
PROJECT DESCRIPTION: Remove and replace (R/R) concrete steps and repair landing at the west entrance of bldg. 4251 on JBER-E. Replace handrails to meet code compliance for general industry stairs with Galvanized hand railing. 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
1.3 PROJECT LOCATION: Project 35592 shall be completed as a routine requirement at building 11550, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours 0700-1700.
PROJECT DESCRIPTION: Remove existing dilapidated sidewalk on the north east side of the 3 Wing Headquarters building. 11550 and replace with 8’ wide “arctic sidewalk and curb used for egress leading to and from the rear main entrance from Slammer Ave.
1.4 PROJECT LOCATION: Project shall be completed as a routine requirement at building 5250, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
PROJECT DESCRIPTION: Remove and replace (R/R) 1 set of concrete steps and landing on the north side entrance of bldg. 5250 “center north entrance” see attached drawing for exact location on JBER-E. Repair/replace handrails with galvanized hand rails to meet code compliance for general industry stairs. 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
1.5 PROJECT LOCATION: Project shall be completed as a routine requirement at building 5257, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
PROJECT DESCRIPTION: Remove and replace (R/R) 3 sets of concrete steps and landings on the south side entrances of bldg. 5257 “west, center & east entrances” see attached drawing for exact location on JBER-E. Repair/replace handrails with galvanized hand rails to meet code compliance for general industry stairs. 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
1.6 PROJECT LOCATION: Project 34815 shall be completed as a routine requirement at building 1102, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
UNIQUE REQUIREMENT: The concrete landing at the top of the steps is covered by a canopy. This canopy must be repaired or replaced along with support posts, metal bolts and tie down plates. Upon completion of Demo work, 773 CES must perform a site visit to ensure demo work is fully completed. Also, 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
PROJECT DESCRIPTION: Remove and replacement (R/R) concrete steps and landing at the west entrance of bldg. 1102 NCOA on JBER-R. And repair or replace the canopy that covers the landing.
1.7 PROJECT LOCATION: Project 33742 shall be completed as a routine requirement at building 7377, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours 0700-1700.
PROJECT DESCRIPTION: Remove and Replace dilapidated sidewalks on the East side of the Denali Child Development Center Bldg. 7377 and along Luke Ave. from the intersection of 15th St. The sidewalks are located on the East side of the Bldg. 7377. 2 lateral sidewalks used for egress leading to the main sidewalk from running along Luke Ave. 570’ from the intersection of 15th St.
1.8 PROJECT LOCATION: Project 33204 shall be completed as a routine requirement at building 8324, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours 0700-1700.
PROJECT DESCRIPTION: Request a 12’ X 30” concrete pad to support the 8K gallon tank and fluid capacity plus electrical capability (there is electrical service in the area) to operate a servicing pump.
1.9 PROJECT LOCATION: Project shall be completed as a routine requirement at building 5257, on Joint Base Elmendorf-Richardson, Alaska. Project hours are standard work hours.
PROJECT DESCRIPTION: Remove and replace (R/R) 3 sets of concrete steps and landings on the south side entrances of bldg 5257 “west, center & east entrances” see attached drawing for exact location on JBER-E. Repair/replace handrails with galvanized hand rails to meet code compliance for general industry stairs. 773 CES must perform a site visit upon completion of the renovation to ensure all work is done in an acceptable manner.
GENERAL REQUIREMENTS
1.10 WORKMANSHIP: All work shall be accomplished in accordance with all applicable building codes and Municipality of Anchorage Standard Specifications; Portland cement and Concrete Division 30. Contractor shall field verify all measurements, quantities, or value stated by the Government.
1.11 WORKSITE: The contractor shall limit the work to the areas indicated by the government. The contractor shall make every effort not to disturb or obstruct portions of the facilities still occupied by the government. The contractor shall make efforts to minimize interference to Government operations. The contractor shall coordinate and schedule work with the facility manager prior to the start of any operation. The Government will provide a dump site on JBER for concrete only. The contractor shall dispose of all other construction debris off base at an approved disposal facility.
1.12 SITE SECURITY AND PROTECTION: The contractor shall make every effort to secure the work site, allowing only authorized personnel to enter the work site. The contractor shall protect the site from damage from weather, vandalism, and/or theft.
1.13 DAMAGE TO EXISTING SITE CONDITIONS: The contractor shall be responsible to repair any and all damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the government.
1.14 HEALTH/SAFETY: The contractor shall be responsible for compliance with the regulations of the United Sates Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA) and Air Force Occupational and Environmental Safety, Fire Protection and Health Program (AFOSH) Publication. The contractor shall maintain a clean and debris free site at all times. The contractor shall keep driveways and entrances into and around the work site clear at all times. The contractor shall not use driveways or entrances for employee parking or storage of materials unless otherwise stated. The contractor shall minimize storage of materials and equipment on site.
1.15 Permits: Contractor will be required to obtain a Base Civil Engineering Work Clearance Request, 673 ABW Form 3 (dig permit) for all areas prior to work beginning. Contractor shall maintain control of all vehicle and foot traffic in and around the work zone at all times. Contractor shall supply and maintain all vehicle and foot traffic control devices, i.e. signage.
1.16 GREEN PROCURMENT: Contractor must incorporate “green procurement” principles into the design of their projects. Specifically, consult the EPA’s web site at www.epa.gov/cpg/products.htm for a listing of the products that have been mandated as green procurement requirements for purchases.
SECTION 2: DESCRIPTION OF WORK AND SPECIAL INSTRUCTIONS
2.1 General Requirements (Div. 1)
2.1.1. Remove and replace concrete steps, stem walls and landing to the east entrance of building 4251 on JBER. And repair supports for the canopy that covers the entrance and handrails for steps to include support posts, metal bolts and tie down plates.
2.1.2. Contractor shall remove all unsuitable materials up to 1’ in depth of the sub base and subgrade in all locations and backfill with approved select materials and compact to at least ninety-five percent (95%) of maximum density (AASHTO T-180 D) for preparation of Portland Concrete (PC) steps and landing construction.
2.1.3. Contractor will set all forms to the lines, grade and dimensions to match existing steps and landing. Forms shall be thoroughly braced and secure to resist deformation or placement under load and installed to permit easy removal without hammering or prying against the fresh concrete.
2.1.4. Contractor will use a 6 sack TYPE I epoxy coated #5 rebar, steel reinforced PC when replacing steps. The Contractor shall use 5.5 sac fiber reinforced PC when replacing sidewalks per the SOW. All PC will meet or exceeds industry standards.
2.1.5. Contractor shall apply membrane curing compound to all exposed surfaces immediately after finishing.
2.1.6. Contractor shall protect all newly placed concrete from damage of any kind to prevent disfigurement during the curing period. Damaged concrete shall be repaired or replaced to Government representative’s satisfaction at no additional cost.
2.1.7. Contractor will adhere to all concrete practices when placing concrete prescribed in Municipality of Anchorage Standard Specifications, Standard Construction Specification for Portland Cement Concrete Division 30.
2.1.8. Contractor shall backfill excavated and newly constructed sidewalks with a minimum of 4” top soil, leveled and compacted with surrounding area and/or sloped, hydro-seed and fertilize.
2.1.9. Once the project is completed the Contractor shall haul away and dispose of all construction debris from work area to an approved dump site.
